An exemplary method includes performing a capture threshold assessment using a bipolar electrode configuration, deciding if capture occurred for a maximum energy value of the capture threshold assessment and, if capture did not occur, then performing a lead impedance test for the lead associated with the bipolar electrode configuration. Such a test may aim to detect an insulation defect and/or a conductor defect. Other exemplary methods, devices, systems, etc., are also disclosed.

A method for diagnosing an implantable cardiac device including a plurality of implanted leads may include: monitoring a plurality of parameters associated with the plurality of implanted leads; detecting a change in one of the parameters; evaluating at least one of the other parameters upon detection of the change; and diagnosing a problem with the implantable cardiac device based on the detected change and the evaluation. A system for diagnosing an implantable cardiac device including a plurality of implanted leads may include an implantable pacing device and a processor. The processor may be configured to: monitor a plurality of parameters associated with the plurality of implanted leads; detect a change in one of the parameters; evaluating at least one of the other parameters upon detection of the change; and diagnose a problem with the implantable cardiac device based on the detected change and the evaluation.

Systems and methods for diagnosing an implantable device

A real-time impedance monitoring system is provided for use with an implantable medical device having an implantable electrical lead. The impedance monitoring system includes components for determining the electrical impedance of the lead as a function of time, with the determination being made substantially in real-time, and components for graphically displaying the electrical impedance of the lead as a function of time, with the display also being generating substantially in real-time. In one specific example described herein, the implantable medical device is a pacemaker and the impedance monitoring system is within an external programmer device separate from the pacemaker. The programmer device includes a computer display screen or a computer printout device for presenting real-time graphical displays of the lead impedance. The programmer may alternatively generate graphical displays of lead impedance as a function of time based upon pre-recorded data.

A system and method for safely altering the function of an implanted pacemaker in a noninvasive manner includes an implantable programmable pacemaker and a non-implantable programming device. The pacemaker includes a pulse generator that generates stimulation pulses as controlled by a control program. The control program, and associated control parameters, are stored in an implantable memory included within the pacemaker. The pacemaker further includes a telemetry circuit that allows the control parameters to be selectively changed or altered from a location remote from the pacemaker (i.e., a non-implanted location). The programmer includes a telemetry head for establishing a telemetry link with the pacemaker's telemetry circuit. Once a telemetry link is established, the programmer may be selectively operated to download a new control program into the pacemaker memory, thereby replacing the old control program previously stored in the pacemaker memory. As the downloading of the new control program takes place (which may require several minutes), the pacemaker includes backup control circuits, or equivalent, for controlling the pulse generator so that stimulation pulses are provided, as needed, until the downloading operation has been successfully completed. In this manner, the control program of the implantable pacemaker is noninvasively updated without having to explant the pacemaker, and without having to cease operation of the pacemaker.
