A radiation treatment system may include a gantry configured to rotate around an object, a treatment head moving with the gantry, a plurality of imaging radiation sources configured to emit imaging beams toward the object, and one or more first detectors configured to detect at least a portion of the imaging beams. When the treatment head is delivering a treatment beam to the object, the plurality of imaging radiation sources and the one or more first detectors may be positioned outside at least a portion of a maximum treatment radiation region so as not to interfere with the treatment beam. At least one of the plurality of imaging radiation sources and the one or more first detectors may be positioned at or near an edge of the maximum treatment radiation region.

A method may include obtaining input data relating to a target treatment plan for performing radiotherapy on a lesion using a radiation device. The input data may include a first target image of the lesion. The method may also include obtaining a segment shape estimation model. The method may also include estimating, based on the segment shape estimation model and the input data, a plurality of target location combinations of the target treatment plan and a plurality of target segment shapes of a collimator of the radiation device. One of the plurality of target location combinations may indicate a location of the collimator relative to the lesion. The positioning feature may have a plurality of feature values. Each feature value may correspond to one of the plurality of cross sections.

The method may include obtaining a first set of imaging data affording a sagittal view relating to a subject and a first couch supporting the subject. The first couch may have a plurality of first positions reflected in the first set of imaging data as a first conformation. The method includes retrieving a planning image of the treatment volume. The planning image may include a plurality of first voxels. The method also includes retrieving a treatment image that represents at least part of the treatment volume. The treatment image may include a plurality of second voxels. For at least one of the plurality of second voxels, the method further includes determining at least one reference voxel among the plurality of first voxels in the planning image and determining a dose value corresponding to the at least one second voxel. The system may also acquire at least one second image relating to the subject and the positioning line using the imaging component at an imaging position. The system may further determine a treatment isocenter of a target of the subject based on the at least one second image, and determine a treatment position of the subject based on the first image(s), the second image(s), and the positioning line.
