A medical device for implantation into vessels or luminal structures within the body is provided, which stimulates positive blood vessel remodeling. The medical device, such as a stent and a synthetic graft, is coated with a pharmaceutical composition consisting of a controlled-release matrix and one or more pharmaceutical substances for direct delivery of drugs to surrounding tissues. The coating on the medical device further comprises a ligand such as a peptide, an antibody or a small molecule for capturing progenitor endothelial cells in the blood contacting surface of the device for restoring an endothelium at the site of injury. In particular, the drug-coated stents are for use, for example, in balloon angioplasty procedures for preventing or inhibiting restenosis.

The invention relates to a method for healing blood vessels by stimulating the formation of a confluent endothelial autologous cell layer in vivo on an implantable metallic stent having a lumen and a luminal surface, and an exterior surface. More specifically, the method includes implanting the stent with a coating in a patient in need of thereof; wherein the coating includes one or more layers of a matrix covalently adherent on said luminal and exterior surface of said stent containing one or more pharmaceutical substances on said exterior surface and a therapeutically effective amount of a single type of antibody, antibody fragments or combinations thereof being compatible to binding selectively to a specific cell surface antigen of circulating autologous endothelial progenitor cells in peripheral blood. In addition, genetically engineered endothelial progenitor cells can be captured on said luminal surface of stent in vivo, to proliferate to form rapidly a confluent endothelium in situ.

Medical device with coating that promotes endothelial cell adherence

The stent of this invention is a self-expanding stent created by a scaffolding lattice. The stent may be made from a nickel-titanium alloy. The lattice is formed from two different types of helices that proceed circumferentially in opposite directions along the longitudinal axis of the stent. The helices have no free ends. The first type of helix is formed by a series of undulations and the second type of helix is formed from a series of connection elements. The undulations may be in a zigzag or sinusoidal pattern. The connection elements connect the junction points lying on adjacent turns of the first type of helix. The junction points are formed by the ascending and descending arms of the undulations or zigzags. The ends of the stent may be formed by a closed circumferential element which is linked by connection elements to a transition zone. The transition zone is formed by a closed loop that connects directly to the first helix. The amplitude of the undulations or zigzags forming the transition zone increases from the closed loop to the point connecting the transition zone with the first type of helix. The closed circumferential element may be made from a radiopaque material. The scaffolding lattice design of the stent provides a stent having a high degree of flexibility as well as radial strength.

Self-expanding stent

An intraluminal endoprosthesis having a conically shaped first end and a tubular shaped balloon-expandable stent for a main body is disclosed. The conically shaped first end may form a flare to the main body and is particularly well suited for in ostium use. The first end is preferably self-expanding and the main body is preferably balloon-expandable. Also disclosed is a delivery device for delivering intraluminal ostial endoprosthetic devices, especially those disclosed herein, to a site for deployment. The delivery device may comprise an over-the-wire system or may comprise a rapid-exchange shuttle system. The self-expanding portion of the endoprosthesis is encapsulated in a sheath or other restraining apparatus on the delivery device. The balloon-expandable stent portion of the endoprosthesis is crimped onto a balloon delivery device. The delivery system and endoprosthesis of the present invention allow the endoprosthesis to be partially expanded and relocated if it is determined that it is not located in the proper location. To aid in positioning, the delivery device may comprise marker bands.

A medical device for implantation into vessels or luminal structures within the body is provided, which stimulates positive blood vessel remodeling. The medical device, such as a stent and a synthetic graft, is provided with a coating with a pharmaceutical composition containing a controlled-release matrix and one or more pharmaceutical substances for direct delivery of drugs to surrounding tissues. The coating on the medical device further comprises one or more barrier layers, and a ligand such as a peptide, an antibody or a small molecule for capturing progenitor endothelial cells in the blood contacting surface of the device for restoring an endothelium at the site of injury. In particular, the drug-coated stents are for use, for example, in balloon angioplasty procedures for preventing or inhibiting restenosis.

A medical device with a coating for capturing target cells in vivo is provided. In particular, the medical device is coated with at least one layer of matrix and a layer of antibodies, antibody fragments or combinations thereof, which bind with specificity to mature or progenitor endothelial cells at various developmental stages to form an endothelial cell layer on the surface of the device. The coated medical device can be, for example, a stent or a synthetic graft and is useful in therapy of diseases such as restenosis, atherosclerosis, and thromboembolic complications.

