Light sources are incorporated into a guidewire for enabling the ability to render light therapy be added to catheters that do not have that capability. In one exemplary embodiment, a solid guidewire includes a conductive core, and light sources are added to compartments formed in a distal end of the guidewire. In another exemplary embodiment, a light source array is included in a distal end of a hollow guidewire. A plurality of openings are formed into the walls of the hollow guidewire surrounding the array, enabling light to pass through the openings. Conductors extend from the array though the hollow center of the guidewire, to a proximal end of the hollow guidewire. The hollow guidewire can be coated with a conductive material, so that the coating on the guidewire serves as a conductor.

Light generating guide wire for intravascular use

A light transmission system to provide photodynamic treatment to a patient includes a single use integrated control module and catheter assembly having a plurality of light emitting diodes (LEDs) to transmit light toward target cells within a patient. The integrated light catheter and control module are used in combination with a light activated drug. Selected operating parameters may be programmed into the control module, or it may be wirelessly programmable in situ prior to use to allow user flexibility to tailor treatment for a particular patient or condition. Among the features that prevent reuse are that the control module lacks access to recharge the power source, and it may include a deactivation module that destroys circuitry or software when triggered. To prevent patient interference when in use, the control module may also be configured to selectively deactivate.

A substantially thermally sealed storage container includes an outer assembly, including one or more sections of ultra efficient insulation material substantially defining at least one thermally sealed storage region, and an inner assembly, including at least one heat sink unit within the at least one thermally sealed storage region, and at least one stored material dispenser unit, wherein the at least one stored material dispenser unit includes one or more interlocks.

Substantially thermally sealed containers including flexible connectors joining an aperture in the exterior of the container to an aperture in a substantially thermally sealed storage region within the container are described. The flexible connectors include a duct forming an elongated thermal pathway between the exterior of the container and the substantially thermally sealed storage region within the container. The flexible connectors include compression units mated to each end of the duct and a plurality of compression strands connected between the compression units.

Temperature-stabilized storage systems with flexible connectors

A method of manufacture and medical apparatus that provides an apparatus useful in illuminating at least a portion of a lumen of a body. The apparatus includes an elongated flexible member and a polymer encasement portion encasing a plurality of light emitters. The light emitters may be electrically coupled to one another without the use of wire bonds, and in some embodiments may be coupled without intervening electrical paths or traces. A maximum cross-sectional dimension of the polymer encasement portion may be less than twice a dimension of one of the light emitters. In some embodiments the maximum cross-sectional dimension is less than or equal to the sum of the dimension of one of the light emitters and a marginal dimension by which an outer portion of the polymer encasement portion extends beyond the light emitter. Light emitters may be arranged linearly, helically or in partially overlapping back-to-back relation.

Medical apparatus employing flexible light structures

A combined imaging/HIFU probe includes an imaging scan head, a HIFU transducer, and an outlet port that delivers a flow of fluid across the HIFU transducer. At least a portion of the body cavity is filled with fluid in which the probe is immersed. The fluid provides a coupling for transmission of ultrasound energy between the probe and the patient. A flow of fluid may also be used to flush obstructions from an area of tissue near the HIFU transducer. Further described herein is a cuff to help retain fluid in the body cavity, a regulator to regulate fluid flow with respect to the body cavity according to a desired fluid pressure, and a cover for the HIFU transducer that has at least one perforation defined therethrough to allow fluid to flow through the cover. Further disclosed herein are methods of deploying a combined imaging/HIFU probe in a body cavity.

Methods and apparatus for engagement and coupling of an intracavitory imaging and high intensity focused ultrasound probe

Light generating devices for illuminating portions of vascular tissue, to render photodynamic therapy. In one embodiment, a light source array preferably including a plurality of light emitting diodes, a focusing lens, and a light diffusing element are included in a distal end of a catheter. A balloon is optionally provided to interrupt blood flow that can block the transmission of light, and to center the apparatus in a blood vessel. Optical fibers optionally direct light from the light source to the diffusing element. The light source array can have a radial or linear configuration and can produce more than one wavelength of light for activating different photoreactive agents. Linear light source elements are particularly useful to treat elongate portions of tissue in a vessel. One embodiment intended for use with a conventional balloon catheter integrates light sources into a guidewire.

