A dental instrument includes a pneumatic pressure intensifier that supplies a handpiece having an orifice to generate a high velocity liquid jet. A pneumatic system includes valves to supply the pressure intensifier and control the output to the handpiece. A jet forming assembly includes an orifice mount in the handpiece, and a guide tube assembly secured to the orifice mount. The guide tube assembly is variably positioned to select the minimum distance from the jet orifice to the tissue target, so that the jet force may be controlled.

Apparatus for dental treatment using high pressure liquid jet

A medical device that can emulsify tissue with a heated fluid. The device may include an active heating element that is located within an inner channel at a distal end of a cannula. The cannula is adapted to be inserted into tissue such as a cornea. The active heating element can provide heat to a fluid that flows through the inner channel. The heat can be transferred into a bolus of fluid that is periodically generated by a pump. The heated bolus is then directed onto tissue.

Method and apparatus for thermal emulsification

An apparatus for guiding a high speed liquid jet used in forming ophthalmic incisions includes an instrument body having an elongated rectangular portion and a tapered end portion. An applanation suction ring assembly is secured to the tapered end, and is provided with a stepped opening extending axially therethrough. A carriage assembly extends longitudinally within the instrument body, and a pair of high precision crossed roller slides join the carriage and body to permit lateral translation of the carriage. A micro-motor assembly secured within the body is joined to a pinion gear which engages a rack gear joined to the carriage to drive the carriage in lateral translation. The carriage includes a proximal connector for a high pressure fluid hose, and a jet nozzle assembly extending from the distal end. A plurality of guide members are provided, all having common structural features adapted to engage the stepped opening of the distal end of the instrument body. Each guide member includes a generally cylindrical body and a central portion comprised of a reticle and applanation plate, with differing guide members having applanation plates of various thicknesses. The guide member may include an adaptive applanator member that is selectively variably curved. The cylindrical body also includes an annular vacuum suction ring to selectively secure the device on the eye. The guide member has a jet slot extending in the sidewall thereof to receive the jet nozzle for lateral translation in a chordal path. An aspiration groove is formed within the guide member in opposition to the slot to aspirate the liquid and tissue debris from the incision.

Fluid jet keratome apparatus and method for refractive surgery

A method and apparatus for dental procedures using a dental hydrojet tool having a cannula extending therefrom. The cannula is connected to a source of high pressure liquid, and delivers a high velocity, high pressure jet. For root canal procedures, the cannula is directed through an opening formed in the crown of the tooth, and the hydrojet is directed at the pulp, nerve and vascular tissue within the interior chamber. The jet may comprise medical-grade saline, disinfectant, antibiotic or abrasive solutions. All the soft tissue within the tooth is cut, excised, emulsified, and aspirated out of the tooth. The time required to excise, emulsify, and aspirate all the soft tissue within a typical molar is on the order of 10-240 seconds. The interior chamber of the tooth is completely aspirated and verified visually for completion of removal of all soft tissue, and then packed with an appropriate filler material, and the opening in the top of the tooth is filled or provided with a prosthetic crown. The fluid jet may comprise a pulsed jet formed by a pulse intensifier device in the handpiece of the dental hydrojet tool, or a steady stream jet emanating from the orifice. The jet may be used for caries removal, plaque and calculus removal, and soft tissue surgery.

Method and apparatus for dental treatment using high pressure liquid jet

A cylindrical housing is secured to the tapered end, and is provided with a stepped opening extending axially therethrough. A jet carrier includes a longitudinally extending body, and a pair of high precision crossed roller slides join the carrier and body to permit lateral translation of the jet carrier. A micro-motor assembly secured within the rectangular body portion is joined to a pinion gear which engages a rack gear joined to the jet carrier to drive the jet carrier in lateral translation. A plurality of guide members are provided, all having common structural features adapted to engage the stepped opening of the distal end of the instrument body. Each guide member includes a generally cylindrical body and a central portion comprised of a reticle and applanation plate, with differing guide members having applanation plates of various thicknesses. Hydrokeratome for refractive surgery

A method and apparatus for the excision and removal of tissue, such as the lens of the eye includes a pencil-like handpiece having a cannula probe extending from the distal end thereof. The probe includes an inner jet tube to direct a high pressure jet of fluid toward a tissue target, and an outer concentric aspiration tube to aspirate and remove fluid and tissue. The jet tube is recessed proximally within the concentric aspiration tube, and the aspiration tube has an end area significantly larger than the end area of the jet tube. These factors cooperate so that the negative pressure exerted by the aspiration tube creates a suction force that offsets and exceeds the force of the fluid jet. The jet tube is connected to a fluid pressure system including a positive displacement pump, a pressure regulator, safety release, control valve, and a pulse former. The jet tube emits pulses of high pressure fluid that impinge reiteratively on the target, creating shock waves that fracture and emulsify the lens tissue, and the fluid also acts as a solvent to transport the emulsified tissue into the aspiration tube. The handpiece also includes a vacuum bypass port disposed to be selectively occluded by a finger of the surgeon, so that vacuum pressure may be released immediately when needed.
