Embodiments described herein include a process for detecting energy tool activations. The process can begin by receiving a surgical video of a surgical procedure involving energy tool activations. The process then applies a sequence of sampling windows to the surgical video to generate a sequence of windowed samples of the surgical video. Next, for each windowed sample in the sequence of windowed samples, the process applies a deep-learning model to a sequence of video frames within the windowed sample to generate an activation/non-activation inference and a confidence level associated with the activation/non-activation inference for the windowed sample. As a result, a sequence of activation/non-activation inferences and a sequence of associated confidence levels are generated. The process subsequently identifies a sequence of activation events in the surgical video based on the sequence of activation/non-activation inferences and the sequence of associated confidence levels.

Energy tool activation detection in surgical videos using deep learning

An apparatus includes a shaft assembly, an ultrasonic blade, and a clamp assembly. The shaft assembly includes an acoustic waveguide operable to transmit ultrasonic vibrations to the blade. The clamp assembly includes a clamp arm pivotable toward and away from the blade about a pivot axis, to clamp tissue between the clamp arm and the blade. A rotation feature may provide rotation of the blade relative to the clamp arm about the longitudinal axis of the waveguide. Alternatively, the rotation feature may provide rotation of the clamp arm relative to the blade about the longitudinal axis. The rotation feature may be driven based on pivotal positioning of the clamp arm relative to the blade about the pivot axis. The rotation feature may selectively lock and unlock the angular position of either the blade or the clamp arm about the longitudinal axis at any of a number of predetermined angular positions.

In various embodiments, a tissue thickness compensator can comprise a compressible extracellular matrix and a bioabsorbable material dispersed within the extracellular matrix, wherein the bioapsorption of the bioabsorbable material is configured to leave behind channels in the extracellular matrix. The tissue thickness compensator can also comprise generation means for generating the ingrowth of tissue into the channels. In at least one embodiment, the tissue thickness compensator can comprise dissolvable wicking members which, when dissolved, can leave behind channels in the tissue thickness compensator. Embodiments described herein provide various techniques and systems for building machine-learning surgical tool presence/absence detection models for processing surgical videos and predicting whether a surgical tool is present or absent in each video frame of a surgical video. In one aspect, a process for ensuring patient safety during a laparoscopic or robotic surgery involving an energy tool is disclosed. The process can begin receiving a real-time control signal indicating an operating state of an energy tool during the surgery. Next, the process receives real-time endoscope video images of the surgery. The process simultaneously applies a machine-learning surgical tool presence/absence detection model to the real-time endoscope video images to generate real-time decisions on a location of the energy tool in the real-time endoscope video images. The process then checks the real-time control signal against the real-time decisions to identify an unsafe event and takes a proper action when an unsafe event is identified.

In various embodiments, a surgical stapling instrument can include a handle, a shaft extending from the handle, wherein the shaft defines an axis, and a disposable loading unit which is assembled to the shaft in a direction which is transverse to the shaft axis. Such a connection between the disposable loading unit and the shaft can prevent, or at least inhibit, the disposable loading unit from being unintentionally displaced proximally and/or distally relative to the shaft of the surgical instrument. The surgical stapling instrument and/or disposable loading unit can further include a threaded collar and/or detent assembly configured to hold the disposable loading unit in place. In various embodiments, a disposable loading unit can include a lockout feature which can prevent, or at least inhibit, an expended disposable loading unit from being reassembled to the elongated body of the surgical instrument.

A system configured to control operation of an ultrasonic surgical instrument is disclosed. The system comprises a generator configured to generate a signal, and a computer communicably coupled to the generator. The computer is configured to receive a first set of variables corresponding to a first signal at a first power level of the instrument, evaluate the first set of variables with a model to generate a first output, wherein the first output comprises a value between the first set of variables and a condition set, and wherein the condition set comprises a plurality of conditions that correspond to a plurality of responses, determine the first output meets a first threshold value for a first condition; determine a first response corresponding to the first condition, wherein the first response is a second power level for the instrument, and transmit the second power level to the generator for a second signal.

An apparatus can include a coupler for coupling a robotic arm to a surgical table having a table top on which a patient can be disposed. The coupler can include a first portion configured to couple to a surgical table and a second portion configured to couple to a robotic arm. The second portion may include a post that may translate into the first portion. The first portion may comprise a locking mechanism having one or more stages to constrain movement of the second portion relative to the first portion in six degrees of freedom. The coupler can thus provide secure coupling of the robotic arm to the surgical table.

The arm cart can include an arm container and a base. The arm container can be configured to receive and contain one or more robotic arms. The arm cart can include a first coupling member configured to engage with a second coupling member associated with a surgical table such that, when the first coupling member is engaged with the second coupling member, the one or more robotic arms can be releasably coupled with the surgical table. The elongate shaft assembly comprises a first drive member and a second drive member. The end effector comprises an elongate channel configured to support a staple cartridge therein, an anvil movably coupled to the elongate channel, and a cutting member. When the handle system is engaged with the first drive member, a first actuation of the handle system advances the cutting member a first distance. When the handle system is engaged with the second drive member, a second actuation of the handle system moves the cutting member a second distance. A stapling system including a housing that has a first rotatable disk configured to transmit a first operative motion and a second rotatable disk configured to transmit a second operative motion is disclosed herein. The drive system is responsive to the first operative motion from the first rotatable disk to perform a closing motion of the end effector and responsive to the second operative motion from the second rotatable disk to perform a firing stroke. A control system is configured to assure that the closing motion and the firing stroke are separate and distinct, wherein the control system prevents the firing stroke from occurring prior to the completion of the closing motion.

A method of operating a surgical stapling device comprising a lockout for preventing the surgical stapling device from operating. The method comprises contacting a first authentication key protruding from a surgical staple cartridge in actuation contact with the lockout in the surgical stapling device to move the lockout into an intermediate position, contacting a second authentication key protruding from the surgical stapling cartridge distal to the first authentication key into other actuation contact with the lockout in the surgical stapling device to move the lockout from the intermediate position into an unlocked position, and operating the surgical stapling device.

Several solutions are described for making it easier to move a surgical robotics patient table, either from rest or to turn it. The table has a base, a support, and a table top on which the patient is supported. A number of casters are mounted to a body of the base. In one case, controllable jacks lift the body to release the casters thereby allowing them to swivel to a desired orientation. In another case, a drive assembly is rotatably coupled to the body of the base and includes an electric drive motor and a drive wheel to drive the table across the floor. Other aspects are also described and claimed.
