A buttress is applied to an end effector of a surgical stapler. The buttress is loaded on a platform of a buttress applier cartridge. The end effector is closed upon the platform. An adhesive layer of the buttress secures the buttress to the end effector. The buttress is thus adhered to the end effector when the end effector is opened. The end effector is then actuated on tissue of a patient, thereby stapling the buttress to the tissue.

Method of applying a buttress to a surgical stapler end effector

A braided barbed suture includes a barbed monofilament insert including an elongated core having a thickness and a plurality of barbs projecting outwardly from opposite sides of the elongated core. The braided barbed suture includes a braided sheath surrounding the elongated core to form a composite core of the braided barbed suture. The composite core has a thickness and the elongated core is located in a center of the composite core. The elongated core thickness is about 6-8 mil and the composite core thickness is about 13-18 mil. A ratio of the elongated core thickness relative to the composite core thickness is between about 0.16 to 0.91. The elongated core has transition zones that form a concave profile. The braided barbed suture has a single breaking point for both the braided sheath and the barbed monofilament insert. The barbed monofilament insert has a PDS monofilament core, and the braided sheath is a VICRYL multifilament yarn braided sheath.

A device for guiding a barbed suture into a braider preferably includes an elongated body having a proximal end, a distal end, and a longitudinal axis that extends from the proximal end to the distal end of the elongated body. The elongated body includes a first channel extending along the longitudinal axis of the elongated body and having a first distal opening at the distal end of the elongated body, whereby the said first channel has a first cross-sectional area, and a second channel extending along the longitudinal axis of the elongated body and having a second distal opening at the distal end of the elongated body, whereby the second channel has a second cross-sectional area that is different than the first cross-sectional area of the first channel. The device has an elongated slot extending along the longitudinal axis of the elongated body that interconnects the first and second channels. A barbed suture is loaded into the elongated body. The barbed suture includes an elongated core having a proximal end, a distal end, a barbed section including barbs extending outwardly from the elongated core, an end effector secured to the proximal end of the elongated core, and an interconnecting segment of the elongated core that is distal to the end effector and proximal to the barbed section. The barbed section of the barbed suture is disposed within the first channel, the end effector of the barbed suture is disposed within the second channel, and the interconnecting segment of said barbed suture extends through the elongated slot.

Method of applying a buttress to a surgical stapler and effector

A stretchable buttress assembly includes a planar fabric layer and a biocompatible adhesive layer. The planar fabric layer is configured to stretch along a stretch axis. The planar fabric layer includes extensible fibers and non-extensible fibers. The extensible fibers are arranged in a repeatable pattern. The extensible fibers are oriented along respective paths that are each parallel to the stretch axis. The non-extensible fibers are arranged in a repeatable pattern and engaged with the extensible fibers. The adhesive layer is applied to the planar fabric and is configured to removably adhere the planar fabric layer to an end effector of a surgical stapler.
