This disclosure describes a variety of transitional or terminal components that may be implanted as part of a spinal fixation construct to decrease the potential for subsequent development of junctional disease. The fixation construct may extend any number of levels from a single level construct to a long construct spanning multiple spinal levels and multiple spinal regions from the lumbosacral to cervical regions, and with any variety of combination of anchors, rods, and connectors. Terminal and/or transitional components may be utilized at the caudal and or cephalad ends of the fixation construct to reduce stresses endured by the construct adjacent pathology and prevent or reduce incidence and degree of junctional disease.

Spinal fixation constructs and related methods

A retention system for maintaining a screw to a vertebral plate is described herein. One or more screws extend through apertures within the vertebral plate. A cavity is positioned adjacent to and overlaps into the aperture. A ring is positioned within the cavity and held in position by a cap. The cap attaches to the plate to prevent removal of the ring. The ring is deflectable between a first shape to allow the screw to be inserted and removed from the aperture, and a second shape to prevent screw back-out from the aperture.

Method and apparatus for retaining screw in a plate

An anterior fixation system includes a plate defining a plurality of screw holes, a number of screws and a number of locking assemblies for fixing the screws to the plate. The system includes two bone screws, a fixed angle screw and a variable angle screw, that are configured to extend through the same screw openings in the fixation plate. The surgeon can select either the fixed or variable angled screws to be implanted with a single plate and can place either type of screw into any of the screw holes along the plate. The fixation plate can include several screw holes in various patterns that provide the surgeon with great flexibility in the placement of bone screws depending upon the spinal anatomy and pathology. Also contemplated is a locking assembly to lock one or more bone screws within a respective screw hole.

Anterior cervical plating system

A prosthesis comprising a component for engaging a surface of a vertebral body is disclosed. The component comprises a bone-engaging surface and an outer surface that is not bone-engaging, wherein the bone-engaging surface is configured to engage a portion of the surface of the vertebral body, wherein the vertebral body has been configured to engage with the bone-engaging surface, and wherein after the component is engaged with the vertebral body, at least at a location of an area of a junction between a vertical surface and an endplate surface of the vertebral body, the outer surface of the component has a shape that is substantially similar to an anatomical shape of the vertebral body.

Prosthesis connection mechanism

A retaining system for affixing a component to a vertebral body is disclosed. The system comprises a component and a retaining mechanism. The component comprises an inner surface configured to engage at least a portion of an endplate surface of the vertebral body, and an outer surface configured to engage the retaining mechanism, wherein the outer surface of the component comprises a first face directed substantially toward the vertebral body. The retaining mechanism comprises an inner surface configured to engage the outer surface of the component, and an outer surface configured to receive a fastener, wherein the inner surface of the retaining mechanism comprising a second face directed substantially away from the vertebral body.

One embodiment of a hollow spinal spacer () includes a curved anterior wall () having opposite ends (), a posterior wall () having opposite ends (), two lateral walls (), each integrally connected between the opposite ends () of the anterior () and posterior () walls to define a chamber (). The walls () include a superior face () and an inferior face (). The superior face () defines a first opening () in communication with the chamber () and includes a first vertebral engaging surface (). Intervertebral spacers

A method and instrumentation for spinal interbody fusion is disclosed. The instruments and methods are particularly adapted for interbody fusion from a posterior approach to the spine. One instrument is a retractor having a lockable pivotally mounted handle. Another instrument is a template for straddling the dura. A modular distractor is also provided and preferably includes a tapered shaft with a visualization window disposed therein. Yet another instrument is a depth gauge to verify bone opening depth and dimension, preferably including a radiopaque portion. A method contemplates the use of these instruments to prepare a disc space to receive an implant.

Distractor for posterior interbody fusion

A spinal stabilization system comprises an elongated stabilization device including a curved configuration along a longitudinal axis thereof. The elongated stabilization device includes a length and cross-section sized for positioning through a pathway formed from an opening in a lateral mass of a first vertebra and into the first vertebra, through the facet joint formed by adjacent articular surfaces of the first vertebra and an adjacent bony structure, and into the adjacent bony structure. Instruments can be provided to form the pathway and insert the stabilization device into the pathway.

Occipital and cervical stabilization systems and methods

A retention system for maintaining a screw to a vertebral plate. One or more screws extend through apertures within the vertebral plate. A cavity is positioned adjacent to and overlaps into the aperture. A ring is positioned within the cavity and held in position by a cap. The cap attaches to the plate to prevent removal of the ring. The ring is deflectable between a first shape to allow the screw to be inserted and removed from the aperture, and a second shape to prevent screw back-out from the aperture. A method of using the system is also included and comprises positioning the ring within a cavity in the plate, maintaining the position of the ring to the plate by attaching a cap, inserting a screw through the aperture and attaching the plate to a vertebral member, and positioning the ring over the screw head and preventing the screw from backing-out.

Method and instrumentation for posterior interbody fusion

Instrumentation and techniques for preparing vertebral members for insertion of an implant. Foundation instruments provide a reference relative to the vertebral members. The foundation devices guide the placement of one or more instruments that prepare the vertebral members. The instruments operatively align with the foundation instruments in a specific orientation to ensure proper placement relative to the vertebral members. Preparatory instruments may be used to further remove bone from the vertebral members. The preparatory instruments may be used prior to or after the other instruments. An insert is inserted and mounted to the vertebral members after the bone has been removed. A holder may be used for holding and placing the implant relative to the vertebral members.

Technique and instrumentation for preparation of vertebral members

An intervertebral device comprises a first endplate assembly including a first exterior surface, a second endplate assembly including a second exterior surface, and a bone in-growth area located on the first exterior surface. The bone in-growth area includes an osteoconductive surface. The intervertebral device also comprises a bone in-growth indicator located on the first exterior surface and an access guide on the first endplate. The access guide is adapted for directing a revision instrument to the bone in-growth area.

Artificial spinal discs and associated implantation and revision methods

In one embodiment of the invention, an anterior fixation system includes a plate defining a plurality of screw holes, a number of screws and a number of locking assemblies for fixing the screws to the plate. The system includes two bone screws a fixed angle screw and a variable angle screw, that are configured to extend through the same screw openings in the fixation plate. The surgeon can select either the fixed or variable angled screws to be implanted with a single plate and can place either type of screw into any of the screw holes along the plate. The fixation plate according to the invention can include several screw holes in various patterns that provide the surgeon with great flexibility in the placement of bone screws depending upon the spinal anatomy and pathology. The invention further contemplates a locking assembly to lock one or more bone screws within a respective screw hole. In one embodiment, the locking assembly includes a washer that is held to the plate by a staked locking screw. The washer includes an outer surface that overlaps one or more screw holes. The washer is initially loosely held to the plate by the locking screw so that various tools and bone screws can be passed through the screw holes. In one embodiment of the washer, the washer includes cut-outs corresponding to the screw holes, along with a notch and key configuration for setting the locking washer in its locked configuration.

A modular intervertebral prosthesis system includes first and second baseplates and an insert designed to mate to the baseplates. The baseplates include a mounting section and a intervertebral section extending away from the mounting section generally perpendicular thereto. The intervertebral section has at least one aperture passing therethrough. When installed, the insert is disposed between the intervertebral sections of the baseplates and faces the apertures. In this way, the insert is not insulated from the vertebral members, thereby allowing for osteoinduction, osteoconduction, and/or osteointegration, if desired. The insert may take the form of an articulating-type insert or a fusion-type insert, with the baseplates being operative with both types of inserts. After installation of the insert between the baseplates, the insert may be readily removed, at least initially.
