A medical instrument includes a handpiece, an electromechanical transducer disposed in the handpiece, and an electrical circuit disposed at least partially in the handpiece for supplying alternating electrical current of a predetermined frequency to the transducer. A probe is operatively connected to the transducer for transmitting vibrations generated by the transducer to an operative site in a patient. A switching device is mounted to the handpiece and is operatively connected to the circuit and the transducer for enabling the supply of power to the transducer during a motion of the probe in a preselected direction relative to the handpiece and for disabling the supply of power to the transducer upon a termination of motion of the probe in the preselected direction.

An ultrasonic medical treatment device has a probe, a transducer for mechanically vibrating the probe at an ultrasonic frequency, a voltage source for energizing the transducer, and another electrical voltage source for feeding to the probe a high-frequency alternating waveform of limited current and limited voltage to be conducted into a patient through the operative tip of the probe after placement of the operative tip into contact with the patient. The alternating waveform has a current and a voltage so limited as to prevent damage to organic tissues while stimulating nerves to reduce or suppress pain.

A surgical method utilizes a cutting blade having a thickness along a cutting edge of between about 0.0005 inch and about 0.020 inch and preferably between about 0.001 inch and 0.010 inch. The blade is moved in contact with relatively hardly tissues which are disposed adjacent to softer tissues at a surgical site in a patient. The blade is ultrasonically vibrated during the moving of the blade, whereby the hard tissue is cut with a modicum of damage being inflicted on the soft tissue.

A surgical device includes a probe, a transducer assembly operatively coupled to the probe for generating an ultrasonic resonant vibration therein, and a frequency generator operatively coupled to the transducer assembly for energizing the transducer component. A control component is operatively connected to the frequency generator for inducing the frequency generator to produce an alternating signal sequence including a first electrical excitation signal and a second electrical excitation signal, wherein the first electrical excitation signal has at least one first ultrasonic frequency and at least one first amplitude collectively selected to generate cavitation bubbles at a wound site to fragment damaged tissue and debride the wound site. The second electrical excitation signal has at least one second ultrasonic frequency and at least one second amplitude collectively selected to generate cavitation bubbles in a substantially reduced amount, thereby allowing for increased transmission of vibratory energy into the debrided tissues for enhancing healing.

Ultrasonic wound treatment method and apparatus

An ultrasonic medical probe comprises an elongate shaft formed integrally with a head portion having a distal end face oriented at least partially transversely to a longitudinal axis of the shaft. The shaft is provided with an internal longitudinal channel or bore extending to the end face. The end face is formed with an indentation communicating with the channel or bore at a distal end thereof, whereby liquid is guided over an extended surface of the end face relative to the channel or bore. The head portion also has a lateral surface extending substantially parallel to the longitudinal axis of the probe. The lateral surface is provided with at least one outwardly or radially extending projection. The projection enables the application of ultrasonic cavitation energy to a tissue surface that is in contact with the lateral or side surface of the probe head.

A surgical instrument comprises, in accordance with the present invention, an elongate probe, a sleeve, a sleeve extension and at least one cauterization electrode. The probe is an ultrasonic element, serving to convey ultrasonic vibrations (as standing waves) to organic tissues at a surgical site. The probe has a working tip and a longitudinal axis. The sleeve surrounds the probe with the working tip of the probe projecting from a distal or free end of the sleeve. The sleeve extension is disposed at the distal or free end of the sleeve and defines a multiplicity of apertures having respective axes oriented at least substantially parallel to the longitudinal axis. The electrode has a distal or free end removably inserted through one of the apertures.

A tranducer assembly for an ultrasonic surgical instrument includes a front driver having an elongate shaft in one direction and a stud extending in an opposite direction. An electromechanical transducer element is disposed around the stud. The transducer assembly also comprises a rear driver disposed around the stud on a side of the electromechanical transducer element opposite the front driver, the electromechanical transducer element being clamped between the front driver and the rear driver. An inertial or damping mass is fixedly connected to the stud at a point spaced from the rear driver. The stud is of length and thinness to act as a flexible vibration damping element.

A surgical device for ultrasonic ablation and RF cauterization includes a forceps member having at least one prong, the prong being formed as an electrode. At least one electrical connector is provided on the forceps member for operatively connecting the prong to an electrical power source. The surgical device further includes a mechanical connector attached to the forceps member for removably fastening the forceps member to a housing of an ultrasonic probe. The forceps member can thus function to carry out a cauterization procedure during a surgical operation utilizing the probe for ultrasonic ablation.

The electrode member is connectable to an RF voltage source and is mounted at least indirectly to the casing so as to permit relative motion between the electrode member and the probe.

The electrode members are connectable to an RF voltage source and are mounted at least indirectly to the casing so as to permit relative motion between the electrode members and the probe.
