A tray for positioning, in a fixed positional relationship, a medical vialtogether with a vial adapteris disclosed. A tray memberincludes a vial accommodation cavityand a vial adapter accommodation cavity, each comprises retaining membersconfigured to position the vial adapterand the vialspaced apart from each other and in axial alignment in an intermediate position. The retaining membersare further configured to guide a relative axial movement of the vialand vial adapterfrom the intermediate position to a transfer position while maintaining the axial alignment of the vialwith the vial adapter. In the transfer position the vial adapteris locked to the vialand a piercing mandrelpierces a vial stopperfor liquid transfer.
