Abstract
A form assembly for casting a lining wall in an existing manhole includes a cylindrical lower form assembly, a circular horizontal form assembly, and a cylindrical upper form assembly. All three assemblies are formed of smaller segments which are assembled together within the manhole and which can be disassembled after the manhole has been repaired.
