Method and instrumentation to sense thermal lesion formation by ultrasound imaging

An electrode device adapted for use in medical devices, such as tissue ablation apparatus, includes a sheet of flexible fabric including at least one surface electrode formed as part of the fabric. More particularly, the sheet is made from a plurality of filaments. At least some of the filaments are made at least partially from an electrically conductive material and cooperate so as to form the electrode. The filaments are formed into a plurality of yarns which are interwoven to form the sheet.

Flexible electrode device and surgical apparatus equipped with same

Stapler and ablation clamping heads can be interchangeably coupled to a hand-operated actuation mechanism using a quick connect/quick release coupling. In the actuation mechanism, a first user-operable lever controls clamping of the jaws of either head, while a second user-operable lever controls the firing of staples in the stapler head. A removable staple holder may be used with the stapler head to allow easy reloading of staples as well as the use of different sizes and type of staples. A corresponding removable anvil may be used as well. A removable electrode holder may be used with the ablation head to allow the use of different electrode configurations. Moreover, a removable electrode holder may be used over the jaws of a stapler head as an overlay.

Impression devices are provided for making an impression of a desired feature. The impression devices include: a flexible bladder having a cavity formed therein, the flexible bladder being capable of conforming to the details of the desired feature; a rheological material, such as a magnetorheological fluid, disposed in the cavity of the flexible bladder; and an activator for applying an external stimulus to the rheological material to change the state of the rheological material and thereby capture an impression of the desired feature.

Methods and devices utilizing rheological materials

An access device for providing access into a hollow organ during an open surgical procedure. The access device includes: a body for insertion into an opening in a wall of the hollow organ, the body having a bore for passage of at least a distal portion of an instrument into an interior of the hollow organ; a valve disposed in the bore for allowing passage of the instrument and substantially preventing a fluid in the interior of the hollow organ from leaking outside the hollow organ; and a mechanism for securing the body to the wall of the hollow organ; wherein the body has a low-profile length in an axial direction of the bore.

Device for providing intracardiac access in an open chest

An apparatus for ablating tissue, the apparatus having first and second opposing jaws operative to compress tissue to be allayed therebetween, the first jaw having a first ablation surface directing ablative energy into the tissue and the second jaw having a second ablation surface reflecting incident ablative energy into the tissue. The ablative energy may be ultrasonic, microwave, cryoablation, radio-frequency, photodynamic, laser, and cautery energy. The instrument may also have a pointed tip for piercing tissue, allowing the instrument to clamp the tissue wall of a hollow organ before ablation. Alternately, the instrument may clamp two or more tissue layers of a hollow organ without piercing prior to clamping an ablation.

High intensity ablation device

An instrument for ablation of tissue. The instrument including: a body having at least one surface for contacting a tissue surface, the at least one surface being substantially planar; an ultrasonic transducer disposed in the body for generating ultrasonic energy and directing at least a portion of the ultrasonic energy to the tissue surface, the ultrasonic transducer being operatively connected to an ultrasonic generator; at least one radio-frequency electrode disposed on the at least one surface for directing radio frequency energy to the tissue surface, the at least one radio-frequency electrode being operatively connected to a power source; and one or more switches for selectively coupling at least one of the ultrasonic transducer to the ultrasonic generator and the at least one radio-frequency electrode to the power source.

Multi-modality ablation device

A system for forming linear lesions in tissue, the system comprising a control unit having an ultrasound vibration driving section, and an ultrasonic applicator operatively connected to the control unit. The ultrasonic applicator comprises an ultrasonic transducer having an acoustic window, and an ultrasonic vibratory element, the ultrasonic vibratory element having a convergent shape with a focus located in the direction of the acoustic window. An air gap is adjacent to the ultrasonic vibratory element in an opposite direction from the focal point. The system further comprises means for controlling the depth of the lesion formed in the tissue.

System for creating linear lesions for the treatment of atrial fibrillation

A method of forming a lesion at a predetermined heart location is provided. An energy-delivering electrode is positioned proximal to the predetermined heart location; a return electrode is positioned in contact with cardiac tissue. An electrosurgical current, capable of delivering energy through the energy-delivering electrode to the predetermined heart location at a magnitude and for a duration effective to form a lesion is established. A system for treating atrial fibrillation by forming transmural lesions and a heart-positioning device including a return electrode are also provided. In one embodiment, one of the electrodes includes at least a portion that is selectively movable between a first, retracted position in which the electrode is adapted to be deployed through tissue, and a second, expanded position in which the electrode is adapted to be placed within a chamber of a heart.

Expandable intracardiac return electrode and method of use

A method of forming a lesion at a predetermined heart location is provided. An energy-delivering electrode is positioned proximal to the predetermined heart location; a return electrode is positioned in contact with cardiac tissue. An electrosurgical current, capable of delivering energy through the energy-delivering electrode to the predetermined heart location at a magnitude and for a duration effective to form a lesion is established. A system for treating atrial fibrillation by forming transmural lesions and a heart-positioning device including a return electrode are also provided.

System and method for performing cardiac tissue ablation

An ablation method and device contacts one surface of a target layer and positions the tissue to ablate entirely through the layer. The tissue may be a wall of the heart (ablated, for example, to form blocking lesions for atrial fibrillation), and ablation may be performed with the heart stopped or beating, and effected by either endocardial or epicardial contact. Access may be through an open incision or a minimally invasive technique involving a small opening through which one or more elongated surgical tools are inserted. Illustratively, an atrial ablation treatment can be performed prophylactically (after CABG), or independently to treat an existing condition. A tool of the present invention has a handle at a proximal end, a bipolar ablation head, and an elongated body interconnecting the handle and the ablation head. A channel or contoured surface fixes and bends the tissue such that inter-electrode conduction paths span the thickness of the layer, thus dependably creating a fully transmural ablation lesion. The oriented transmural energy conduction paths reduce the possibility of inducing coagulation of blood in the heart chamber. One bipolar assembly has an electrode spacing of approximately ten millimeters across a channel of about five millimeters depth. Preferably the ablation head forms a suction holder, connected through internal passages to a vacuum pressure of about 100-500 mmHg, and immobilizes the contacted tissue.

Transmural ablation tool and method

A surgical instrument useful in harvesting blood vessels such as veins and arteries and for manipulating and grasping tissue. The instrument has a pair of jaws and a closing tube to open and close the jaws. One of the jaws is preferably movable between the open and closed positions and has an insert for providing lateral strength and stiffness while providing the flexibility necessary to open and close the jaws.
