A positioning tool for a joint socket cutting instrument or a implant is designed for use with a minimally invasive surgical procedure and in conjunction with a computer assisted surgical procedure. The positioning tool has a longitudinally extending drive shaft having a moveable joint at a first end and a drive coupling for connecting to a power source at a second end. A holder for mounting a cutting tool such a drill or as an acetabular cutting instrument or for mounting an acetabular implant is coupled to the moveable joint at the first end of the drive shaft for movement with respect to the drive shaft. The holder is rotatable about a central axis thereof when the drive shaft is rotated. The drive shaft includes a shaft bearing mounted thereon which is pivotally coupled to the shaft at a fixed longitudinal position and is pivotally coupled to a longitudinally extending first arm having a handle. A tracker system which is capable of being utilized by a computer-aided surgical system is mounted on the first arm. A second arm is provided which is pivotally connected to the holder at a first end and pivotally connected to the first arm at a second end. The resulting four bar linkage allows the holder and the cutting instrument/implant to be manipulated in any position while the known geometric relationship between the tracker and the holder allows the location of the holder to be displayed by the computer on a cathode ray tube with respect to a joint.

Apparatus and method for locating holes to be drilled in natural bone, such as a scapula at a site for a prosthetic implant component, such as a glenoid component to be affixed to the scapula for providing a bearing for a humeral head in a shoulder prosthesis, the apparatus and method including seating a guide block against the scapula at the site for the glenoid component, the guide block having an upper end and a lower end spaced in a longitudinal direction from the upper end, drilling a first hole in the scapula along a first transverse direction by guiding a drill through a first drill guide bore adjacent the upper end of the guide block and extending in the first transverse direction through the guide block, the guide block including a second drill guide bore adjacent the lower end of the guide block and extending in a second transverse direction through the guide block, inserting a locator pin into the first hole in the scapula, with the locator pin projecting from the guide block along one of the first and second transverse directions for reception within the first hole along the first transverse direction, and seating the guide block on the scapula to locate the second transverse direction relative to the scapula, and drilling a second hole in the scapula along the second transverse direction by guiding a drill through the second drill guide bore.

An acetabular shell is implanted at an acetabulum in a deficient pelvis to establish an effective acetabular site with increased support in posterior and superior directions for an acetabular bearing insert to be affixed in place in the acetabular shell for the reception of a femoral head of a femoral component of a prosthetic hip implant, the acetabular shell including a domed portion having a convex outer surface for engaging the acetabulum, a concave inner surface for reception of the bearing insert, the inner surface extending along a spherical contour up to a generally hemispherical extent, and a rim between the outer and inner surfaces, the rim being placed adjacent an equatorial location along the domed portion and including a superior segment, an inferior segment longitudinally opposite the superior segment, a posterior segment and an anterior segment laterally opposite the posterior segment, an anchoring plate integral with the domed portion adjacent the superior segment of the rim, and a supplemental support web extending along the rim from the posterior segment to the plate at the superior segment, the support web projecting beyond the equatorial location for providing supplemental support for the bearing insert in the posterior and superior directions.

Affixation of a glenoid component of a prosthetic shoulder implant is enhanced and forces exerted upon the glenoid component during service are managed by the location and orientation of affixation pegs on the glenoid component, and especially the location of a medially extending superior affixation peg closely adjacent the superior edge of the glenoid component, and the offset orientation of an inferior affixation peg, relative to the superior affixation peg, the glenoid component including intermediate affixation pegs extending medially and spaced from one another in the anterior-posterior direction intermediate the superior affixation peg and the inferior affixation peg.

An improvement in an acetabular cup component of a prosthetic hip implant in which an acetabular shell receives a bearing member within the interior of the acetabular shell and has at least one bone screw hole, the improvement enabling the interoperative selection of implanting the acetabular shell without the employment of a bone screw or with the employment of at least one bone screw for assisting securement of the acetabular shell within the natural bone at the implant site, the improvement including a plug for seating in the bone screw hole to close the bone screw hole, the plug including a fastener arrangement for engaging the acetabular shell to fasten the plug within the bone screw hole, and a sealing arrangement for engaging the acetabular shell essentially to seal the bone screw hole against the migration of debris from the bearing member through the bone screw hole, the fastener arrangement including an operator arrangement for being selectively operated from the interior of the acetabular shell to selectively release the plug from the bone screw hole for removal of the plug through the interior of the acetabular shell to thereby selectively open the bone screw hole for reception of the bone screw.

A modular humeral shoulder prosthesis for implant in a predetermined position in the natural humerus to replace the natural humeral head of a shoulder joint with a prosthetic implant has a humeral head component including a humeral head member with a spherical bearing surface of predetermined radius extending from an origin to the bearing surface, an undersurface extending in a first direction, an elongate post integral with and projecting in a given direction from the humeral head member, the given direction making a first acute angle with the first direction, and a post portion on the post, and a humeral stem component including a platform having an upper surface, a stem integral with and depending from the platform, the stem extending along a longitudinal axis for alignment generally longitudinally along the natural humerus, the upper surface extending in a second direction making a second acute angle with the longitudinal axis, and an elongate recess in the stem, the recess extending along the stem and including at least a recess portion complementary to the post portion for reception of the post portion within the recess portion in an integrated coupled assembly wherein the humeral head component is affixed to the humeral stem component with the undersurface of the humeral head member confronting the upper surface of the platform when the bearing surface is in the predetermined position to replace the natural humeral head.
