A needle hub assembly for a pen needle includes a needle hub, an inner shield and an outer cover. The open end of the outer cover is closed by a removable seal to access the needle hub. The needle hub has a body with a shoulder and a tower extending from the body for supporting a cannula. The inner shield fits over the tower and the cannula and includes a flange that has an outer dimension complementing the outer dimension of the needle hub and contacts the shoulder of the needle hub in the assembled condition. The outer cover encloses the needle hub and the inner shield and includes a stop member on an inner surface that contacts the flange of the inner shield to capture the inner shield between the shoulder of the needle hub and the outer cover.

Pen needle assembly

A percutaneous system in which tubular retractors are utilized as a means of access, visualization, and a controlled orientation for preparation of posterior spinal elements and delivery of graft material for posterior spinal fusion. The tubular retractors are affixed to screw extensions to rotate and articulate relative to multi-axial screws percutaneously inserted into respective pedicles of vertebral bodies. The procedure is done bilaterally, although it could be performed unilaterally in cases where the anatomy or surgeon preference dictates. A number of different deployable graft concepts are disclosed, the primary features being to allow for creation of a graft space volume and a means for delivery of the graft material preferred by the surgeon.

A screwdriver with a release for driving a multi-axial bone screw into a vertebra of the spine includes an elongate shaft defining a driving tip at its distal end for engaging and rotating the head of the bone screw. The screw driver includes a screw engagement member affixed to the shaft for joint rotational movement therewith, the screw engagement member comprising a releasable retention member for releasable attachment to the bone screw yoke. A release member is disposed about the elongate shaft comprising a hollow grip sleeve and a hollow push sleeve joined together for common axial movement along the elongate shaft. The grip sleeve is freely rotatable about the shaft and the push sleeve is rotatable relative to the grip sleeve and engaged with a proximal portion of the shaft for joint rotational movement therewith.

A screw system includes a screw and a washer assembly captive to the screw. The washer assembly is polyaxially pivotable relative to the screw. The screw may be freely rotated in one direction relative to the washer assembly, but frictionally binds with the washer assembly when rotated in a second direction. Various auxiliary fixation features are disclosed.
