Embodiments described in this disclosure include a process for collecting energy tool usage data from surgical videos and using such data for post surgery analysis. The process can begin by receiving a plurality of surgical videos of a surgical procedure involving an energy tool. For each surgical video in the plurality of surgical videos, the process detects a set of activation events in the surgical video, wherein each detected activation event includes an identified starting timestamp and a duration. The process further extracts a set of energy tool usage data based on the set of detected activation events, and then stores the extracted set of energy tool usage data in a database indexed based on a set of energy tool usage metrics. Next, in response to a user search request, the process returns the stored energy tool usage data that matches the search request from the database.

Post-surgery analysis of energy tool usage in surgical videos using deep learning

Embodiments described herein include a process for detecting energy tool activations. The process can begin by receiving a surgical video of a surgical procedure involving energy tool activations. The process then applies a sequence of sampling windows to the surgical video to generate a sequence of windowed samples of the surgical video. Next, for each windowed sample in the sequence of windowed samples, the process applies a deep-learning model to a sequence of video frames within the windowed sample to generate an activation/non-activation inference and a confidence level associated with the activation/non-activation inference for the windowed sample. As a result, a sequence of activation/non-activation inferences and a sequence of associated confidence levels are generated. The process subsequently identifies a sequence of activation events in the surgical video based on the sequence of activation/non-activation inferences and the sequence of associated confidence levels.

Energy tool activation detection in surgical videos using deep learning

An apparatus includes a shaft assembly, an ultrasonic blade, and a clamp assembly. The shaft assembly includes an acoustic waveguide operable to transmit ultrasonic vibrations to the blade. The clamp assembly includes a clamp arm pivotable toward and away from the blade about a pivot axis, to clamp tissue between the clamp arm and the blade. A rotation feature may provide rotation of the blade relative to the clamp arm about the longitudinal axis of the waveguide. Alternatively, the rotation feature may provide rotation of the clamp arm relative to the blade about the longitudinal axis. The rotation feature may be driven based on pivotal positioning of the clamp arm relative to the blade about the pivot axis. The rotation feature may selectively lock and unlock the angular position of either the blade or the clamp arm about the longitudinal axis at any of a number of predetermined angular positions.

Embodiments described herein provide various techniques and systems for building machine-learning surgical tool presence/absence detection models for processing surgical videos and predicting whether a surgical tool is present or absent in each video frame of a surgical video. In one aspect, a process for ensuring patient safety during a laparoscopic or robotic surgery involving an energy tool is disclosed. The process can begin receiving a real-time control signal indicating an operating state of an energy tool during the surgery. Next, the process receives real-time endoscope video images of the surgery. The process simultaneously applies a machine-learning surgical tool presence/absence detection model to the real-time endoscope video images to generate real-time decisions on a location of the energy tool in the real-time endoscope video images. The process then checks the real-time control signal against the real-time decisions to identify an unsafe event and takes a proper action when an unsafe event is identified.

In various embodiments, a surgical stapling instrument can include a handle, a shaft extending from the handle, wherein the shaft defines an axis, and a disposable loading unit which is assembled to the shaft in a direction which is transverse to the shaft axis. Such a connection between the disposable loading unit and the shaft can prevent, or at least inhibit, the disposable loading unit from being unintentionally displaced proximally and/or distally relative to the shaft of the surgical instrument. The surgical stapling instrument and/or disposable loading unit can further include a threaded collar and/or detent assembly configured to hold the disposable loading unit in place. In various embodiments, a disposable loading unit can include a lockout feature which can prevent, or at least inhibit, an expended disposable loading unit from being reassembled to the elongated body of the surgical instrument.

The system comprises a generator configured to generate a signal, and a computer communicably coupled to the generator. The computer is configured to receive a first set of variables corresponding to a first signal at a first power level of the instrument, evaluate the first set of variables with a model to generate a first output, wherein the first output comprises a value between the first set of variables and a condition set, and wherein the condition set comprises a plurality of conditions that correspond to a plurality of responses, determine the first output meets a first threshold value for a first condition; determine a first response corresponding to the first condition, wherein the first response is a second power level for the instrument, and transmit the second power level to the generator for a second signal.

An apparatus can include a coupler for coupling a robotic arm to a surgical table having a table top on which a patient can be disposed. The coupler can include a first portion configured to couple to a surgical table and a second portion configured to couple to a robotic arm. The second portion may include a post that may translate into the first portion. The first portion may comprise a locking mechanism having one or more stages to constrain movement of the second portion relative to the first portion in six degrees of freedom. The coupler can thus provide secure coupling of the robotic arm to the surgical table.
