A method for securing an elongate rod to a spinal fixation structure having a receiver and a threaded closure. The method includes providing the elongate rod, the receiver, and the closure. The closure includes a thread form having a stab flank, a loading flank, an outer crest surface with a crest height, and a root surface. The thread form also includes a base height that is greater than the crest height.

After the proximal capture portion of the bone anchor is positioned within internal cavity, the pressure insert is downwardly movable within the central bore to force the exterior surface at least partially past the interference engagement structure to inhibit the pressure insert from moving back up within the central bore.

The assembly also includes a screw shank having a shank head at a proximal end and an anchor portion at a distal end configured for fixation to the bone, with the shank head having an outer spherical surface with a single common radius and an annular planar top end surface entirely surrounding an internal drive socket formed into a top end of the shank head. The assembly further includes a pressure insert having an upper surface for receiving the rod and a lower surface for engaging the outer spherical surface of the shank head to direct a downwardly-directed force from the rod to the screw shank in a locked configuration. The annular planar top end surface on the shank head remains spaced apart from the lower surface of the insert so as to not directly receive the downwardly-directed force therefrom in the locked configuration.

A method of assembling a pivotal bone anchor assembly includes loading an insert into a central bore of a receiver, with the receiver having the central bore extending upward from a bottom opening to and through a first channel for receiving a rod, and the insert having an upper portion defining a second channel, opposite lateral protrusions extending outwardly in a direction transverse to the second channel, a lower collet portion, and a center opening. The method also includes positioning the insert into the central bore with the opposite lateral protrusions located adjacent to and above interference abutment surfaces formed into the central bore, and positioning a load ring within the center opening of the insert. The method further includes uploading a shank head into the central bore of the receiver through the bottom opening, capturing the shank head within the lower collet portion of the insert, and then forcibly downward displacing the insert within the central bore with the tooling so as to move the opposite lateral protrusions below the interference abutment surfaces.

A pivotal bone anchor assembly includes a receiver having a first channel for receiving a rod and a central bore with a seating surface adjacent a bottom opening with a first cut-out portion. The assembly also includes a bone anchor having an upper end portion pivotably engageable with the seating surface, an anchor portion opposite the upper end portion, and a neck extending between the upper end and anchor portions. The assembly further includes an insert positionable in the central bore with a second channel alignable with the first channel, a concave lower surface engageable with the upper end portion of the bone anchor, and a second cut-out portion alignable with the first cut-out portion. The neck and at least a portion of the upper end portion of the bone anchor are positionable into the first cut-out portion and the second cut-out portion, respectively, to provide for increased pivotal angulation of the bone anchor with respect to the receiver in the direction of the cut-out portions.

Each receiver sub-assembly also includes one of a multiplanar retaining structure, a monoplanar retaining structure, or a monoaxial retaining structure positioned therein and slidably engageable with the seating surface after capturing the shank head upon its uploading through the bottom opening.

The assembly also includes an insert positionable into the central bore above the shank head and having a second channel alignable with the first channel, opposite extensions extending outwardly in the direction of the channels to block rotation between the insert and the receiver, and upward-facing surfaces located laterally outward from and on each side of the second open channel. The upward-facing surfaces are configured for releasable compressive engagement by a compressing tool after the elongate rod is received within the channels and prior to a final locking of the assembly so as to provisionally lock the shank from pivoting with respect to the receiver.

A surgical frame and method for use thereof is provided. The surgical frame is capable of reconfiguration before, during, or after surgery. The surgical frame includes a main beam that can be rotated, raised/lowered, and tilted upwardly/downwardly to afford positioning and repositioning of a patient supported thereon. The surgical frame also includes a reconfigurable upper leg support for supporting portions of the upper legs, the hips, and the lower back of the patient to facilitate positioning and repositioning there during surgery. The upper leg support via reconfiguration thereof can accommodate patients of different sizes, can provide flexure of the patient's lumbar spine to facilitate surgical access thereto, and can prevent unwanted torsion of a patient's spine during such reconfiguration.

The compression insert can be positionable with the receiver, and can have an upper portion including at least one notch at least partially formed in an outer surface of the upper portion, and defining at least one resiliently deflectable upwardly-facing surface extending over or above the at least one notch. In one embodiment, the at least one resiliently deflectable upward facing surface being deflectable when the head portion of the shank and the compression insert are positioned in the receiver to provide a pre-lock friction fit in the assembly prior to securing a longitudinal connecting member in the channel with a closure.

A pivotal bone anchor assembly includes a receiver defining a central bore with a circumferentially-extending horizontal interference structure located above an upper expansion region and a lower locking region, and a shank having an upper end portion up-loadable into the central bore through a bottom opening. The assembly also includes a retainer having an upper portion and a lower expansion portion pre-positioned within the upper expansion region and being operable to capture the upper end portion of the shank, and a compression insert postionable within the central bore having an upwardly-facing surface for engaging a rod, an exterior side surface for engaging the horizontal interference structure, and a lower recess for receiving the upper portion of the retaining structure in a side-to-side overlapping engagement. The compression insert is restrained from moving downwardly within the central bore until forcible downward displacement by direct engagement with tooling to drive the exterior side surface of the compression insert at least partially through the horizontal interference structure.

A pivotal bone anchor assembly includes a receiver having an internal cavity with a circumferential groove adjacent a bottom opening, an upper channel with a downward-facing surface below a closure mating structure, and an internal cylindrical surface with a fixed rotation abutment surface. The assembly also includes an insert positionable into the upper channel having outer side surfaces of upstanding arms with a protruding abutment structure, a shank with a capture portion uploadable through the bottom opening, and a ring retainer uploadable into the circumferential groove to engage and hold the capture portion within the cavity. The insert is rotatable about a centerline axis of the receiver until an upwardly-facing surface on the insert rotates beneath the downward-facing surface and an outer surface of the protruding abutment structure abuts the fixed rotation abutment surface to inhibit both upward movement and further rotation, respectively, of the insert within the receiver.

A receiver assembly for capturing an elongate rod in a receiver of a bone anchor includes the receiver, a first threaded closure, and a second threaded closure. The receiver includes an upper portion that defines a first channel, interior surfaces that have helically wound thread, and a lower pressure insert that can be positioned within the first channel and has upright arms. Each of the first threaded closure and the second threaded closure have a cylindrical body with an external surface having continuous helically wound thread that can rotatably mate with the helically wound receiver thread. The first threaded closure and the second threaded closure are interchangeable for different locking configurations in the receiver assembly. The first threaded closure has a downward protrusion that can compressively engage the elongate rod. The second threaded closure has a bottom surface that can compressively engage the upright arms of the lower pressure insert.
