A system for locally stabilizing an anastomotic site of a vessel of a beating heart during a surgical procedure is disclosed. The system includes bonding a pad to the myocardium and attaching a control arm to the pad, all by use of a bioabsorbable adhesive. The control arm is grasped and manipulated by a retractor thereby stabilizing the anastomotic area during the surgical procedure. The pad is optional and can be ommitted in one embodiment where the control arm has feet that are directly bonded to tissue. The pad may also have barbs or can include suction cups. The pad can include a helix-shaped fastener or a mechanical fastener that can be easily linked or hooked to a surgical instrument for manipulation.

Apparatus and method for tissue and organ stabilization

A method and system for recanalizing a totally or near totally occluded body lumen such as an artery which has an elongated catheter shaft with means such as a cutting or ablating element at the distal end of the shaft to remove occluding material and with ultrasonic imaging means preferably located on the shaft proximal to the cutting or ablating element to allow the operator to maintain the radial position of the distal end of the catheter shaft within the body lumen so that no contact is made between the cutting or ablating means and the wall defining the body lumen. The imaging system preferably includes one or more ultrasonic transducers which emit ultrasound in a direction having a substantial radial component, or a substantial axial component or both. Ultrasonic energy reflected from the wall defining the body lumen is received by transducers which generate signals representing the received ultrasonic energy, which in turn are used to form an image of the wall on a video monitor and the radial location of the distal end of the catheter shaft or the means to remove occluding material. The image projected on the video monitor may then be used by the physician to control the position of the distal end of the catheter shaft and/or the means to remove occluding material within the body lumen.

Treating a totally or near totally occluded lumen

A grant-and-stent combination is disclosed for deploying by an intravascular delivery system at the site of an aneurysm, the combination being expandable from a first diameter to a second, larger diameter to hold open and press the graft against regions of the body lumen just proximal and distal to the aneurysm. The combination is characterized by one or more stents that are flexible due to independently expandable and interconnected cylindrical elements, which provide sufficient radial strength to the combination when the graft-and-stent are being attached to the vessel, but which nonetheless afford sufficient resiliency to allow a great degree of expansion at a rapid rate when the combination is being deployed. Attachment elements in the form of hooks are provided to securely affix the ends of the stent extending distally and proximally of the graft after expansion to the walls of the vessel just distally and proximally of the aneurysm.

Intraluminal stent for attaching a graft

An apparatus and method for temporarily implanting an intraluminal stent in a body lumen and its subsequent removal. A stent is formed from a thin sheet that has been wound around itself into a generally cylindrical roll. The stent has a deployed configuration with a deployed diameter generally the size of the lumen diameter. The stent has a delivery configuration, formed by winding the stent more tightly than the deployed configuration, with the delivery diameter smaller than the deployed diameter in order to allow low profile delivery into the body lumen. The stent will transform from the delivery configuration to the deployed configuration when exposed to body heat. The stent can be deployed by an intraluminal delivery catheter that secures the stent in its delivery configuration during intraluminal delivery of the stent. The stent can be removed from the body lumen via a removal catheter that coils the stent into a sheath at the end of the removal catheter.

Removable stent and method of deployment

The intravascular multi-anchor stent includes a plurality of cylindrical elements that are independently expandable in the radial direction and that are interconnected so as to be generally aligned on a common longitudinal axis, and a plurality of barbs along the entire circumference of the stent that face outwardly when the stent is in an expanded condition. A plurality of connecting elements are provided for interconnecting only the cylindrical elements that are adjacent to each other so that the stent, when expanded in the radial direction, retains its overall length without appreciable shortening.

Multi-anchor stent

An apparatus and method for forming or shaping an intraluminal stent for low profile delivery into a body lumen. A nickel titanium intraluminal stent has a tubular member formed from a plurality of coils which are bent to form lobe-shaped members, like a cloverleaf, to reduce the profile of the tubular member and form an involuted spring stent. The involuted spring stent will transform to the implanted diameter of the tubular member when exposed to body heat. The involuted spring stent can be attached to a tubular graft, or coated with a polymeric material to form an involuted spring graft, both of which can be used to repair aortic aneurysms.

