A subcutaneous implantable cardioverter-defibrillator (S-ICD) comprising shocking electrodes configured to reduce the defibrillation threshold. The S-ICD may include a canister housing a source of electrical energy, a capacitor, and operational circuitry that senses heart rhythms and an electrode and lead assembly. The electrode and lead assembly may comprise a lead, at least one sensing electrode, and at least one shocking electrode. The at least one shocking electrode may extend over a length in the range of 50 to 110 millimeters and a width in the range of 1 to 40 millimeters.

Electrode designs in implantable defibrillator systems

Aspects herein include an implantable medical device, such as an implantable cardiac rhythm management device. The implantable medical device can include a housing, control circuitry disposed within the housing, and telemetry circuitry in electrical communication with the control circuitry. The control circuitry can be configured to execute a post-procedural system check procedure after the expiration of a preselected first time period. The first time period can be greater than or equal to 0.5 hours and less than or equal to 48 hours. In various aspects, the post-procedural system check procedure can include measuring diagnostic properties for one or more electrodes of an electrical stimulation lead attached to the implantable medical device. The first time period can begin to elapse after detection of a triggering event or a command by a system operator.

Implantable medical devices and methods including post-procedural system diagnostics

A cardiac rhythm management system includes techniques for computing an indicated pacing interval, AV delay, or other timing interval. In one embodiment, a variable indicated pacing interval is computed based at least in part on an underlying intrinsic heart rate. The indicated pacing interval is used to time the delivery of biventricular coordination therapy even when ventricular heart rates are irregular, such as in the presence of atrial fibrillation. In another embodiment, a variable filter indicated AV interval is computed based at least in part on an underlying intrinsic AV interval. The indicated AV interval is used to time the delivery of atrial tracking biventricular coordination therapy when atrial heart rhythms are not arrhythmic. Other indicated timing intervals may be similarly determined. The indicated pacing interval, AV delay, or other timing interval can also be used in combination with a sensor indicated rate indicator.

A weighted averager such as an infinite impulse response (IIR) filter adjusts the first indicated pacing interval for sensed beats and differently adjusts the first indicated pacing interval for paced beats. The system regularizes ventricular rhythms by pacing the ventricle, but inhibits pacing when the ventricular rhythms are stable.

A method of determining the cause of upper rate limit behavior in a dual chamber pacemaker incorporating a system for distinguishing Wenckebach episodes from pacemaker mediated tachycardia episodes based on variations in VA intervals is disclosed which provides for adjusting the MTR upward in response to a threshold frequency of incidences of Wenckebach.

Determination of pacemaker wenckebach and adjustment of upper rate limit

Exertion levels of a patient are measured by monitoring signals from adaptive-rate sensors such as an accelerometer and or a minute ventilation sensor; sensor data is collected for conversion into metabolic equivalent measurements. The data obtained can be used to evaluate patient physical activity levels and can be used to assess the patient's condition and change pacing therapy or other treatments accordingly. An automatic adjustment of the adaptive-rate pacing therapy may be based on the activity levels detected by the metabolic equivalent measurements made by the pacemaker.

Apparatus and methods for METS measurement by accelerometer and minute ventilation sensors

A dual chamber pacemaker incorporating a system for distinguishing Wenckebach episodes from pacemaker mediated tachycardia episodes based on variations in VA intervals is disclosed which provides for adjusting the MTR upward in response to a threshold frequency of incidences of Wenckebach.

A method and apparatus for cardiac stimulation for addressing vasovagal syncope incorporates a detection algorithm in which a precipitous rate drop from a pre-existing average intrinsic heart rate value to a lower rate limit is sensed and if the patient is awake and the intrinsic rate remains below the lower rate limit for a predetermined number of beats, the patient's heart is paced at a rate that is a programmed differential above the prior average intrinsic rate for a second programmed time interval, at the conclusion of which the pacing rate is gradually decreased to the average heart rate existing prior to the sudden rate drop or to the lower rate limit.
