An airflow signal corresponding to the breathing of the patient is obtained. A section corresponding to inspiration, having a front portion and a middle portion, is found within the airflow signal. A peak value of the front portion is found, which is compared with a value representing the airflow of the middle portion. The presence of resisted breathing is determined based on the comparison between the peak value of the front portion with a value representing the airflow of the middle portion. A baseline value of the airflow signal is found by calculating the mode of values within an airflow signal representing the pressure within a patient's naris. The section corresponding to inspiration is determined by finding peak inspiration and baseline values within a breath and searching from the peak until the baseline is reached to find the section of inspiration.

Detection of airway resistance

A system for reducing central sleep apnea (CSA) is described in which certain methods of increasing a patient's rebreathing during periods of the sleep cycle are used. By increasing rebreathing during periods of overbreathing, the over-oxygenation which typically results from the overbreathing period can be reduced, thus reducing the compensating underbreathing period and effectively reducing the loop gain associated with the central sleep apnea. Nasal occlusion and a leak resistant oral interface provide control for gas leaks from a patent interface.

Ventilatory stabilization technology

A system for reducing central sleep apnea (CSA) is described in which certain methods of increasing a patient's rebreathing during periods of the sleep cycle are used. By increasing rebreathing during periods of overbreathing, the over-oxygenation which typically results from the overbreathing period can be reduced, thus reducing the compensating underbreathing period and effectively reducing the loop gain associated with the central sleep apnea.

The present invention relates to systems and methods for automatically and continuously regulating the amount of mandibular displacement to an optimal value during obstructive sleep apnea treatment. Obstructive sleep apnea therapy is implemented by a device which automatically reevaluates an applied mandibular displacement and continually searches for a minimum displacement required to adequately distend a patient's pharyngeal airway. The minimum optimal displacement varies with body position, stage of sleep throughout the night, the patient's body weight, and whether alcohol or sleeping medicine has been ingested.

A remote-controlled mandibular positioner for conveniently and precisely positioning the mandible during a sleep center or in-hospital study of patients who are candidates for mandibular positioner therapy. The remote-controlled mandibular positioner is used during sleep to perform a therapeutic titration study during full polysomnographic monitoring to determine the mandible position effective in treating obstructive sleep apnea. During the titration study, the mandible is advanced by a technician using the remote-controlled mandibular positioner until all apneas and hypopneas are eliminated. The titration study determines whether a mandibular positioner will be effective in treating a particular patient, and if so, the degree of mandibular ventral displacement required for effective treatment of obstructive sleep apnea.
