A catheter for indirect left atrial mapping from the left pulmonary artery is disclosed. The catheter includes an elongated flexible member with a distal end and a proximal end. Located at the distal end is a balloon and an array of mapping electrodes. Located at the proximal end is a manifold to which various ports are secured. In a second embodiment, the catheter differs from the first embodiment in that interspersed within the array of mapping electrodes are defibrillation electrodes. Also, located proximally of the array of mapping electrodes is an array of defibrillation electrodes. In a third embodiment, the catheter differs from the first embodiment in that five defibrillation electrodes are located between the array of mapping electrodes and the balloon. Located proximally of the array of mapping electrodes is an array of defibrillation electrodes.

A heart tissue mapping and lasing catheter well suited for control of tachyarrhythmias is disclosed. A distal end region of the catheter has an electrode envelope constituted by plural surface electrode pairs spaced from one another and is provided with at least one fiber optic within the electrode envelope. The emissive face of the fiber optic is situated between adjacent electrode pairs in the electrode envelope.

Continuous heart tissue mapping and lasing catheter

The method comprises defibrillating or cardioverting a heart, employing either an implanted or an external defibrillation/cardioversion device capable of generating electrical shocks, by transvenously inserting into the lower left pulmonary artery a pulmonary arterial lead comprising a distal defibrillation electrode, sensing the occurrence of fibrillation or an arrhythmia, and defibrillating or cardioverting the heart by electrical shocks between such electrode and a transvenously-inserted additional defibrillation electrode. The additional defibrillation electrode may comprise a proximal defibrillation electrode located on the pulmonary arterial lead, with its distal end having been positioned in the right atrium/superior vena cava. Alternatively, the method comprises transvenously inserting a second lead comprising said additional electrode, and positioning the same in the right ventricle, the right atrium, or the superior vena cava. Sensing/pacing electrode means may be positioned in the right ventricle, and optionally also in the right atrium, for sensing and pacing the heart.

Tachyarrhythmias are corrected by in vivo ablation of human heart tissue employing laser energy delivered by means of elongated optical fibers in a series of short discrete pulses to sites of tachyarrhythmia foci. The operation may be conducted by a thoracotomy or by means of an optical fiber carried by a catheter passed into the heart via a blood vessel. An argon ion laser is preferred. The processes have been demonstrated successfully in 20 clinical operations pursuant to an experimental protocol.

Defibrillation of ventricular fibrillation and cardioversion of ventricular tachycardia is accomplished by single, bidirectional high energy shocks from two anodes to a common cathode. All the electrical apparatus is implanted without a thoracotomy.
