Certain aspects of the disclosure pertain to methods and apparatus for providing positive fixation of medical components to a portion of incised pericardial tissue. Accordingly, a resilient member protrudes through an incision in the pericardium and produces a positive biasing force to adjacent pericardial tissue against a side surface of an attached body structure. The resilient member can optionally be compressed during implantation and then relaxed to thereafter provide the positive biasing force. Diverse medical components can thus be safely and reliably chronically deployed into the pericardial space, including without limitation, cardiac sensing/pacing, defibrillation and/or cardioversion electrodes, mechanical and/or metabolic sensors and the like. More than one body structure can be linked to a single medical electrical lead and the medical components can couple within and/or upon a portion of the body structure, the resilient member, and the lead in myriad configurations.

A directional brain stimulation lead assembly provides a lead body and an insulating member defining one or more windows that selectively expose portions of electrodes carried by the lead body to produce a directional stimulation current field. The lead assembly can achieve more effective localization of electrical stimulation to very small brain targets, and thereby reduce the incidence of material side effects caused by collateral stimulation of brain tissue adjoining a desired brain target. In addition, the directional lead can sense brain activity on a more localized basis.

The invention is directed to neurological lead extensions without set screws, set screw blocks, or set screw inserts. Such elimination of set screws, blocks and inserts can reduce the amount of metal and ferromagnetic material in the extension. The neurological lead extension may include a proximal end configured to couple to a neurological device and a distal end configured to couple to an implanted neurological lead. A set of electrical contacts are disposed in proximity to the distal end to provide electrical contact to an inserted end of the implanted neurological lead. Various elements and structures are described which can mechanically secure the inserted end of the implanted neurological lead to the distal end of the extension.

The present invention relates to an intramuscular medical electrical lead, a system for providing electrical stimulation or sensing using such a lead, and methods of implanting, making and using same. The lead includes at least a first fixation member disposed distally or proximally from a sensing or stimulating electrode, and may further include a second fixation member disposed at the opposite end of the electrode form the first fixation member. The fixation members permit the electrode to be reliably and fixedly secured within a patient's muscle tissue at a desired site, and to remain so secured for a predetermined suitable period of time.

The system includes a rigid hollow needle having a lumen, a flexible lead body disposed within the lumen of the needle, the lead body having a insulated coiled proximal section and an electrode section, the proximal section comprising a conductor which is coiled and insulated, the electrode section comprises a portion of the coiled conductor which is not insulated. In an alternative embodiment the electrode section features a crimp core around which a distal end of the coiled conductor which is not insulated is crimped, the rigid hollow needle is metal but which is partially covered along its outer surface with an insulation. In still further embodiments the flexible lead body has a stylet lumen therein and the lead body also has a connector pin for electrically connecting the electrical conductor to a pulse generator. Preferably this connector pin located on a proximal end of the lead body and having a diameter no greater than the inner diameter of the needle. A lead for brain target localization in functional stereotactic brain surgery, the lead having at its distal end a micro-electrode for use in recording single cell discharge patterns so as to identify functional boundaries between brain structures; and a macro-electrode for delivering test stimulation for determining target location for optimal therapeutic benefit and minimum side effects. The macro-test stimulation electrode is carried at the end of the lead length, and has a conventional surface area in the range of 1-20 mm.sup.2 for stimulating chronic stimulation conditions. The micro-electrode is positioned at the distal end of an axially aligned probe which extends distally from the macro-electrode, and has a surface area adapted to enable a single cell recording; e.g., between 0.01 and 1000 .mu.m.sup.2. A system and method for providing electrical and/or fluid treatment within a patient's brain, the system and method utilizing a feedthrough assembly made of a compressible material, and a disk-like pinch-on part, or resilient clamping part, which is secured around the feedthrough assembly to provide radially inward compressive force to reliably fix the lead after it has been positioned relative to the patient's skull. The feedthrough assembly is suitably made of a compressible biocompatible material such as silicone rubber, and has a annular receiving groove which is located so that it is accessible above the cranium after the feedthrough assembly is fixed within a burr hole in the patient's head. The pinch-on part, or clamping means is manufactured in a normally closed position, but can be opened, as by use of a forceps-type instrument, for positioning around the assembly annular groove.