A medical device coated with one or more antibodies and one or more layers of a matrix is disclosed. The antibodies or fragments thereof react with an endothelial cell surface antigen. Also disclosed are compositions and methods for producing the medical device. The matrix coating the medical device may be composed of a synthetic material, such as a fullerene, or a naturally occurring material. The fullerenes range from about C60 to about C100. The medical device may be a stent or a synthetic graft. The antibodies promote the adherence of cells captured in vivo on the medical device. The antibodies may be mixed with the matrix or covalently tethered through a linker molecule to the matrix. Following adherence to the medical device, the cells differentiate and proliferate on the medical device. The antibodies may be different types of monoclonal antibodies. By facilitating adherence of cells to the surface of the medical device, the disclosed methods and compositions will decrease the incidence of restenosis as well as other thromboembolic complications resulting from implantation of medical devices.

An endoprosthesis is provided which includes a stent component and a graft component capturing a portion of the stent component. The stent component is made of generally malleable material arranged to provide the stent component with a collapsed transluminal positioning configuration and an expanded, deployed configuration. The stent component has adjacent end windings that are welded together. In a preferred arrangement, a plurality of these welds define a spine-like welded arrangement, and a number of these arrangements are positioned generally circumferentially around the ends of the stent component. The graft component extends generally between the welded end portions of the stent component, with limited overlap being possible. Also provided is a method for forming this endoprosthesis and a procedure by which the endoprosthesis is implanted by deployment with a suitable expansion device.

A stent loading device is provided for loading a stent on a catheter of the type used in percutaneous transluminal angioplasty (PTA) or percutaneous transluminal coronary angioplasty (PTCA) procedures. The device has an end unit for holding a stent, a collet having a plurality of radially compressible fingers fitted to said end unit, a main body having a plurality of finger-like extensions whose distal ends can be fitted into openings in said base unit, a proximal extension for receiving a cap with compressible washer therein fitted to said main body proximal extension, a rotatable sleeve with collet driver fitted about said collet and main body finger-like extensions and a guide wire embedded in end unit and longitudinally extending through said device to either terminate at the proximal end of said cap or extend a distance from the proximal end of said cap. The stent loading device serves as a stent delivery tool within which a catheter is inserted, after which the stent is crimped onto the catheter balloon. The stent loading device is disposable and well-suited for one-time use.

A biopsy forceps device is presented herein which employs a forceps assembly having a pair of forceps connected to the distal end of a control wire having its proximal end extending into a hub. A control wire actuator is connected to the proximal end of the control wire and the actuator is slidably mounted to the hub for slidable movement relative thereto between proximal and distal positions for respectively causing the forceps to be in a closed condition and in an open condition. A releasable locking mechanism normally blocks forward slidable movement of the actuator to prevent the forceps from being actuated to an open condition. A release mechanism releases the locking means from blocking forward movement of the actuator to thereby permit forward movement of the actuator causing the forceps to be actuated to an open condition.

Biopsy forceps device having improved handle assembly

A biopsy forceps device is provided exhibiting improved torqueable and formable characteristics. The device includes a handle and a core wire connected at its proximal end to the handle and connected at its distal end to a forceps assembly. The control wire displacing device is carried by the handle and serves to displace the core wire for moving the core wire between a forceps open position and a forceps closed position. The core wire includes at least three elongated portions including a proximal portion having a proximal end secured to the wire displacing means, a distal portion having a distal end secured to the forceps assembly and an intermediate portion located between the proximal and distal portions. The proximal portion is of greater length than either the intermediate or distal portions and is of greater diameter than the intermediate portion which, in turn, is of greater diameter than the distal portion. The first tapered portion is located between the distal portion and the intermediate portion and a second tapered portion is located between the intermediate portion and the proximal portion.

Torqueable and formable biopsy forceps

A biopsy forceps device is presented herein wherein an elongated flexible hollow body portion having a lumen extending therethrough is provided with proximal and distal ends. A forceps assembly is coupled to the distal end of the body portion and the assembly includes a pair of forceps. A control wire having proximal and distal ends extends through the lumen of the body portion and is coupled at its proximal end to a handle and at its distal end to a forceps assembly. A trigger is mounted to the handle for slidable movement relative thereto between a proximal position and a distal position. A gripping device grips the proximal end of the control wire and is movable between first and second positions for respectively moving the control wire between a forceps open condition and a forceps closed condition. The gripping device is resiliently biased toward its first position. The lever arm engages and moves the gripping device from its first position to its second position against the resilient forces. A releasable locking mechanism is carried by the trigger and the lever arm to lock the gripping device in the forceps closed condition so long as the trigger is in its distal position.
