The system () includes a client computer () having an input data map () for receiving and storing care-receiver data () including predetermined, critical categories of care-receiver information. A system processor () processes the care-receiver data () through knowledge management software () to evaluate and select from the care-receiver data () at least physical functional status (), cognitive and sensory status (), prospective functional status (), living environment status (), and long-term care resource status abstractions (). A meta needs-resource weighting engine () assigns care-receiver specific values (A) to the care-receiver data abstractions (). A dynamic data base () allocates values to the weighted care-receiver data abstractions (). A report generator () produces a care plan options report () for the care receiver.
