A duplex label system. The duplex label system comprises a first face ply, a second face ply, and a liner ply having a top side and a bottom side. The top side and bottom side is secured to the first face ply and second face ply respectively using a first adhesive layer and the second adhesive layer. The duplex label system includes lines of weakness that form a first central portion, a first perimeter portion, a second central portion, and a second perimeter portion. The first central portion comprises an inboard portion of the first face ply inboard the lines of weakness. The second central portion comprises inboard portions of the second face ply and the liner ply inboard the lines of weakness. The duplex label system is adhered to a first substrate. The second ply inboard portion is removable from the substrate and securable to a second substrate.

Tamper evident form

A business form may include a first ply and a second ply coupled thereto, each ply having an upper portion and a lower portion. A wristband is die cut into the second ply upper portion and releasably secured to the first ply, the wristband having a printable central portion and a strap portion. A plurality of compound elements is located in the second ply lower portion, each compound element comprising a release liner area and a die cut label area, the label area having an alert label disposed thereon, the alert label being bounded by the release liner area. A plurality of chart labels are disposed on the first ply lower portion, the chart labels being releasably secured to the form.

Combination wristband and label form

A wristband and label form. The form comprises a first portion having a plurality of releasable labels die cut therein and a second portion having a wristband formed of a single ply. The wristband is releasably secured to the second portion and has a first panel and a second panel. The first panel has a printable area comprising printable coating. The printable area is accessible through the void and is configured to receive indicia from a printer through the void while the wristband is releasably secured to the second portion. Upon removal of the wristband from the second portion, the second panel is foldable over the first panel to laminate the printable area.

A combination wristband and label form. A combination wristband and label includes a front side formed of paper and a back side. The back side has a polyester section having a first section and a second section. The first section is removable from the form and the second section is permanently adhered to the form. The first section comprises a removable wristband and a removable extender. The wristband has a first panel and a second panel, a first strap extending outboard from said first panel and a second strap extending outboard from said second panel. The first panel has the printable area defined therein and a surrounding area surrounds the printable area. A size of the printable area is disparate from a size of said first panel.

A combination wristband and label form. The form comprises a front side formed of paper. The front side has a first portion having a plurality of labels die cut therein and a second portion having a void. A back side of the form has a polyester section comprising a wristband defined by die cuts in the polyester section and removable from the form. The single-ply wristband has a strap and a foldable having a first panel and a second panel. The first panel has an imaging area defined by a coating and the second panel is configured to laminate only a portion of the imaging area. A size of the second panel is disparate from a size of the first panel.

The wristband is releasably secured to the second portion and has a first panel and a second panel. The first panel has a printable area comprising printable coating. The printable area is accessible through the void and is configured to receive indicia from a printer through the void while the wristband is releasably secured to the second portion. Upon removal of the wristband from the second portion, the second panel is foldable over the first panel to laminate the printable area.

A combination wristband and label form. The form has a front sheet and a back sheet. The front sheet has a void. The back sheet has a wristband die cut therein and separable therefrom. The wristband has a printable portion and a laminating portion. The printable portion is coated with ink that faces away from the back sheet. The wristband is devoid of paper. The printable portion is configured to be printed through the void while the wristband is attached to the combination wristband and label form.

A patient-identification wristband. The wristband comprises a strap. A terminal portion of the wristband has an upper section and a lower lamination section. The upper section comprises a printable ink defining a print area, a first slot inboard the print area and between the print area and the strap, a second slot outboard the print area, and a third slot outboard the second slot. The lower lamination section has a fourth slot below the second slot and a fifth slot outboard the fourth slot. A size of the fourth slot is bigger than a size of the first slot. A length of the upper section is greater than a length of the lower lamination section such that when the lower lamination section is folded over for lamination, the lower lamination section covers only a first part of the print area and leaves a second part of the print area exposed.

Wristband and label form

A method for printing and applying labels comprises printing indicia on a first label using a printer to form a first printed label and holding the first printed label in a holding tray. The method includes moving an arm from an original position to a first position to collect the first printed label from the holding tray, and rotating the arm in a first direction to move the first printed label towards a substrate. The method comprises moistening the first printed label to activate a dissolvable liner of the first printed label. The method includes using the arm to apply the moistened first printed label to the substrate. The arm is rotated in a second direction away from the substrate. The printer prints a second label before the arm returns to the original position to collect the second label from the holding tray.
