An anchor assembly for use with a tensioning strand includes an anchor body including a bore therethrough, the bore configured to receive the strand therethrough, an inner surface of the bore including a frustoconical wedge-receiving cavity, at least one frustoconical wedge including a strand-engaging inner surface and an anchor body engaging outer surface, the wedge being at least partially received in the wedge-receiving cavity, an encapsulating layer at least partially surrounding the anchor body, and a wedge retention disk mechanically coupled to the anchor body and positioned so as to prevent the wedge from fully exiting the wedge-receiving cavity. The wedge retention disk may be spaced apart from the anchor body such that the wedge can move axially with respect to the anchor body between a seated position in which the wedge engages the wedge-receiving cavity and an unseated position in which the wedge engages the wedge retention disk.

A coupler may include a first chuck, the first chuck including an outer surface and first external threads and a second chuck, the second chuck including an outer surface and second external threads having a reverse orientation relative to the first external threads. The coupler may also include a coupler body including a first end, a second end, and a passageway extending between the first end and the second end, the first end secured to the first chuck, the second end secured to the second chuck, the first end including first internal threads engaging the first external threads, the second end including second internal threads engaging the second external threads.

A cover is provided for sealing a concrete anchor including a flange and a bore extending through the flange. The bore defines an axis and the flange has a first side and a second side. The cover includes a first cover portion for covering a portion of the anchor proximate the first side of the flange, a second cover portion for covering a portion of the anchor proximate the second side of the flange, and a latch coupling the first cover portion and the second cover portion together with the anchor disposed between the latch.
