A pivotal bone anchor assembly includes a receiver having a cavity with a bottom opening and a first channel for receiving a rod, and a shank having a head positionable into the cavity with the shank extending downward through the bottom opening. The assembly also includes an insert positionable into the central bore above the shank head and having a second channel alignable with the first channel, opposite extensions extending outwardly in the direction of the channels to block rotation between the insert and the receiver, and upward-facing surfaces located laterally outward from and on each side of the second open channel. The upward-facing surfaces are configured for releasable compressive engagement by a compressing tool after the elongate rod is received within the channels and prior to a final locking of the assembly so as to provisionally lock the shank from pivoting with respect to the receiver.

A surgical frame and method for use thereof is provided. The surgical frame is capable of reconfiguration before, during, or after surgery. The surgical frame includes a main beam that can be rotated, raised/lowered, and tilted upwardly/downwardly to afford positioning and repositioning of a patient supported thereon. The surgical frame also includes a reconfigurable upper leg support for supporting portions of the upper legs, the hips, and the lower back of the patient to facilitate positioning and repositioning there during surgery. The upper leg support via reconfiguration thereof can accommodate patients of different sizes, can provide flexure of the patient's lumbar spine to facilitate surgical access thereto, and can prevent unwanted torsion of a patient's spine during such reconfiguration.

Reconfigurable upper leg support for a surgical frame

A pivotal bone anchor assembly is provided that includes a receiver, a shank, and a compression insert. The receiver can have a shank holding lower portion and an upper portion with a channel. The shank can have a head portion and an anchor portion attachable to bone. The compression insert can be positionable with the receiver, and can have an upper portion including at least one notch at least partially formed in an outer surface of the upper portion, and defining at least one resiliently deflectable upwardly-facing surface extending over or above the at least one notch. In one embodiment, the at least one resiliently deflectable upward facing surface being deflectable when the head portion of the shank and the compression insert are positioned in the receiver to provide a pre-lock friction fit in the assembly prior to securing a longitudinal connecting member in the channel with a closure.

Pivotal bone anchor assembly and method for use thereof

A pivotal bone anchor assembly includes a receiver defining a central bore with a circumferentially-extending horizontal interference structure located above an upper expansion region and a lower locking region, and a shank having an upper end portion up-loadable into the central bore through a bottom opening. The assembly also includes a retainer having an upper portion and a lower expansion portion pre-positioned within the upper expansion region and being operable to capture the upper end portion of the shank, and a compression insert postionable within the central bore having an upwardly-facing surface for engaging a rod, an exterior side surface for engaging the horizontal interference structure, and a lower recess for receiving the upper portion of the retaining structure in a side-to-side overlapping engagement. The compression insert is restrained from moving downwardly within the central bore until forcible downward displacement by direct engagement with tooling to drive the exterior side surface of the compression insert at least partially through the horizontal interference structure.

A pivotal bone anchor assembly includes a receiver having an internal cavity with a circumferential groove adjacent a bottom opening, an upper channel with a downward-facing surface below a closure mating structure, and an internal cylindrical surface with a fixed rotation abutment surface. The assembly also includes an insert positionable into the upper channel having outer side surfaces of upstanding arms with a protruding abutment structure, a shank with a capture portion uploadable through the bottom opening, and a ring retainer uploadable into the circumferential groove to engage and hold the capture portion within the cavity. The insert is rotatable about a centerline axis of the receiver until an upwardly-facing surface on the insert rotates beneath the downward-facing surface and an outer surface of the protruding abutment structure abuts the fixed rotation abutment surface to inhibit both upward movement and further rotation, respectively, of the insert within the receiver.

A receiver assembly for capturing an elongate rod in a receiver of a bone anchor includes the receiver, a first threaded closure, and a second threaded closure. The receiver includes an upper portion that defines a first channel, interior surfaces that have helically wound thread, and a lower pressure insert that can be positioned within the first channel and has upright arms. Each of the first threaded closure and the second threaded closure have a cylindrical body with an external surface having continuous helically wound thread that can rotatably mate with the helically wound receiver thread. The first threaded closure and the second threaded closure are interchangeable for different locking configurations in the receiver assembly. The first threaded closure has a downward protrusion that can compressively engage the elongate rod. The second threaded closure has a bottom surface that can compressively engage the upright arms of the lower pressure insert.

A method of assembling a pivotal bone anchor assembly includes positioning an upper portion of a bone screw and a ring shaped structure into a central bore of a receiver, the central bore having an internal retaining recess above a lower opening, the upper portion having a partial spherical lower surface providing for pivotal motion of the bone screw with respect to the receiver. The method also includes positioning an insert into the central bore, the insert having a bottom surface for transferring a compressive force to lock the pivotal bone anchor assembly and a pair of insert arms defining an insert channel for receiving a rod, the insert arms including upwardly-facing surfaces extending between front and back faces of the insert. The method further includes pressing the insert into position within the central bore such that the upwardly-facing surfaces are positioned under a downwardly-facing surface of the internal retaining recess to inhibit upward movement of the insert within the receiver.

The assembly also includes a shank with a head portion pivotably supported by the support surface with the shank extending downwardly through the bottom opening, and an insert with a lower surface engageable with the head portion of the shank, an insert channel for receiving the rod, an upward-facing surface located radially outward from the insert channel, and one or more slots configured to make the insert resiliently axially compressible when the upward-facing surface of the insert abuts the downward-facing surface of the central bore to apply a downwardly directed pressure against the head portion of the shank.

A method of assembling a pivotal bone anchor assembly includes positioning an insert and a two-part retainer into a central bore of a receiver having a seating surface proximate a bottom opening, a closure mating structure proximate a top opening, and integral engagement structures protruding inwardly between the closure mating structure and the seating surface. The insert includes an upper surface for engaging a rod and side surfaces engageable with the integral engagement structures to restrict motion of the insert with the central bore. The retainer includes two separable retainer parts together defining outer retainer surfaces engageable with the seating surface and inner retainer surfaces. The method also includes uploading a shank head of a shank through the bottom opening and into the retainer, with the two retainer parts operable to separate and then move back together to capture the shank head, after which a downwardly-directed force is configured to drive the outer retainer surfaces into frictional engagement with the seating surface to lock the angular orientation of the shank with respect to the receiver.

A receiver assembly for securing an elongate rod to a bone anchor including a receiver and a threaded plug. The receiver including an upper portion defining a channel to receive the elongate rod, an interior surface having helically wound receiver thread, and an exterior surface having in some embodiments first and second horizontal curvate extending instrument engaging grooves. The threaded plug including a longitudinal axis, a lower surface having a protrusion, and an external side surface having a continuous helically wound closure thread to rotatably mate with the helically wound receiver thread. The continuous helically wound closure thread including a lower leading surface and an upper trailing surface, both in some embodiments extending perpendicular to the longitudinal axis, and a cylindrical outer crest surface extending parallel to the longitudinal axis. When the protrusion on the threaded plug engages the elongate rod, the threaded plug does not extend overtop a top surface of the receiver.
