A vessel harvesting system removes a target vessel from a patient for use as a bypass. An elongated harvesting instrument inserts into a body along a path of a target vessel which includes at least one side branch. The harvesting instrument includes a cutter for applying thermal energy to sever and cauterize the side branch. An endoscopic camera captures visible-light images from a distal tip of the instrument within a dissected tunnel around the target vessel. A thermal camera captures thermograms coinciding with the visible-light images to characterize a temperature present at respective surfaces in the tunnel. An image processor (e.g., an electronic controller) renders a video stream including the visible-light images and an overlay depicting the temperatures present on at least some of the respective surfaces when applying the thermal energy. A display presenting the video stream and overlay to a user can be an augmented-reality display.

A vessel harvesting apparatus for removing a blood vessel from a patient includes collection and conditioning (i.e., treatment) of expelled insufflation gas prior to releasing the gas into the air of the operating room. An endoscopic instrument has a distal end with a vessel harvesting tip and has a proximal end with a handle. An insufflation channel is configured to convey an insufflation gas subcutaneously into a dissected space within the patient. A removal channel is configured to evacuate fluidic contents from the dissected space, wherein the fluidic contents include insufflation gas and biological impurities. A processor/separator is coupled to the removal channel to process the fluidic contents to retain at least some of the biological impurities and to exhaust the insufflation gas.

An endoscopic vessel harvester cuts and cauterizes side branches from a target vessel using a cutter member that extends at a distal end of an insertion member. The cutter member has a plate defining a slit for receiving a side branch. A cutting electrode is disposed at a base of the slit and a pair of grounding electrodes are each mounted to an outer surface of the plate on opposite sides of the slit. Each grounding electrode has a raised longitudinal ridge adjoining a portion of the slit including the base of the slit and has a spot cautery wing extending laterally away from the slit with a surface configured to engage a surface of the tunnel. The longitudinal ridges each have an upright jamb surface configured to contact the side branch remotely from the cutting electrode so that it is unnecessary to make a grounding contact on the tunnel.

A device prepares leaflets for cardiovascular valve reconstruction from a pericardial tissue sheet harvested from a patient. A cutting tool is adapted to cut a predetermined pattern having a selected leaflet size. The device is continuously adjustable using a plurality of slidable blade segments forming a closed periphery. Radially-slidable links are slidably attached to respective blade segments. An adjustment disc is coupled to the links, wherein rotation of the adjustment disc varies a radial position of the links to modify an overlapping of the blade segments in order to select a corresponding size of the closed periphery.

A device prepares leaflets for cardiovascular valve reconstruction from a pericardial tissue sheet harvested from a patient. A cutter is adapted to cut a predetermined pattern having a selected leaflet size. A tissue marker automatically aligned with the predetermined cutting pattern is configured to mark suture positions on the leaflet in response to placement of the cutter. Cutting of a leaflet from the tissue sheet and marking of suture positions on the leaflet are obtained concurrently. A set of such devices spanning a variety of leaflet sizes may be provided in a kit that results in an ability to quickly obtain a properly sized and marked leaflet for reconstruction.

A marking/drying tool applies ink to biological tissue using a single handheld device. A cylindrical body retains a felt tip movable between a retracted position within the body and an extended position outside the body for marking the tissue. A gas source has a manual valve to selectably dispensing a drying gas onto the tissue. In a preferred embodiment, an interlock prevents dispensing of drying gas when the tip is in the extended position and prevents the tip moving to the extended position when the manual valve is dispensing the drying gas.

A device for joining individual leaflets for repairing a cardiac valve of a patient has a base and projection fingers rising from the base to support adjacent leaflets for tacking together by sewing overlapping edges of adjacent leaflets prior to emplacement into the patient. The invention provides a cutting tool for excising leaflets from a pericardial tissue sheet according to predetermined sizes and shapes. A leaflet having a desired contour and size is obtained by pressing the tool formed as a cutter block against the tissue sheet. A set of blocks can be provided wherein each cutter block has an outer edge following a respective contour and size for a leaflet, and wherein the outer edge includes a raised cutting edge for pressing through the pericardial tissue sheet. The blocks can be held in a press for assisting in pushing the cutting edge through the tissue.

