A tissue-removing catheter for removing tissue in a body lumen includes an elongate body having an axis. A handle is mounted to a proximal end portion of the elongate body. A tissue-removing element is mounted on a distal end portion of the elongate body. An inner liner is received within the elongate body and defines a guidewire lumen. An advancer is mounted on the handle and is movable relative to the housing. The inner liner is coupled to the advancer at a proximal end portion of the inner liner such that movement of the advancer causes a corresponding movement of the inner liner to exert a push force on the tissue-removing element to advance the tissue-removing element and a pull force on the tissue-removing element to retract the tissue-removing element for moving the tissue-removing element relative to the handle.

Tissue-removing catheter with a coupled inner liner

A tissue-removing catheter for removing tissue in a body lumen includes an elongate body having an axis, and proximal and distal end portions spaced apart from one another along the axis. The elongate body is sized and shaped to be received in the body lumen. A handle is mounted to the proximal end portion of the elongate body. The handle includes a housing enclosing components operable to cause rotation of the elongate body. A tissue-removing element is mounted on the distal end portion of the elongate body. An inner liner is received within the elongate body and defines a guidewire lumen. An advancer is mounted on the handle and is movable relative to the housing. A guide tube is mounted in the handle and is coupled to the advancer such that movement of the advancer relative to the housing causes movement of the guide tube.

Tissue-removing catheter with inner liner key

A tissue-removing catheter for removing tissue in a body lumen includes a catheter body assembly having an axis and proximal and distal end portions spaced apart from one another along the axis. At least a portion of the catheter body assembly is sized and shaped to be received in the body lumen. A handle is mounted to the proximal end portion of the catheter body assembly and operable to cause rotation of the catheter body assembly. The handle includes internal handle components that interface with the catheter body assembly. The internal handle components provide at least four interface locations spaced axially along the catheter body assembly. A tissue-removing element is mounted on the distal end portion of the catheter body assembly. The tissue-removing element is configured to remove the tissue as the tissue-removing element is rotated by the catheter body assembly within the body lumen.

Tissue-removing catheter

Transcatheter guidewire delivery systems, catheter assemblies and associated methods for percutaneous guidewire delivery across heart valves are disclosed herein. A catheter assembly configured in accordance herewith includes an elongated tubular component and an alignment assembly at a distal portion of the tubular component and which is adapted to be located at a target location adjacent a heart valve of a patient. In one embodiment, the alignment assembly deploys to a shape set loop configuration with a side port in an open configuration positioned to allow advancement of a guidewire to exit the catheter in an aligned path with a leaflet coaptation region of the heart valve. In another embodiment, the alignment assembly has a plurality of spaced apart side ports that a guidewire may advance therethrough and toward the heart valve. In some embodiments, a wire guide is used to align the guidewire with a selected side port.

Transcatheter guidewire delivery systems, catheter assemblies for guidewire delivery, and methods for percutaneous guidewire delivery across heart valves

A kit for intravascular implantation of an implantable medical device (IMD) within a patient includes the IMD, an elongated shaft, and a locking mandrel. The IMD comprises a fixation assembly comprising a loop and defines at least one longitudinal lumen and a port in fluid communication with the lumen. The shaft is sized to traverse a vasculature of the patient. The port is sized to receive at least a portion of the loop. The locking mandrel is configured to be positioned within the at least one lumen of the shaft and to pass through the loop within the lumen at the port. A reduced profile portion of the shaft defines a reduced profile with respect to at least one other portion of the shaft. At least a portion of the reduced profile portion is configured to be adjacent to the IMD when the IMD is positioned on the shaft.

Delivery catheter for implantable medical device

In a system for retrieving an implanted device, a flared inner surface of a catheter tubular sidewall may define a distal-most opening of a device receptacle; the opening has a first diameter equal to that of the receptacle, and a second diameter, coincident with a distal-most edge of the tubular sidewall, and at least 5% greater than the receptacle diameter. Alternately, a retrieval tool in sliding engagement within a lumen of a catheter includes a shaft assembly, through which a snare member passes, and which includes a collapsible spring-biased perimeter sidewall; the sidewall defines a capture member passageway approximately coaxial, and in fluid communication with a lumen of the shaft assembly. A distal-most opening of the passageway has a spring-biased diameter that is greater than that of a distal-most opening of a device receptacle of the catheter, and a collapsed diameter that is less than the receptacle distal-most opening diameter.

Interventional medical systems, catheters, and methods

A kit for implantation of an implantable medical device (IMD) comprises an elongated outer shaft, a tether, and an elongated inner shaft. The IMD comprises a fixation element comprising a looped portion. The outer shaft is sized to traverse a vasculature of the patient and defines a longitudinal lumen and a port in fluid communication with the lumen and located proximal a distal end of the outer shaft. A first portion of the tether is configured to pass through the lumen. A second portion of the tether is configured to exit the lumen through the port and pass through the looped portion of the fixation element of the IMD outside of the outer shaft. A third portion of the tether defines a looped portion of the tether. A portion of the inner shaft is configured to pass through the lumen and to pass through the looped portion of the tether.