Involuted spring stent and graft assembly and method of use

This invention provides a graft delivery system and methods for its use comprising a prosthetic graft secured to an expandable anchoring member, such as a stent, by a butt-joint attachment means to minimize the system's profile. The delivery system employs a catheter with a relatively small diameter shaft for flexibility and a relatively large diameter delivery base for supporting expansion of the anchoring member. The butt-joint attachment means comprises any means suitable to secure the graft to the anchoring member when deployed within the patient's vasculature and allows the anchoring member and graft to expand during deployment. In a preferred embodiment, the delivery system is configured so that the expansion of the anchoring member pulls the distal end of the graft over the proximal end of the anchoring member and the final expansion of the anchoring member seals the distal end of the graft between the proximal end of the anchoring member and the vessel. One preferred method of use is to deploy a second anchoring member coaxially within the already deployed first anchoring member and graft assembly, overlapping at least the distal end of the graft in order to further secure the graft and minimize flow by.

Delivery catheter and graft for aneurysm repair

An intraluminal stent for implanting in a body lumen in which a plurality of connecting members are deformed radially outwardly to form projecting barbs for attaching the stent to a body lumen. The stent has a first, unexpanded low profile diameter for intraluminal delivery, and a second, larger expanded diameter for implanting in a body lumen in which projecting barbs are formed and which penetrate the body lumen to assist in attaching the stent to the walls of the body lumen.

Expandable stent forming projecting barbs and method for deploying

A system for delivering a probe into a body part to perform a therapeutic or diagnostic procedure therein which includes an elongated probe device which is disposed within a protective sheath and which has a rotary drive unit connected to its proximal extremity to rotate the probe about its longitudinal axis, preferably at a rate above 50 rpms while applying axial pressure thereto to cause the distal end of the probe to penetrate through the patient's tissue to a desired location in the body part where the procedure is to occur. In a presently preferred embodiment, the probe is an optical fiber which is delivered into a male patient's prostate gland through the patient's prostatic urethral wall. The optical fiber is optically connected to a source for laser energy (815 nm) which is emitted from the distal end of the optical fiber to ablate prostatic tissue surrounding the distal end. A sheath is disposed about the optical fiber to support the shaft thereof and to minimize damage to the working channel of a cystoscope through which the device is usually advanced to reach the desired location within the patient's prostatic urethra.

Delivery of intracorporeal probes

An cutter apparatus for removing obstructive tissue has a distal housing having a window for invaginating tissue. An axially reciprocating and rotating cutting blade makes successive longitudinal cuts while slowly rotating across the open window. An axial guide assembly holds the cutting edge of the blade in a cutting path immediately adjacent to the inner surface of the housing. The shape and motion of the cutting blade pares off a generally uniformly thick slice of tissue from the stenosis and directs it toward the interior of the housing. A spiral parting edge opposed to the rotating cutting edge provides clean separation of the tissue slice from the stenosis. The tissue slice is received in a chamber internal to the housing and is retained by the blade and guide assembly for later removal from the patient.

Bi-axial cutter apparatus for catheter

A vascular catheter includes a blade tip at its distal end. In the specific embodiments, a blade is mounted within the blade tip and a mechanism provided for extending the blade transversely when the blade tip is located within a region of stenosis. By extending the blade and axially translating the catheter so that the blade forms an incision within the region of stenosis, improved angioplastic treatment can be achieved.

Catheter and method for making intravascular incisions

An atherectomy device for removing stenosis materials from a vascular vessel is provided. The atherectomy device includes a flexible tubular member, a flexible drive means disposed within the tubular member, and a cutting assembly carried by the distal extremity of the tubular member. The cutting assembly includes a collection chamber which receives a cutting bit carried by the drive means. The cutting bit is rotated to remove materials from the stenosis and cause the removed materials to be withdrawn into the collection chamber as the bit is advanced. A second embodiment of the atherectomy device includes an anchor member coaxially mounted within a tubular cutting blade at the distal end of the catheter. The anchor member is first advanced into a region of stenosis where it becomes embedded, and the tubular blade is subsequently advanced over the anchor member to sever the stenosis. The tubular blade and anchor member may be retracted within the catheter and thus used to withdraw the several material from the blood vessel.

Atherectomy device and method

An atherectomy catheter for removal of plaque buildup from the walls of a blood vessel. The atherectomy catheter includes a cutter assembly disposed at the distal end of a flexible guide tube that may be inserted into a vessel and directed to the occlusion. A proximal actuator assembly is adapted to impart both rotary and axial movement to the cutter. The cutter assembly includes a cutter having a forwardly extending annular cutting surface. A guide wire with a flexible spring tip extends forward of the cutter for guiding the cutter as it travels axially through the artery. All of the removed atheroma materials are collected within the device and a retention member carried by the guide wire and positioned in front of the cutter forms a cap to retain the collected atheroma materials. In a disclosed embodiment, the retention member is expandable, and in one of the preferred embodiments, the retention member may be expanded to engage the vessel walls to anchor the device during the cutting stroke.
