An endobronchial probe includes a deflector having a bore extending therethrough at an angle to its long axis for passage of a tool. The probe includes a location sensor and an ultrasound imager. A push-pull anchoring system comprises a plurality of guides and wires that can extend beyond the guides and retract within the guides. When extended the wires diverge from the long axis sufficiently to engage a bronchus. The probe includes a balloon disposed on the distal segment contralateral to bore that when inflated urges the mouth of the bore into contact with the bronchial wall.

In one embodiment, there is disclosed a device adapted to be used for percutaneous minimally invasive heart surgery including a catheter having a proximal end and a distal end adapted to be fed through a blood vessel, a pump associated with the distal end of the catheter, and a tool associated with the distal end of the catheter. The tool is adapted to be used for percutaneous minimally invasive heart surgery. Examples of the tools include a tool for excising natural tissue, a tool for excising a natural valve, and anthretomy device, a prosthetic valve delivery system, a balloon catheter, a knife, a stapler, and a screwdriver. The pump is used for maintaining blood flow around the site of the heart surgery while the surgery is taking place without the use of cardiopulmonary bypass.

An apparatus and method to perform therapeutic treatment and diagnosis of a patient's vasculature through the use of an intravascular device having an optical fiber disposed therein. In an embodiment, the apparatus includes an intravascular device to perform a therapeutic treatment and at least one optical fiber disposed through the intravascular device. The optical fiber is configured to provide diagnostic information before, during, and after the therapeutic treatment. In an embodiment, diagnostic information includes vessel and blood characteristics such as hemodynamic characteristics, hematological parameters related to blood and blood components, and thermal parameters of the vasculature.

A method of treating vulnerable plaque comprising intentionally damaging or rupturing the vulnerable plaque using a wingless balloon which is inflated from a wingless unexpanded diameter to a limited expanded diameter. This process produces significant increase in ECM synthesis at the site of the damage or rupture. As a result, the method strengthens the vulnerable plaque while minimizing or avoiding damage to the surrounding wall of the body lumen or damaging a stable plaque mistakenly believed to be a vulnerable plaque. The method of the invention is particularly useful in treating a fibroatheroma type of vulnerable plaque. In one embodiment, the balloon is self-limiting such that it expands compliantly at initial inflation pressures, and above nominal pressure it expands noncompliantly. In an alternative embodiment, the balloon is inflated using a diameter-limiting device, such as a device which limits the inflation pressure or the volume of inflation fluid in the balloon.

An intravascular device for locating a coronary sinus of a patient's heart and method of using the same. In one embodiment, the intravascular device includes an elongated member having at least one lumen and at least one optical fiber coupled to the elongated member. The optical fiber has a distal end configured to contact with a blood flow within the patient's heart and a proximal end coupled to a laser Doppler velocimetry system for performing a plurality of laser Doppler velocity measurements of the blood flow within the heart to locate the coronary sinus of the heart. Incorporating LDV technology into an intravascular device such as a catheter or a guidewire system provides diagnostic information of the physiologic environment of the lesion before, during, or after a therapeutic procedure.
