A method of suturing a trocar path incision in a tissue of a patient with an obturator includes inserting the obturator through the tissue such that a shaft of the obturator extends through a tissue opening about the trocar path incision and a distal tip of the obturator is positioned within a cavity of the patient. The method also includes directing the suture via a suturing feature with the obturator inserted through the tissue in order to direct the suture relative to the tissue. Furthermore, the method includes closing the tissue opening about the trocar path incision with the suture.

A surgical access device includes a tissue retractor including a flexible body configured to engage tissue surrounding a tissue opening. A plurality of surgical instrument channels is arranged in a central portion of the tissue retractor. Each surgical instrument channel is configured to guide a surgical instrument distally through the surgical access device. At least one needle entrance port is arranged on a proximal portion of the surgical access device. At least one needle exit port is arranged distally of the needle entrance port. The at least one needle entrance port and the at least one needle exit port are configured to cooperate to define a needle channel extending distally through the surgical access device and obliquely relative to a central axis thereof. The needle channel is configured to guide a suture passer needle through the surgical access device and adjacent tissue to facilitate closure of the tissue opening.

An apparatus includes a staple cartridge. The staple cartridge includes staples, a deck, and a buttress. The deck defines a plurality of openings. Each opening of the plurality of openings is associated with a corresponding staple of the plurality of staples, such that each staple is configured to pass through a corresponding opening of the plurality of openings. The buttress is configured to detach from the staple deck in response to the staples being driven through the corresponding openings of the deck. The buttress is resiliently biased to transition from a first configuration while attached to the deck to a second configuration when detached from the deck. The buttress has a first length in the first configuration and a second length in the second configuration. The first length is longer than the second length.

Apparatus and method for pleating a bodily lumen

An apparatus includes a staple cartridge including a plurality of staples and a deck defining a plurality of openings. Each opening of the plurality of openings is associated with a corresponding staple of the plurality of staples, such that each staple is configured to pass through a corresponding opening of the plurality of openings. The apparatus further includes a deformable feature configured to be coupled to tissue in response to actuation of the staple cartridge. The deformable feature is configured to retain at least one outer portion of stapled tissue in an inwardly directed position relative to an inner portion of stapled tissue.

Methods and devices create an intestinal braking effect, are non-invasive or minimally invasive, and may be reversible. These methods and devices are accomplished via stabilized implantable systems, and ingestible pills. In one embodiment, the implantable system comprises a device delivering a therapeutic substance to a target location within the gastrointestinal tract of a patient in order to initiate an intestinal braking effect which would promote sensations of satiety and stimulate excess weight loss for the patient.

An apparatus includes a locking portion, a flexible pulling portion, a flexible body, and a plurality of barb features extending laterally from the body. The locking portion is configured to receive the pulling portion. The pulling portion is configured to engage and translate relative to the locking portion after the locking portion has received the pulling portion. The flexible body extends from the locking portion to the pulling portion. The flexible body is configured to form a loop when the pulling portion engages the locking portion. The loop is configured to decrease in size in response to pulling of the pulling portion relative to the locking portion. The barb features are configured to engage a tubular region of tissue and thereby form pleats in the tubular region of tissue.

Apparatus and method for radially bunching a bodily lumen

A surgical system comprises an end effector for operating on tissue and a screen positioned around the end effector. The screen is fluidly coupled with a suction source by a conduit such as a tube. The screen is configured to communicate suction through the screen while preventing tissue from contacting the end effector. The screen is further configured to retract proximally relative to the end effector to thereby expose the end effector.

An ultrasonic instrument comprises a body, a shaft assembly, an ultrasonic blade, and a pivoting member. The shaft assembly extends distally from the body. The ultrasonic blade is positioned distal to the shaft assembly. The pivoting member is pivotable with respect to the blade from an open position to a closed position to thereby clamp tissue between the pivoting member and the blade. The shaft assembly comprises a feature to provide for the removal of surgical debris (e.g. tissue, coagulated blood, etc.), body fluid, etc. from the shaft assembly. The feature may remove the surgical debris, body fluid, etc. from a cavity within the shaft assembly. The feature may also prevent the collection of surgical debris, body fluid, etc. within the shaft assembly.

In one embodiment, a method of producing satiety comprising the steps of accessing a gastrointestinal tract of a patient and implanting an intraintestinal therapeutic substance eluting implant. The implant is capable of eluting a satiety inducing substance selected from at least one of a nutrient, a specific satiety inducing bio-active substance, pancreatic polypeptides, free fatty acids, cholecystokinin, amino acids, glutamine, lipids, linoleic acid, or a combination thereof, from the implant into the gastrointestinal tract.

A surgical stapler includes first and second tissue clamping members and a knife member. The first tissue clamping member is configured to receive a plurality of staples. The second tissue clamping member comprises an anvil configured to form the staples. A tissue treatment member is positionable between the first and second tissue clamping members. The tissue treatment member comprises a rupturable pouch or baffle that is configured to receive a medical fluid including at least one tissue cell. The tissue treatment member may have a spiral shape, an annular disc shape, or an elongate sleeve shape. The tissue treatment member may also include outwardly extending tabs. The tissue treatment member may also include an injection port. In use, the staples and/or the knife pierce the tissue treatment member to release at least a portion of the medical fluid held by the tissue treatment member.

Methods and apparatus for delivering tissue treatment compositions to stapled tissue

Methods and devices reroute chyme to induce intestinal brake in order to improve the effectiveness of bariatric surgical procedures and to improve comorbidity resolution. A bowel is manipulated to provide a shortened path for chyme to travel to the ileum. These methods and devices of rerouting chyme to induce intestinal brake may include one or more of a surgical procedure, an implanted device, or a combination of an implant with an improved surgical procedure.

Methods and devices for the rerouting of chyme to induce intestinal brake

A surgical device is operable to coextrude a multilayered biocompatible tissue repair plug in situ within a lumen in a patient. The device comprises a handle, a shaft extending from the handle, a plurality of conduits extending through the shaft, a plurality of media chambers, and an actuator. At least one of the media chambers contains a suspension of at least one tissue fragment having at least one viable cell in a biocompatible carrier. The actuator is operable to cause media in each of the chambers to be urged through, and expelled from the orifice of, the conduit in communication therewith. The orifices are located at the distal end of the shaft such that media from the media chambers may be ejected into a lumen in a patient in order to form a multilayered biocompatible tissue repair plug in situ. The conduits and their orifices are all coaxially aligned.
