Abstract

A medical test form having a biological sample receiving material mounted thereto. In one embodiment, the sample receiving material is incorporated in a sample receiving sub-assembly that is secured to a multi-panel carrier substrate that has instructions and/or patient information printed thereon. In another embodiment, the sample receiving material provided on one panel of a V-fold mailer. If necessary or desirable one or more additional panels can be secured to the panels of the V-fold mailer.
