Techniques for adjusting radiotherapy treatment for a patient in real time are provided. The techniques include retrieving a reference plan that includes three-dimensional (3D) volume representation of information for the patient and a plurality of radiotherapy beam delivery segments; identifying a first portion of the 3D volume representation of information for a first beam delivery segment of the plurality of radiotherapy beam delivery segments that includes a volumetric portion of a target irradiated by the first beam delivery segment; accessing a deformation model representing patient deformation during a radiotherapy treatment fraction; deforming the first portion of the 3D volume representation of information based on the deformation model; and updating one or more parameters of a radiotherapy treatment device based on the deformed first portion of the 3D volume representation of information.

Geometry-based real-time adaptive radiotherapy

Methods for dose or treatment planning for a radiotherapy system including a radiotherapy unit are provided. A spatial dose delivered can be changed by adjusting beam shape settings, and the delivered radiation is determined using an optimization problem that steers the delivered radiation according to objectives reflecting criteria for regions of interest including at least one of: targets to be treated during treatment of the patient, organs at risk and/or healthy tissue. The method includes determining an inner set of voxels and providing a first frame description for the inner set of voxels, where the first frame description reflects criteria for the inner set of voxels. Determining an outer set of voxels encompassing the target volume and the inner set of voxels and a frame description for the outer set of voxels is provided where each reflecting criteria for the outer set of voxels. The frame descriptions are then used in the optimization problem that steers the delivered radiation.

Methods for inverse planning

In the field of radiotherapy, methods for dose or treatment planning for a radiation therapy system having a collimator, includes determining shots to be delivered during said treatment, each shot being associated with an isocenter and being modelled by a spatial dose volume distribution of radiation, the shape of said spatial distribution depending on the specific collimator setting and said selected dose level, including selecting isocenter positions within a target in a predetermined angle range; evaluating each isocenter based on predetermined conditions; selecting at least a specific collimator and sector setting for each isocenter based on the evaluation; calculating a dose for the selected isocenters; repeating the steps until at least one stopping criteria has been reached, wherein a final set of isocenters are provided; and using the final set of isocenters in treatment planning.

Methods for treatment planning

Techniques for adjusting radiotherapy treatment for a patient in real-time are provided. The techniques include determining a patient anatomy at a first time within a given radiotherapy treatment fraction after a radiotherapy treatment dose has been delivered by a radiotherapy device; retrieving a reference patient anatomy for the given radiotherapy treatment fraction that indicates a prescribed dose parameter to be delivered within the given radiotherapy treatment fraction; comparing the patient anatomy at the first time with the reference patient anatomy during the given radiotherapy treatment fraction; and based on the comparison of the patient anatomy at the first time with the reference patient anatomy, adjusting a parameter of the radiotherapy device to change an amount of radiotherapy treatment dose delivered at a second time, following the first time, within the given radiotherapy treatment fraction in accordance with the prescribed dose parameter to be delivered within the given radiotherapy treatment fraction.

Dose guided real-time adaptive radiotherapy

In the field of radiotherapy, methods for dose or treatment planning for a radiotherapy system are disclosed, wherein a spatial dose delivered can be adjusted and delivered radiation is determined using an optimization problem that steers the delivered radiation according to a frame description reflecting criteria for regions of interest that include at least one of targets to be treated during treatment of the patient, organs at risk and/or healthy tissue. The method includes estimating a voxel set receiving a higher dose than a predetermined threshold dose level, which voxel set includes voxels from at least one target volume. Further, a low dose voxel set is determined and a frame description for the voxels in the low dose voxel set is provided where voxels receiving a dose exceeding a predetermined threshold value is penalized such that the dose delivered to the low dose voxel set is suppressed. The frame description is then used in the optimization problem that steers the delivered radiation.
