An implantable medical device monitors ST segment data collected from EGM. ST trends are established and monitored over time. The IMD is able to discern whether the data indicate supply ischemia, demand ischemia, or other physiological causes. The IMD is then able to provide appropriate information and alerts.

System and method for ischemia classification with implantable medical device

A system, comprising a sterilizable package; an implantable medical device placed inside the sterilizable package; and an electrical interface electrically coupled to the implantable medical device and extending from inside the sterilizable package to outside the sterilizable package. In various embodiments, the interface may include package contacts electrically coupled to electrode terminals on the implantable medical device, patient terminals and conductors extending between the package contacts and the patient terminals.

Medical device packaging system

An implantable medical device (IMD) performs periodic testing of a patient to determine ischemia threshold information. At selected times while the patient is at rest, the IMD increases the pacing rate over time until it receives feedback either from the patient or from an ischemia sensor. The IMD determines the threshold based upon the pacing rate at the time when the feedback was received. The threshold information can be used to adjust the upper pacing rate that can be used during rate adaptive pacing, to determine the effects of drug therapy, and to provide a general indication of the state of coronary artery disease in the patient. The periodic increase of pacing rate to the ischemic zone also provides a preconditioning of the myocardium to allow the patient greater exercise benefit without angina.

Implantable medical device with automatic ischemia threshold determination

An implantable medical device system and method in which the implantable device is adapted to operate in a minimum ventricular pacing mode. The device delivers cardiac pacing pulses in a first pacing mode during a normal mode of operation and upon detecting myocardial ischemia alters the first pacing mode in response to the myocardial ischemia detection.

An implantable medical device and associated method monitor a physiological signal for sensing physiological events and detecting a physiological condition in response to the sensed physiological events. The device senses a first event from the physiological signal, senses a noise signal in the physiological signal and senses a next event from the physiological signal wherein the first event and the next event define a signal interval. The signal interval is declared as a noisy interval in response to the sensed noise signal.

Method and apparatus for detecting noise in an implantable medical device

An analog physiologic signal, e.g., the cardiac EGM, sensed by an IMD is filtered with a high pass filter (HPF), the cut-off frequency of the HPF being within a predetermined frequency bandwidth, wherein a low-band portion of the predetermined frequency bandwidth is attenuated in the filtered physiologic signal. The filtered physiologic signal is digitized in real time order, and the digital data set is filtered in reverse time order employing a digital IIR filter having characteristics substantially matching the cut-off frequency and filter characteristics of the HPF. When the system is implemented within an IMD, the filtered digital data set is compressed by lossy compression algorithm, and the compressed data set is filtered in reverse time order. In certain embodiments, the filtered digital data set is uplink telemetry transmitted to an external medical device, and the uplink telemetered data set is filtered in reverse time order employing a digital backward IIR filter resident in the external medical device.

Methods and apparatus for filtering EGM signals detected by an implantable medical device

An implantable medical device (IMD) for gauging the severity of ischemia in a patient's heart includes a data collection module configured to receive data about the patient's heart, a data processing module configured to process the data to identify an episode of ischemia in the patient and to determine if the episode is stable or unstable, and a reporting module configured to provide an alarm to the patient if the episode is unstable. A method executable by an implantable medical device to detect ischemia in a human heart suitably includes the steps of receiving data about the heart at the implantable medical device, processing the data within the implantable medical device to determine the severity of ischemia, and providing a response from the implantable medical device to the patient if ischemia is indicated.

Method and apparatus for gauging severity of myocardial ischemic episodes

A signal measuring system for use with an Implantable Medical Device (IMD) is provided for measuring physiologic signals having a relatively large effective dynamic range. In one embodiment, the system includes a High-Pass Filter (HPF), an Analog-to-Digital Converter (ADC), a Decimation Filter (DF), and a Compensation Filter (CF). The HPF receives an input signal that includes both the baseline wander imposed on a physiological signal. According to one aspect of the invention, the HPF attenuates low frequency components of the input signal, including a portion of the frequency band within the desired output signal bandwidth. The ADC then oversamples the output signal of the HPF. The DF receives the output samples from the ADC and generates output samples at rate that is at least twice the maximum frequency of the desired output signal. The CF then amplifies the low frequency end of the DF output samples. The gain and cutoff frequency of the CF are selected to offset the HPF attenuation of those low frequency components of the input signal below the cutoff frequency of the HPF and above the minimum frequency of the desired output signal. In one embodiment, the DF and CF are implemented by a processing circuit executing programmed instructions. The processing circuit may be included within the IMD, or may be partially or fully implemented within a device that is external to the IMD.
