Abstract

A valve, such as a ball valve, which is turned 90.degree. between a fully open and a fully closed position. A lock-out plate is secured on the valve stem and has at least one lock-out opening positioned to mate with a lock-out opening in either an indexing plate or the valve body, such that the valve may be securely locked in a fully closed position. An indexing plate may also be used to fit on the valve stem and cooperate with flat surfaces on the valve stem for limiting the turning movement of the valve to fully open and fully closed positions.