In an interventional medical system, a tethering member of a catheter assembly and a holding member of a relatively compact implantable medical device are configured to interlock with one another. The tethering member includes an elongate shaft portion and a connector portion coupled to a distal end of the shaft portion, wherein the connector portion is configured to pass through a proximal-facing opening of a cavity defined by the device holding member, and to interlock with the holding member within the cavity. The connector portion of the tethering member includes a lumen extending longitudinally therethrough, which has a distal opening in fluid communication with the cavity of the device holding member, when the connector portion interlocks therewith. The catheter assembly further includes an elongate mandrel, which has a distal tip configured to extend within the lumen of the tethering member connector portion, being in sliding engagement therewith.

Interventional medical systems, associated assemblies and methods

Catheters including elements configured to deliver energy to nerves at or near a treatment location within a body lumen. A treatment assembly is transformable between a low-profile delivery configuration wherein a pair of electrodes are in a staggered arrangement relative to each other along the longitudinal axis, and an expanded deployed configuration wherein the pair of electrodes is aligned along an electrode axis that is orthogonal relative to the longitudinal axis.

Low profile catheter assemblies and associated systems and methods

A catheter inner assembly includes a receptacle with an interior contour configured to mate with a head and neck contour of a holding member of an implantable medical device; and a laterally facing opening of the receptacle has a profile matching a longitudinal profile of the head and neck. Thus, the opening allows only a properly oriented passage of the holding member therethrough, the passage being without deformation of holding member or opening. The inner assembly further includes a lock and release member useful for securing the device to the catheter, when the holding member has been passed through the receptacle's opening to mate with the receptacle's interior contour. The lock and release member may include an arcuate sidewall moveable between a first location, at which the sidewall overlays the laterally facing opening, and a second location, at which the sidewall is located proximal to the opening.

A delivery system for an implantable medical device includes a tool and a tethering member extending side-by-side within an inner shaft thereof; the tool extends within a first lumen, being in sliding engagement therein, and includes a distal end coupling feature that protrudes from a distal end of the inner shaft; and the tethering member has a first segment extending within a second lumen, and a second segment extending from the first segment and distally from the distal end of the inner shaft to an end of the tethering member, which is configured to engage with a holding member of the device, and with which the coupling feature of the tool is configured to couple. A retainer may be joined to another end of the tethering member that protrudes from a proximal port of the system.

Delivery systems for implantable medical devices, and associated tethering assemblies and methods

A shaft of an interventional medical system catheter has a distal end formed by opposing elastically deformable retention features that protrude into a lumen of the shaft, and that define an expandable distal opening of the lumen. The retention features secure an implantable medical device to the catheter by an interlocking engagement within a gap defined by a necked-in portion of a device holding member. Each retention feature includes a distal-facing surface defining the distal opening, and, when an elongate release member of the catheter, which extends within the shaft lumen, applies a push force against proximal-facing surfaces of the features, the distal opening expands from a constricted to an open configuration, wherein the open configuration allows passage of the device holding member through the distal opening, and the constricted configuration allows for the interlocking engagement with the device.

Interventional medical systems and associated methods

The disclosure describes techniques and systems for delivering an implantable medical device. In one example, an implantable medical device (IMD) delivery system may include an elongated member comprising a first distal end configured to mate with the IMD, a resilient member disposed along at least a portion of the elongated member, a housing configured to accept a first proximal end of the elongated member and a second proximal end of the resilient member, a rotation control mechanism wherein user movement of the rotation control mechanism causes rotation of the elongated member with respect to the housing and a fixation element of the IMD into tissue, and a deflection control mechanism wherein user movement of the deflection control mechanism causes longitudinal displacement of the resilient member along a longitudinal axis of the elongated member and the housing resulting in angular deflection of the first distal end of the elongated member.

Medical device delivery system

An engagement subassembly of a catheter, for retrieving an implanted medical device, may include at least three segments or, alternately, capture members being spring biased outward from a longitudinal axis of the subassembly, wherein a device receptacle of the catheter forces the capture members/segments, against the spring bias thereof, toward the axis. Each capture member includes a spring-biased wire and a grip, which is located between proximal and distal portions of the corresponding wire, and which is configured to interlock within a gap between a device attachment feature and housing; a length of each wire distal portion may be approximately equal to that of the device housing. An interlocking edge of each segment interlocks within the gap between the attachment feature and housing, when a distal-facing surface of each segment, which tapers outward from the axis in a distal direction, comes into confronting engagement with the device housing.
