Implantable medical systems enter an exposure mode of operation, either manually via a down linked programming instruction or by automatic detection by the implantable system of exposure to a magnetic disturbance. A controller then determines the appropriate exposure mode by considering various pieces of information including the device type including whether the device has defibrillation capability, pre-exposure mode of therapy including which chambers have been paced, and pre-exposure cardiac activity that is either intrinsic or paced rates. Additional considerations may include determining whether a sensed rate during the exposure mode is physiologic or artificially produced by the magnetic disturbance. When the sensed rate is physiologic, then the controller uses the sensed rate to trigger pacing and otherwise uses asynchronous pacing at a fixed rate.

Methods and implantable medical systems that implement exposure modes of therapy that allow for continued operation during exposure to a magnetic disturbance

A method of employing an implantable medical device to detect pacemaker mediated tachycardia (PMT) and adjusting a parameter (e.g. PVARP) in response to confirmation of the PMT. The method comprises, with a processor located in the implantable medical device, (1) measuring a first Vp-As (VA1), (2) lengthening SAV by 50 ms for 1 beat in response to measuring VA1, (3) measuring a second Vp-As (VA2), (4) returning to a normal SAV, (5) measuring a third Vp-As (VA3), (6) determining whether VA1=VA2 and VA1=VA3, (7) in response to determining whether VA1=VA2 and VA1=VA3, confirming presence of PMT, and (8) in response to confirming presence of PMT, PVARP is extended.

Pacemaker mediated tachycardia detection and intervention

A system and method is provided for reliably indicating that an implantable medical device is in need of replacement. An implantable medical device includes a battery and a replacement indicator timer. The battery provides power to the implantable medical device. The replacement indicator timer counts a replacement time period to a determined replacement date for the implantable medical device. The replacement indicator timer starts the counting when an operational characteristic of the battery reaches a selected value.

An implantable medical device operates with an algorithm that promotes intrinsic conduction and reduces ventricular pacing. The IMD monitors the occurrence of necessary ventricular pacing and takes certain actions based upon whether this occurrence has been relatively high or relatively low. When noise is detected, asynchronous pacing is provided when the occurrence is relatively high and is not provided when relatively low. When atrial threshold testing is performed, the incidence will determine which methodology is utilized.

An implantable cardiac pacing device delivering minimum ventricular pacing and an associated method control intervals timed by the device during sensing and pacing. An atrial-only pacing mode is set in response to sensing intrinsic ventricular events in the ventricular chamber. A first post-ventricular atrial refractory period is set following each of a plurality of ventricular events sensed in the ventricular chamber, and atrial events each being sensed during first post-ventricular atrial refractory periods are detected. A second post-ventricular atrial refractory period is set in response to detecting the atrial events each being sensed during the first post-ventricular atrial refractory period.
