An iridocorneal angle of the eye can be opened with a plurality of treatment locations at least about 2 mm radially outward from a limbus of the eye. The opening on the angle can be beneficial for treating both narrow angle glaucoma and open angle glaucoma. The plurality of treatment locations located away from the limbus can decrease invasiveness and complexity of the procedure. The plurality of treatment locations at least about 2 mm away from the limbus can provide tensioning to zonules coupled to the lens of the eye to flatten the lens of the eye, which can allow the iris to move posteriorly so as to open the iridocorneal angle. The plurality of treatment locations may comprise scleral locations, in which shrinkage of scleral tissue at the plurality of treatment locations provides tensioning to the zonules.

Angle-opening glaucoma treatment methods and apparatus

An ab externo automated laser treatment system for treating an eye in a subject, includes a non-contact laser source configured to generate a laser beam having at least one wavelength to treat the eye by directing the laser beam from a location spaced from the eye, wherein the at least one wavelength is a near-infrared wavelength in the range of about 0.5-2.2 &#x3bc;m, a laser scanner optically coupled to the non-contact laser source to receive the laser beam from the non-contact laser source and to scan the laser beam relative to the eye, and a processor, and memory including stored computer-readable instructions that, responsive to execution by the processor, cause the laser treatment system to direct the laser beam to a plurality of trans-scleral treatment locations to be irradiated in a predetermined treatment pattern on an external surface of the eye, wherein the trans-scleral treatment locations are 0-4 mm posterior to the corneolimbal junction, and wherein the laser beam is repetitively directed to the same irradiated trans-scleral treatment locations on the surface of the eye, and the trans-scleral treatment locations are irradiated at intervals sufficient to induce protective thermal preconditioning and therapeutic bio-stimulation of one or more of the trabecular meshwork and/or ciliary body without photocoagulation of the tissue of the eye. Trans-pupillary systems, patient interfaces, and methods are also disclosed.

Laser therapy for treatment and prevention of eye diseases

A laser delivery system is configured to delivery light energy to soften and realign the tissue of the eye in order to increase accommodation and treat glaucoma. The laser system can be configured to increase a circumlental space of the eye and increase movement of a posterior vitreous zonule in order to increase accommodation. The light energy may comprise wavelengths strongly absorbed by collagen of the sclera. In many embodiments a heat sink is provided to couple to the conjunctiva and the heat sink comprises a material transmissive to the light energy absorbed by collagen, for example Zinc Selenide. The heat sink can be chilled to inhibit damage to the conjunctiva of the eye. In many embodiments, one or more layers of the epithelium of the eye remain substantially intact above the zone where the eye has been treated when the heat sink has been removed.

A method is provided for treating a targeted area of ocular tissue in a tissue-sparing manner comprising use of two or more therapeutic modalities, including thermal radiation source (such as an CW infrared fiber laser), operative in a wavelength range that has a high absorption in water, and photochemical collagen cross-linking (CXL), together with one or more specific system improvements, such as pen-operative feedback measurements for tailoring of the therapeutic modalities, an ocular tissue surface thermal control/cooling mechanism and a source of deuterated water/riboflavin solution in a delivery system targeting ocular tissue in the presence of the ultraviolet radiation. Additional methods of rapid cross-linking (RXL), are provided that enables cross-linking (CXL) therapy to be combined with thermal therapy.

Method and apparatus for treatment of ocular tissue using combined modalities

Delivery systems and methods for delivering riboflavin (R/F) and UVA irradiation to the sclera are disclosed. The R/F is delivered and then activated with UVA irradiation through the use of LEDs or optical fibers, thereby causing cross-linking of the collagen tissue. Delivery systems include implantable structures which provide surfaces that conform to the sclera. The delivery systems include various types of structures for delivery of R/F onto the sclera surface. Additionally, the delivery systems include UVA sources which provide irradiation of R/F in sclera collagen tissue.

A real-time refractory control system includes a laser refractive surgery instrument for modifying the refraction of the eye, an objective diagnostic apparatus for measuring the refraction and aberrations of the eye, and an aperture-sharing element to inject a refractive surgery beam and a monitoring diagnostic beam. An automated method is described for advantageously preparing and treating an eye of the patient. The method further provides a vision-corrective procedure, such as photothermal keratoplasty. The automated method is useful to dry the cornea to reduce or eliminate a tear film on the corneal surface that might otherwise interfere with or reduce the efficacy of a vision-corrective procedure and thereafter to treat the cornea according to a vision-corrective procedure. The automated method is user-friendly, allowing the treatment provider to develop or to select an appropriate pre-treatment and treatment plan and to activate sane using a familiar 'Windows' environment. The method has particular application in the area of automated corneal preparation and treatment, it may be used in the automated preparation and treatment of other tissues or substrates.

Radiation treatment method for treating eyes to correct vision

An instrument primarily adapted for use in photothermal keratoplasty procedures to shrink collagen tissue within a cornea of an eye being treated by controlled heating of the tissue with infra-red radiation. In a specific implementation example, a moveable multi-faceted prism is a primary optical element that forms a desired treatment infra-red radiation pattern from a single beam source. Multiple exposures to variations of the radiation pattern are provided. Parameters of two or more exposures to spot radiation patterns are stored in a memory of the instrument, and automatically configure the instrument to deliver the two or more exposures in sequence. Automatic calibration of the radiation intensity is also provided. A guide positions the head of the patient with respect to the instrument, and sensors monitoring movement of both the head and eye provide signals used to by the instrument to control radiation delivery. Connection of a number of instruments over the Internet to a central computer system allows maintaining a central patient treatment database, storing data of any instrument failures or faults useful for instrument maintenance and charging the instrument users for the individual procedures performed. A smart card is also useable as a pre-payment device to authorize treatments to be performed by the instrument, and data from the treatment may also be recorded on the smart card.

A system for supplying optical power uses an optical switch or shutter at the handpiece. The optical switch is positioned between an open light-transmitting position and a closed light-reflecting position. A detector at the base of the system can be used to detect the position of the optical shutter in some situations. This can be used to control the operation of a high-intensity light source. In this manner, the system can be controlled at the handpiece without using electrical power at the handpiece.
