An atherectomy device may include an elongated, flexible drive shaft having a distal end for insertion into a vasculature of a patient and having a proximal end opposite the distal end remaining outside the vasculature of the patient, a handle coupled to the proximal end of the drive shaft for controlling the drive shaft, and a head arranged proximate the distal end of the drive shaft wherein the head may include a middle portion having a substantially uninterrupted outer surface and substantially conically-shaped first and second end portions, wherein the middle portion and substantially conically-shaped end portions all have a bore extending therethrough adapted to frictionally engage a drive shaft.

A system, apparatus and method for maximizing efficiency of tissue removal from body passageways is provided. The system comprises a device for opening an occluded lesion, e.g., a rotational atherectomy device or angioplasty device, and a guide wire having an introducer sheath. The guide wire introducer sheath may comprise a hypo tube having columnar strength greater than that of the guide wire alone to assist the guide wire in crossing occluded lesions, wherein the sheath and guide wire are axially moveable relative to each other. The guide wire sheath may further comprise increased flexibility at its distal end to increase flexibility and/or a soft distal tip to help with steerability through the vasculature. The sheath may allow for a smaller outer diameter guide wire which, in turn, may allow for a smaller outer diameter on the occlusion-opening device, e.g., atherectomy or angioplasty device, which is slid over the pre-positioned guide wire.

System, apparatus and method for opening an occluded lesion

An emboli filter for deployment in a body lumen to capture emboli entrained in a fluid flowing through the lumen includes a flexible elongated member sized to be passed through the lumen. A filter media is carried on and substantially surrounding the elongated member. The filter media has a first end secured to the elongated member adjacent the distal end. A second end of the filter media has a periphery movable toward and away from the elongated member. Opposing internal surfaces of the filter media define a volume into which emboli may flow through the second end when the periphery is moved away from the elongated member. The emboli are trapped within the volume when the periphery is moved toward the elongated member. An actuator moves the periphery toward and way from the elongated member. The actuator includes a plurality of elastic loops. The loops are biased to an open loop configuration with the loops urging the periphery away from the elongated member. The actuator is adapted to be manipulated by an operator to urge the loops against the bias to a closed position permitting movement of the periphery toward the elongated member without interference from the loops.

A roll-formed, guidewire extension system for connecting an extension wire to a guidewire is disclosed. The system comprises a connector or sleeve, such as a turnbuckle, having a tubular outer body, a guidewire retention means, an extension wire retention means, a guidewire and an extension wire. A sleeve or turnbuckle nut is produced by rolling a portion of a formable hollow tubular workpiece upon an elongated projection with the projection being disposed at an angle with respect to the axis of the workpiece. This process is repeated on the remaining part of the workpiece to create external threads in the sleeve of opposite pitch. The roll-formed external beads or dents in the sleeve create threads in the interior of the sleeve, the threads running in opposite directions and meeting at approximately the middle of the sleeve. The guidewire and the extension wire engagement means are preferably helically wound wires which are connected e.g., brazed to reduced diameter portions of the guidewire/extension wire. The guidewire is connected to the extension wire by inserting the respective engagement portions into the open ends of the sleeve, and rotating the sleeve in a single direction. In a preferred practice the turnbuckle sleeve, is rotatively affixed to one or the other of the extension wires or guidewires, preferably the extension wire.

Guidewire extension system

A catheter for removing vascular obstruction(s) is disclosed. The catheter employs a configuration comprising generally coaxial, rapidly rotatable drive cable, fluid output lumens or channels, and fluid input lumens or channels. The drive cable is fixedly coupled to an elongate head so as to permit the head to be rapidly rotated by the drive cable. The head includes flats or impellers which, when rapidly rotated, generate a zone of turbulence or a vortex adjacent the vascular obstruction. The turbulence generated by the device tends to break up or dismantle the obstruction. The fluid input and output mechanisms permit fluid to be delivered to the vicinity of the obstruction and debris from the obstruction to be removed. In a preferred practice, a catheter of the invention is hollow, permitting it to be slid over a guide wire which has been steered to the site of the vascular obstruction. The distal end of the catheter, specifically the head, is steered to a location adjacent the obstruction by utilization of the guide wire. Methods for removing vascular obstructions are disclosed. Preferably, pulsatile turbulence generation is employed.
