A nutritional composition comprises fat, protein and carbohydrate such that the fat provides from about 60 to about 90% of the total calories of the composition, the protein provides from about 10 to about 30% of the total calories of the composition, and the carbohydrate provides from about 0.1 to about 15% of the total calories of the composition, and the composition comprises from about 0.01 to about 10 wt % beta-hydroxy-beta-methylbutyrate (HMB). A method of achieving or sustaining metabolic ketosis in an individual comprises administering the nutritional composition to the individual. A method of maintaining lean body mass and muscle strength in an individual during dietary ketosis comprises administering the nutritional composition to the individual.

Disclosed herein are stable oil-in-water emulsion ophthalmic topical liquid compositions having an average particle size less than 1 &#x3bc;m including at least one plant-derived oil other than castor oil wherein the oil comprises only aliphatic side chains free of polar pendent groups. The oil-in-water emulsion ophthalmic topical liquid compositions also include a hydrophilic surfactant having an HLB value between approximately 10 and 14, a vegetable oil-derived hydrophobic non-co-block surfactant having unsaturated side chains that contain less than four oxygen atoms having an HLB value between approximately 4 and 6 and is a liquid at room temperature. There is disclosed structured lipid containing either a gamma-linolenic acid or a dihomogamma-linolenic acid residue, together with an n-3 fatty acid residue and a medium chain fatty acid residue on the same glycerol backbone. This structured lipid is particularly well adapted to the treatment of disease or stress states. The gamma-linolenic or dihomogamma-linolenic acid residues modify the prostanoid synthesis pathway, reducing the level of series '2' prostanoids and elevating the levels of series '1' and '3' prostanoids. The n-3 fatty acid residue enhances the level of series '1' prostanoids as well as increases the production of series '3' prostanoids. The medium chain fatty acid residues enhances the absorption of the structured lipid. There is also disclosed enteral and parenteral diets as well as nutritional supplements containing the structured lipids of the invention.
