A minimally invasive surgical assembly broadly includes an outer hollow needle which has an outer diameter of 3.0 mm or smaller, and a coaxial surgical instrument having a shaft which extends through the outer hollow needle. The coaxial surgical instrument includes end effectors at the end of the shaft which are biased to an open position such that when the end effectors of the surgical instrument extended out of the needle they open, and they are closed by relative movement of the needle over them. The assembly preferably includes a first fixing element which is used to fix the relative location of the surgical instrument and the needle. The assembly also preferably includes a second fixing element which moves relative to the needle and is located on the outside thereof and which is used to fix the relative location of the needle to the patient.

Minimally invasive surgical assembly and methods

An expandable interbody fusion device includes superior and inferior plates that are configured to receive a sequentially inserted stack of expansion members or wafers. The superior and inferior plates include features that at least initially interlock the two plates until the superior plate is dislodged by pressure from the growing wafer stack. The wafers include features on their top and bottom surfaces that interlock the wafers in multiple degrees of freedom so that the wafer stack is not disrupted when the fusion device is fully expanded. Each wafer also includes features that interlock with the inferior plate until the wafer id dislodged by sequential introduction of another wafer.

Expandable interbody fusion device

&#x394;n apparatus and method for distracting, in a given direction, and supporting two tissue surfaces is provided. A plurality of wafers are consecutively inserted using a wafer insertion apparatus between the two tissue surfaces to create a column of wafers. A detachable wafer assembly is provided that includes a base wafer initially associated with a track assembly of a wafer insertion apparatus. The base wafer is dislodged from the track assembly so that the base wafer is left within the distraction site as the track assembly is removed. A top cap wafer is provided that is situated at the top of the wafer stack, in which the top cap wafer is larger than the remaining wafers to form a gap surrounding the stack to receive biologic material.

Tissue distraction device

A medical assembly includes a platform which is securely mountable to an operating room fixture, and a plurality of arms or arm modules coupled to the platform and each with a grasper for holding a surgical instrument or port. The medical assembly is particularly applicable for laparoscopic surgery although it is not limited thereto.

An apparatus and method for distracting, in a given direction, and supporting two tissue surfaces is disclosed using a column of wafers. The column of wafers is oriented so as to expand in the given direction as the wafers are consecutively added to the column. A wafer insertion apparatus includes a wafer cartridge configured to carry a stack of wafers to be inserted into the space, a track assembly including a first track for carrying a wafer toward the space, a second track and means for preventing retrograde movement of a wafer within the first track, an advancer/pusher mechanism slidably disposed within the second track and operable on a wafer within the first track to advance the wafer along the first track, and an advancement gun operably coupled to the advancer/pusher mechanism to propel the mechanism along the second track.