A rotational ablation atherectomy device including a flexible drive shaft and a compressible burr that may be inserted and extracted from a patient using a catheter having a diameter that is smaller than the operational diameter of the burr. In one embodiment, the burr includes a nose portion coupled to the drive shaft and one or more flexible abrasive disks disposed rearwardly from the nose portion. The flexible disks are foldable to be slidably received within a catheter. In another embodiment, the burr includes a support member coupled to the drive shaft, the support member having a resilient panel that spirals outwardly, forming a generally cylindrical ablation surface. The flexible panel can be elastically urged toward the support member and slidably inserted into the catheter. In a third embodiment, the burr includes a plurality of struts that are coupled to the drive shaft. An elastically compressible body disposed between the struts permits the struts to flex inwardly to reduce the burr diameter. In another embodiment, the burr includes a plurality of flexible wires attached at proximal and distal ends to the drive shaft. An abrasive sheath is disposed over the wires. The wires can be bent inwardly to compress the burr and re-expanded by rotation of the burr. In another embodiment, the burr comprises a nose portion and a resilient shell having a compressible, larger diameter abrasive section disposed at the proximal end of the nose portion.

Compressible atherectomy burr

An intravascular device and methods for forming multiple percutaneous myocardial revascularization (PMR) holes in a heart chamber wall simultaneously. One device includes a basket formed of flexible arms carrying cutting probes over their length. The basket arms are outwardly arcuately biased so as to assume an outwardly bowed, arcuate shape when unconstrained. The device includes an inner shaft distally secured to a proximal portion of the basket and slidably disposed within an outer shaft. The inner shaft and collapsed basket can be retracted within the outer shaft, delivered intravascularly to the left ventricle, and distally advanced, forcing the basket to assume the bowed shape. Radio frequency current supplied to the electrical cutting probes burn holes into the ventricle wall and myocardium. One embodiment has high pressure fluid jet cutting means. Another embodiment uses a basket as an anchor to position a steerable cutting probe. Yet another embodiment includes a brush formed of multiple, arcuate, outwardly splayed electrodes that can contact heart chamber walls once advanced distally from a constraining outer shaft.

Percutaneous myocardial revascularization basket delivery system and radiofrequency therapeutic device

A combination catheter includes an ultrasound transducer and RF ablation electrode. The ultrasound transducer transmits ultrasound signals into and receives echo signals from a vessel. The echo signals are processed and used to produce an image of the tissue surrounding the catheter. A driveshaft rotates the ultrasound transducer to obtain a 360&deg; view of the vessel wall. At the distal end of the driveshaft is an electrode. An RF generator is electrically coupled to the driveshaft to deliver RF energy to the electrode at the distal end of the driveshaft to ablate occluding material in the vessel. The electrode may have a variety of tip shapes including concave, roughened, or expandable configurations, depending on the size of the vessel and composition of the occluding material to be ablated.

RF ablation and ultrasound catheter for crossing chronic total occlusions

A system for ablating material in vein grafts includes an ablation burr that is rotated by a driveshaft. The ablation burr preferably includes one or more channels, blades or other mechanisms that direct ablated material and liquid proximally and/or outwardly against a vessel wall. Aspiration is used to remove ablated material and liquid from the treatment area. Finally, methods for treating vein grafts and original native arteries are disclosed.

Method and apparatus for treating vein graft lesions

A system for recanalizing an occluded blood vessel including a centering catheter employed to center an ablative guide wire within the blood vessel as the guide wire traverses the occlusion. The centering catheter includes a catheter body with an operative lumen through which the ablative guide wire is slidingly disposed. The centering catheter further includes a distally disposed centering mechanism that, when activated, centers the ablative guide wire within the blood vessel as it traverses the occlusion. The centering mechanism can comprise various embodiments including a single inflatable balloon or segmented inflatable balloon, which is in fluid communication with an inflation lumen. An airless preparation lumen may be disposed within the inflation lumen for ease of centering catheter preparation. The ablative guide wire includes insulation that is preferably formed of heat shrink tubing, which is stretched prior or concurrently with the heating process. The ablative guide wire includes a distal ablation tip having a non-traumatic structure and at least one discontinuous feature for creating high current densities for more efficient tissue ablation.