A vessel dissector for harvesting a target vessel has a tubular member carrying a blunt transparent tip with a terminus for blunt dissection of tissue and a base affixed to the tubular member. An active ring set has first and second ring segments mounted to distal ends of respective manipulator bars in the tubular member. The ring segments juxtapose to define a closed loop with an inner diameter larger than the outside diameter of the tip base. The ring segments are movable between a retracted position nested at the base and respective extended positions distally forward of the terminus. At least one of the ring segments is energizable to cut and cauterize a cylindrical pedicle including the target vessel. The ring segments independently extend longitudinally to provide a variable gap between the ring segments to capture, cut, and cauterize side branches to the target vessel between the ring segments.

An endoscopic harvesting device removes a vessel from a body. The vessel has an anterior side closest to the skin. A sheath extends in a longitudinal direction with a dissector tip for advancing along the vessel substantially along the anterior side to create a flanking tunnel spaced away from the vessel. A ring-shaped blade is mounted to the sheath and is disposed in a plane substantially perpendicular to the longitudinal direction and proximal of the dissector tip. The blade forms a lateral loop to encircle the vessel from the flanking tunnel. The blade comprises an inductively-heated ferromagnetic member configured to make a vasiform cut including a pedicle around the vessel as the sheath advances.

A vessel dissector for harvesting a target vessel from a donor has a generally cylindrical tip body with a sloping channel formed along a longitudinal side. A channel bottom provides a channel depth that decreases from a distal channel end to a proximal channel end. The tip body has a crescent-shaped forward lip extending distally from the channel. An arcuate collar is slidably disposed in an arcuate recess within the tip body to selectably bridge the channel at the distal end of the channel to form a ring profile with the forward lip. A first ferromagnetic heating element is disposed along a radially outward surface of the forward lip. A second ferromagnetic heating element is disposed on a distal edge of the arcuate collar. The first and second ferromagnetic heating elements are adapted to be energized simultaneously to make a vasiform cut including a pedicle around the target vessel.

An endoscopic vessel harvester includes a vessel keeper for capturing a target vessel within a tunnel that has been dissected along the target vessel. A cutter member cuts and cauterizes side branches while the target vessel is slidably captured in the vessel keeper. A spray nozzle carried by the vessel keeper delivers a preservative fluid via a manual control valve and a conduit. The preservative fluid is sprayed on the target vessel proximate a respective side branch immediately after being cauterized. Thus, a harvested vessel is bathed in preservative fluid prior to being actually removed from the body, enhancing endothelial preservation from cascading events.

Apparatus for marking a blood vessel for use as a bypass graft in a patient includes a preparation plate for supporting an isolated blood vessel that is in a state for anastomosing to the patient. A stencil plate is configured for overlying the blood vessel and preparation plate, wherein the stencil plate has an opening adapted to expose a portion of the blood vessel to a dye applied through the opening. The stencil plate around the opening is adapted to shield the blood vessel from the dye. A dye applicator is configured to direct a metered amount of the dye through the opening and onto the exposed portion of the blood vessel, preferably as a mist. Preferably, the opening is a lineal slot for creating a guide line extending longitudinally along an outer wall of the blood vessel.

A vessel harvesting apparatus is provided for removing a vessel from a patient. An endoscopic instrument has a distal end with a vessel harvesting tip and has a proximal end with a handle. The endoscopic instrument has an insufflation channel adapted to receive an insufflation gas from a regulated source at the proximal end. The insufflation channel has an outlet at the distal end adapted to expel the insufflation gas subcutaneously in the patient. A damping device has a flexible bladder with an inside space in fluid communication with the insufflation channel. The inside space has a volume that varies between a rest volume and an inflated volume in response to an amount of insufflation gas flowing to or from the flexible bladder.
