A valve catheter is advanced through a patient's vasculature while a prosthetic heart valve is attached to flexible extensions along a distal end thereof. The valve catheter is advanced through a tubular sleeve for exposing the prosthetic heart valve and allowing the prosthetic heart valve to self-expand while the extensions remain attached to the prosthetic heart valve. A release mechanism on a handle portion is actuated for releasing the prosthetic heart valve from the extensions, thereby implanting the prosthetic heart in a native heart valve. If the operator is not satisfied with the initial expansion, the prosthetic heart valve may be reoriented before release from the extensions by retracting the prosthetic heart valve back into the tubular sleeve, adjusting the orientation of the prosthetic heart valve and then re-advancing the valve catheter from the tubular sleeve for allowing the prosthetic heart valve to re-self-expand at a more desirable location.

Peptide conjugates for treating pain

N-type voltage-gated calcium channels (CaV2.2) are critical mediators of neurotransmitter release and are thought to be involved with transmission of nociception. The use of conventional CaV2.2 blockers in pain therapeutics is limited by side effects. Reported herein is a means to suppress both inflammatory and neuropathic pain without directly blocking CaV2.2, but rather by inhibiting the binding of the axonal collapsin response mediator protein 2 (CRMP-2), a protein known to enhance CaV2.2 function. A 15 amino acid peptide of CRMP-2 fused to the protein transduction domain of the HIV tat protein (TAT CBD3) reduced meningeal blood flow induced by activation of the trigeminovascular system, prevented inflammation-induced tactile hypernociception induced by intraplantar formalin and nocifensive behavior following corneal capsaicin application, and reversed neuropathic hypernociception produced by the antiretroviral drug 2',3&#x2032;-dideoxycytidine. Preventing CRMP-2&#x2014;mediated enhancement of CaV2.2 function suppressed inflammatory and neuropathic nociception, providing a method for treating pain and inflammation.

Materials and methods for suppressing inflammatory and neuropathic pain

A heart valve delivery system is provided wherein a prosthetic valve is carried on a valve catheter inside a delivery sleeve. A step balloon protrudes from the delivery sleeve and provides a tapered surface for facilitating advancement through a body vessel. The step balloon also aids in crossing the leaflets of a native valve. After the prosthetic valve is positioned within the native valve, the delivery sleeve is retracted to expose the prosthetic valve. In one embodiment, the delivery sleeve is retracted by the use of a lead screw, which effectuates relative movement between the valve catheter and delivery sleeve. The prosthetic valve is preferably self-expandable. If necessary, the step balloon may be expanded to securely seat the prosthetic valve at the site of the native valve. The prosthetic valve is preferably coupled to the valve catheter by a plurality of flexible extension arms which allow the prosthetic valve to be collapsed after initial deployment such that the prosthetic valve may be repositioned if necessary.

Heart valve delivery system with valve catheter

A process and system for compressing bioprosthetic tissue for use in medical implants. The process involves placing the tissue between two compressive surfaces and applying a force to reduce any nonuniformity of thickness in the tissue, while at the same time reduce the overall thickness. One particularly useful application is the compression of bioprosthetic sheet during the preparation of heart valve leaflets. Bovine pericardium may be compressed to reduce its thickness by about 50%, and then formed into heart valve leaflets. The thinned leaflets have substantially the same absolute strength as those made from uncompressed tissue, and are particular desirable for use in minimally invasive valves which must be compressed into a small profile. The thin tissue enables a reduction in the overall size of the minimally invasive heart valve for delivery. For instance, rolled MIS valves may be reduced in size to as small as 20 mm in diameter. Desirably, a cross-linking step is combined with the compression step to fix the reduced thickness of the tissue. One apparatus may include porous compression substrates through which a cross-linking solution may diffuse so as to perform the fixation and compression simultaneously.
