A lead connector arrangement includes a non-cylindrically shaped connector pin coupled to a lead conductor and a connector sleeve assembly for receiving the non-cylindrically shaped connector pin. The connector sleeve assembly includes an insert with an axial bore formed therein that complements the shape of the non-cylindrical connector pin. According to one embodiment, the non-cylindrical connector pin may be provided in the form of a triangular, square, rectangular, or hexagonal shape. The axial bore has a complimentary shape to receive the connector pin. When the connector pin is threaded through the connector sleeve assembly using a pull-wire device, the pull-wire device may be unscrewed from the connector pin without causing axial rotation of the lead conductor when the connector pin is fully inserted within the axial bore.

Medical lead and lead connector system

An implantable medical device (IMD) provides for adaptive timing of the delivery of cardioversion shocks. In particular, the invention IMD provides for an adaptive cardioversion synchronization delay with respect to a cardiac event, such as a sensed P-wave or R-wave. When cardioversion with a first synchronization delay fails to terminate a tachycardia, for example, cardioversion may be attempted again with a second synchronization delay. The IMD may keep track of whether each synchronization delay is effective in terminating the tachycardia, and may employ a historically effective synchronization delay when applying cardioversion therapy to treat a subsequent tachycardia episode.

The disclosure provides methods and apparatus of particular benefit for patients suffering heart failure including cardiac dysfunction, chronic HF, and the like and all variants thereof. According to the disclosure monitoring and therapy delivery for a wide variety of acute and chronic cardiac dysfunctions are described and depicted. Various forms of paired or coupled pacing therapy delivery provided alone or in combination with neurostimulation therapy delivered by both implantable and external apparatus, including defibrillation therapy are also provided herein.

This invention relates to an intraluminal stent having a lumen-wall contacting surface and a lumen-exposed surface wherein the stent comprises a first polymer composition comprising fibrin and wherein the stent is suitable to deliver virus to the wall of a lumen of the body. The invention also relates to methods for making the stent and to methods for delivering nucleic acid to cells accessible from a wall of a body lumen.

Therapeutic intraluminal stents

A physiologic rate responsive pacer which alters the pacer's escape interval in response to the patient's respiratory minute ventilation derived from the electromyogram of selected respiratory muscle groups. The directly detected electromyogram (EMG) signal is amplified and band passed filtered, processed to remove any electrocardiogram (ECG) or pacing impulse signal, full-wave rectified, processed to develop a moving time average signal from which the peak, the maximal slope, and the average slope of the EMG moving time average may be calculated and processed in conjunction with the inspiratory and expiratory times between successive slope detections of the moving time average EMG to develop a rate control signal representative of minute ventilation. The EMG may be selectively picked up from electrodes implanted in or near the parasternal intercostal muscles, the external intercostal muscles, the internal intercostal muscles, the diaphragm, or any other respiratory muscle such as the scalenes, or the sternocleidomastoid, and coupled to conventionally designed or special configuration pacemaker pulse generators and cardiac pace/sense lead systems.

A defibrillation lead for location in a human heart, having an elongated lead body defining a generally straight segment extending from its proximal end and a curved segment extending distally from the straight segment. The curved segment has a sigmoidal configuration including a two or more alternating, generally coplanar curves, and carries an elongated defibrillation electrode extending along its length. A stylet is provided for straightening the curved segment prior to implant.

Transvenous defibrillation lead and method of use

A cardiac pacemaker of the type including a sensor responsive to the patient's metabolic demand for oxygenated blood and which varies the pacing rate in response to the output of the sensor. In the disclosed embodiment, a piezoelectric sensor bonded to the inside surface of the pacemaker's outer shield delivers a pulsatile sensor signal to activity circuitry, where the different peak values of the pulsatile signal correspond to different levels of patient activity. The activity circuit defines a plurality of sensor signal threshold levels. Rate control circuitry increases the pacing rate of the pacemaker by amounts corresponding to the threshold level exceeded by recent peak values of said pulsatile signal; peaks of said pulsatile signal which exceed only the lowest of the thresholds cause the smallest increase in the pacing rate, while sensor signals which exceed higher thresholds cause larger increases in the pacing rate. Several parameters, including upper and lower pacing rate limits and rate-response settings are selectable by means of an external programming device communicating with the pacemaker via radio-frequency telemetry.

Method and apparatus for implementing activity sensing in a pulse

A post-extrasystolic potentiation (PESP) cardiac pacing energy stimulator for applying paired and/or triggered pacing stimulation pulses to the right atrium and/or ventricle incorporating one or more sensors, such as a venous oxygen saturation, ventricular, atrial, or arterial blood pressure, or intracardiac or systemic blood flow sensor, and signal processing circuitry for controlling the frequency of or number of heart cycles between periodic delivery of triggered or paired pacing to induce PESP for the treatment of congestive heart failure or other cardiac dysfunctions. Preferably, a first sensor, e.g., a ventricular or arterial blood pressure or flow sensor, is employed to monitor the performance of the heart, and develop a cardiac performance index (CPI) and a second sensor, e.g., an oxygen saturation sensor positioned in the coronary sinus, is employed to monitor cardiac muscle stress and develop a cardiac stress index (CSI) to balance performance and stress. The PESP stimulator may be incorporated into a dual chamber (DDD) pacing system with or without physiologic rate control and with or without backup cardioversion/defibrillation therapy capabilities or in a separate, single purpose device. The PESP stimulator has particular application in atrial stimulation for augmenting filling of the ventricles.
