An example of an apparatus for percutaneously delivering neurostimulation energy to a patient and sensing from the patient using a test device placed externally to the patient is provided. The apparatus may include a stimulation lead, a sensing reference electrode, a sensing wire, and a connection system. The stimulation lead may be configured to be percutaneously introduced into the patient to place the one or more electrodes in the patient. The sensing reference electrode may be configured to be placed in the patient. The sensing wire may be connected to the sensing reference electrode and configured to be percutaneously introduced into the patient to place the sensing reference electrode in the patient. The connection system may be configured to mate the lead connector and the wire connector and to provide electrical connections between the lead connector and the test device and between the wire connector and the test device.

An adapter to add optical stimulation to a stimulation system includes an adapter body and a connector disposed on the distal end of the adapter body. The connector includes a connector body defining a port and a connector lumen; connector contacts disposed in the connector body and arranged along the connector lumen; and a light source disposed in the connector body. The adapter also includes terminals disposed along the proximal end of the adapter body and conductors extending along the adapter body and electrically coupling the connector contacts and the light source to the terminals. The adapter may be used with an optical stylet that fits into a stimulation lead which is, in turn, coupled to the connector of the adapter. Alternatively, the adapter can include a fiber optic coupled to the light source and configured to extend into the stimulation lead.

A system may include an implantable structure with a plurality of electrodes attached thereto, where the implantable structure is configured to be implanted proximate to a nerve that innervates and is proximate to an organ involved with glucose control. The system may further include a controller configured for use to control which of the plurality of electrodes are modulation electrodes and which of the plurality of electrodes are sense electrodes, a modulation energy generator configured to deliver modulation energy using one or more of the modulation electrodes, and a nerve traffic sensor configured to sense nerve traffic in the nerve using one or more of the sense electrodes. The controller may be configured to determine if the delivered modulation energy captures the nerves based on the sensed neural activity.

The marker may also include the longitudinal band and longitudinal extension without a ring.

An electrical stimulation lead includes at least one lead body having a distal end portion, a proximal end portion, and a longitudinal length. The lead further includes a paddle body extending from the distal end portion of the at least one lead body, electrodes disposed along the paddle body, terminals disposed along the proximal end portion of the at least one lead body, and conductors electrically coupling the terminals to the electrodes. The lead further includes an anchoring device threadably disposed in at least a portion of the paddle body. The anchoring device has a head element and a tissue-engagement element fixed to the head element such that actuation of the head element urges the tissue-engagement element away from or toward the paddle body.

Leads for electrostimulation of peripheral nerves and other targets

An electrical stimulation lead includes a stimulation cuff having an exterior surface and an interior surface that defines a nerve channel having a nerve channel axis. A plurality of electrodes are disposed on the interior surface of the cuff. A longitudinal opening extends through the cuff and further extends along an entire length of the cuff, wherein the opening is operable to receive a target nerve from a region outside of the cuff to within the nerve channel. A mount is disposed on the exterior surface of the cuff and radially offset from the nerve channel axis. A lead body is radially offset from the nerve channel axis and a plurality of conductors extend through the lead body, mount and cuff, with the plurality of conductors electrically coupled to the electrodes. The electrical stimulation lead may include a plurality of slots to permit tissue ingrowth.

A burr hole plug includes a first plug base defining a burr hole aperture and including first grooves; a second plug base to be disposed around the first plug base and including second grooves to receive a portion of an electrical stimulation lead; a cover to be disposed on the second plug base; a cap to be disposed over the burr hole aperture and coupled to the first plug base; and an extension coupled to, and extending away from, the cover. The extension includes conductors and the second plug base and the cover include conductive elements to electrically couple the terminals of the electrical stimulation lead, when disposed in the second grooves, to the conductors of the extension. An alternative burr hole plug includes a lead connector disposed on the cap and having a connector aperture to receive the proximal end of the electrical stimulation lead.
