A system and method for guideline-based discharge planning of a patient from a facility, comprising determining via a processor whether a patient meets predetermined discharge criteria from the facility, generating a first signal indicative of the patient's risk of re-admission to the facility, generating a second signal indicative of a plan for providing care for the patient, generating a third signal indicative of a status of the patient for a post-discharge care report, and generating a fourth signal indicative of instructions to discharge the patient from the facility.

The method comprises: displaying a list of psychological determinants, receiving a selection of a group of psychological determinants chosen from the list of psychological determinants, generating a list of behavioral models using the group of psychological determinants, receiving a selection of at least one selected behavioral model from the list of behavioral models, determining a time line for the global structure of the coaching plan using the at least one selected behavioral model, compiling a coaching object file using the time line, linking the coaching object file to a library of multi-media content to resolve unresolved symbolic links of the time line, integrating the coaching plan into the care plan, and writing the care plan to the computer-readable storage medium.

Computer-implemented method of manufacturing a computer-readable storage medium for a remote patient management system

A communication system includes a HAVi-based in-home networkand a remote deviceoperative to communicate with an intermediate deviceof the in-home network via Internet. The remote device loads a HAVi applet (Havlet)for controlling at least one of the in-home devices using HAVi. The remote device also loads an HAVi API (HJA) emulatorwhich emulates HJA. The HJA emulator provides an interface for the Havlet and communicates with a modulein the intermediate device. The intermediate device includes the actual HJAwhich provides the actual interface functionality for the HAVi functions used by the Havlet. The interface functionality is provided by controlling the intermediate device and/or communicating with other in-home device(s) according to application messages of the in-home application protocol. The intermediate device loads the module for enabling communication between the HJA emulator in the remote device and HJA loaded in the intermediate device, giving a substantially transparent communication path between the portable application program in the remote device and the HJA in the intermediate device.
