A surgical instrument is disclosed including a surgical device including a transducer positioned to provide vibrations along a longitudinal axis at a predetermined frequency and an end effector positioned distally from the transducer. The surgical instrument further includes a sleeve configured to receive the surgical device and a rail positioned along an interior portion of the sleeve. The surgical device includes a feature for receiving the rail. The surgical device is slidable along the rail.

Ultrasonic surgical instruments

A surgical instrument including a transducer and an end effector is disclosed. The transducer may be configured to generate an acoustic standing wave of vibratory motion along a longitudinal axis and may include a piezoelectric stack positioned along the longitudinal axis. A length of the transducer may be less than &#xbd; of the wavelength of the acoustic standing wave. The end effector may be acoustically coupled to and may extend distally from the transducer along the longitudinal axis. A sum of the length of the transducer and a length of the end effector may be an integer multiple of &#xbd; of the wavelength of the acoustic standing wave.

A surgical instrument. The surgical instrument may comprise a transducer and an end effector. The transducer may be configured to provide vibrations along a longitudinal axis at a predetermined frequency and may comprise a piezoelectric stack positioned along the longitudinal axis. The transducer also may comprise a first metallic end mass positioned along the longitudinal axis adjacent a first end of the piezoelectric stack and a second metallic end mass positioned along the longitudinal axis adjacent a second end of the piezoelectric stack. The length of the transducer may be greater than or equal to of one wavelength and less than &#xbd; of one wavelength. The end effector may be coupled to the transducer and may extend along the longitudinal axis. The length of the transducer and the end effector may be a multiple of &#xbd; of one wavelength.

A transgastric instrument provides a sterile and repeatable pathway to and through a gastric wall for the purpose of facilitating endoscopic transgastric procedures. The instrument includes at least one guide tube having a proximal end and a distal end, the distal end being shaped and dimensioned for positioning adjacent a desired location along the gastric wall. The instrument also includes a fixation mechanism at the distal end of the guide tube for selectively securing the distal end of the guide tube at a desired location along the gastric wall. An otomy creation device circumferentially smaller than the guide tube extends through the guide tube, the otomy creation device including a distal end shaped and dimensioned for cutting and dilating the gastric wall.

An endoscopic gastric reduction apparatus adapted for applying a series of pledgets to anterior and posterior gastric walls for the creation of a mattress stitch suture within the stomach includes an applier having a distal end and a proximal end. The applier includes an applier body having a suction slot shaped and dimensioned for receiving tissue therein for the application of at least one pledget housed within the suction slot for selective coupling with tissue suctioned within the suction slot.

Method and apparatus for endoscopically performing gastric reduction surgery in a single step

A surgical tool or applier facilitates laparoscopic or endoscopic implantation through a single bodily tissue lumen of an anastomotic ring device for forming a hollow rivet type of attachment between tissue lumens. In addition to forming a puncture between apposite tissue walls at the anastomosis site, the applier assists or wholly actuates the anastomotic ring device and is retracted to deploy the actuated ring device. Illumination incorporated into a distal portion of a cannula enables confirmation of deployment.

A ring applicator for applying elastic rings to anatomical tissue during surgical procedures includes an inner member with a forceps movably disposed within a middle tubular member having a distal end configured to hold a plurality of elastic rings in an expanded state and a proximal end with a plurality of longitudinally spaced steps. The middle tubular member is movably disposed in an outer tubular member, and an adjustable stop mechanism is mounted on the proximal end of the outer tubular member to limit axial movement of the outer tubular member relative to the middle tubular member to select which rings are to be ejected. A distal handle is connected with the inner member via slots in the outer and middle tubular members, and a proximal handle is pivotally connected to the middle tubular member via a slot in the outer tubular member. A pusher has a distal end movable between a latched or locked position where the distal end of the pusher protrudes into apertures in the outer and middle tubular members to prevent pivotal movement of the proximal handle and an unlatched or unlocked position where the distal end of the pusher is disengaged from the aperture in the middle tubular member to permit pivotal movement of the proximal handle in order to move the outer tubular member axially relative to the middle tubular member to eject an elastic ring. Distal portions of the outer and middle tubular members are preferably transparent with scale markings formed thereon to allow the surgeon to accurately determine the length of the tissue disposed within the middle tubular member prior to ejecting a ring. A prerelease feature is also disclosed for unlocking the pusher to allow ejection of a ring when the forceps has not been completely withdrawn in to the middle tubular member.

Ring applicator and method for applying elastic rings to anatomical tissue structures

An apparatus and method delivers an expandable body member into a uterine cavity of a uterus through a cervical opening and deploys the expandable body member in the uterine cavity. The expandable body member in a compressed state is inserted into the uterine cavity. A fluid is provided to the uterine cavity at a pressure sufficient to inflate the uterine cavity. The expandable body member expands from the compressed state to the expanded state. After expansion of the expandable body member and inflation of the uterine cavity, the fluid pressure is relieved thereby collapsing the uterine cavity about the expandable body member in the expanded state such that an endometrial layer of the uterus and a surface of the expandable body member substantially contact each other in a facially opposing relationship maximizing contact therebetween. The apparatus which performs the method includes an elongated tubular member having the expandable body member connected to a distal portion of the elongated tubular member and a fluid-providing device connected to a proximal portion of the elongated tubular member.

Apparatus and method for delivering and deploying an expandable body member in a uterine cavity

A device useful in minimally-invasive procedures involving the visualization, dissection and/or harvesting of vessels in the body includes a shaft having a handle mounted on one end and a dissecting tip on the other end. The dissecting tip optionally includes a light source for directing light transverse of the tip and structures to retain the tip in position adjacent the vessel as the tip is advanced along the vessel. Also disclosed are associated methods for transilluminating a vessel, dissecting the vessel from surrounding tissue, and transecting the vessel and side branches and removing the vessel from the body.

A system for establishing an anastomotic passageway between first and second structures within an animal, the first and second structures each having an intima-exposing opening comprises a first plate sized and configured for attachment to the opening of the first structure, the first plate including an attaching portion having means for gripping first structure at its opening, the first plate further including a coupling portion which is positioned exterior of the first structure when the first plate is attached to the first structure; a second plate sized and configured for attachment to the opening of the second structure, the second plate including an attaching portion having means for gripping the second structure at its opening, the second plate further including a coupling portion which is positioned exterior of the second structure when the second plate is attached to the second structure; and means for connecting the coupling portion of said first plate with the coupling portion of said second plate.
