A biomechanical optimization (BMO) prosthetic implant utilizes a thin cross-section of metallic material that is conformable. Preferably, the BMO prosthetic implant is conformable both at the time of implant in response to manipulation and fixation by the surgeon, as well as during the life of the implant in response to stresses and loads experienced by the implant and thereby communicated and responded to by living bone tissue. For most metallic alloys, the BMO prosthetic implant will have an effective cross-sectional thickness of 4 mm or less, and preferably 3 mm or less. In one embodiment, the BMO prosthetic implant is provided with one or more fins extending from the fixation surface(s) of the implant which preferably includes retaining structures, such as cross-pinned apertures or T-shaped edge ridge.

Method and apparatus for femoral and tibial resection

A cutting tool to be utilized in the resection or removal of bone tissue from patients includes a handle that tensions a wire or cable-like cutting member with a small diameter between at least two features on the handle to present a thin cutting profile. The design of the cutting tool includes features that protect against soft tissue damage and minimize the incision size necessary to utilize the tool. Some embodiments feature details of the cutting tool that interface with a surgical cutting guide system. Other embodiments describe a cutting tool with a selectively changeable length of the cutting profile of the wire cutting member. In one embodiment, the wire cutting member of the cutting tool is energized by mechanical energy in the form of a unidirectional rotation of the cutting member, a mechanical vibration of the cutting member, or an oscillating movement of the wire cutting member.

Method and apparatus for wireplasty bone resection

Systems for use in total knee arthroplasty include a cutting guide having only a single slot. The single slot is configured to be positioned adjacent an anterior side of the medial compartment of the knee joint with no portion positioned adjacent an anterior side of the lateral compartment of the knee joint. The single slot is adapted to guide an oscillating saw blade across the knee bone from the medial compartment to resect at least a portion of the lateral compartment. The system also includes a total knee arthroplasty implant adapted to be implanted on the resected surface.

A system for use in a total knee arthroplasty procedure can include a first cutting guide, an alignment guide and a femoral prosthetic implant. The first cutting guide can include at least one planar guide surface adapted to guide a cutting tool to cut femur. The alignment guide can contact the femur and provide for markings to be made in the femur that dictate a location and orientation of the first cutting guide. The implant is adapted to be implanted on the resected femur. The implant can have an inwardly protruding structure to provide medial-lateral fixation stability, and can have an anterior fixation path that diverges proximally away from a long axis of the femur and a posterior fixation path that converges proximally towards the long axis such that the posterior fixation path and anterior fixation path converge toward one another proximally of a distal contact surface.

Alignment guides, cutting guides, cutting tools and soft tissue management techniques for profile based resection (PBR) arthroplasty facilitate intraoperative and postoperative efficacy and ease of use. In one embodiment, a manual alignment guide is provided that permits less invasive incisions by providing soft tissue accommodating contours or reliefs. In another embodiment, a single medial drill guide plate is used for the PBR arthroplasty.

Cutting guides, cutting tools, and soft tissue management techniques permit the use of pivoting guide surfaces to facilitate in controlling the sweep or arc of a cutting tool used in connection with resection and arthroplasty procedures. In accordance with one embodiment, a guide structure is provided with one or more guide pivot aperture(s) and one or more guide pivot reference surface(s) that mate with a bushing assembly controlling a cutting tool. The bushing assembly possesses a bushing reference plan which mates with the pivot reference surface(s) of the guide structure and a bushing pivot pin which mates with the guide pivot aperture(s) of the guide structure. In one embodiment, a bushing guide lumen is operably coupled to the guide structure for articulated and/or axial guidance of the cutting tool.

Methods and apparatus for pivotable guide surfaces for arthroplasty

A prosthetic implant utilizes lateral retaining structures as part of the interior surface of the implant to more effectively secure and retain the implant while reducing the overall size and mass of the implant. In one embodiment, the prosthetic implant is provided with one or more T-shaped members extending from the inner surface of the implant, with the cross-member of the T-shaped member forming the laterally retaining structure that mate with a correspondingly shaped channel formed in the bone and are inserted into that channel at one or more oversize locations along the channel. In another embodiment, the prosthetic implant is provided with one or more retentions apertures in a projection structure extending inwardly from the inner surface of the implant that are laterally secured with a force fitted cross pin inserted through the retention aperture.

Methods and apparatus for enhanced retention of prosthetic implants

Milling and drilling tools and soft tissue management techniques are provided for arthroplasty that facilitate intraoperative and postoperative efficacy and ease of use. In one embodiment, resiliently biased soft tissue protective sleeves surround a side cutting tool and are interposed along the longitudinal axis of the cutting tool, preferably adjacent each side of the bone. The soft tissue protective sleeves are biased to track along the contour of the side of the bone and prevent the cutting tool from being exposed to soft tissue during the resection. In another embodiment, a pilot drill is initially utilized to create an initial bore in the bone to be resected. The pilot drill preferably has an end cutting arrangement and a non-cutting removal channel that minimizes the tendency of the pilot drill bit to drift off-axis as the initial bore in the bone is created. Once the initial bore is created, the pilot drill bit is withdrawn and a second side cutting tool, such as a milling bit, is then inserted into the bore to perform the desired surface resection.
