The present disclosure pertains to a system that facilitates transitions between sleep states and/or stages during a sleep session of a subject. The system facilitates transitions between sleep states and/or stages by monitoring the brain activity of a subject and providing sensory stimulation to guide brain activity parameters into target ranges.
