Abstract

The form is constructed of a face sheet, one or more underlying sheets which are die cut into a series of strip portions and a back sheet. The face sheet is divided into an ordered array of sets of designated spaces with each set overlying a strip portion of an underlying sheet so that information recorded on the face sheet is simultaneously recorded on the underlying strips. In addition, the designated spaces may be arranged to impart a cross-footing ability. The underlying strips are removable for use in other processes while the face sheet is retained for control and/or recording purposes. These strips may also contain serial numbers or other coded indicia coded to the face sheet.
