Intravascular delivery systems, devices, and methods are disclosed herein. A representative delivery device can include an inner shaft defining a lumen extending along a length of the delivery device, an outer shaft surrounding the inner shaft along at least a portion of the length of the delivery device, and a tip portion distal to the outer shaft. The inner shaft can include a recess configured to receive a self-expandable implant. The outer shaft can be retractable relative to the inner shaft, and can include a functional member that provides increased tensile strength to the outer shaft, and a coil. The tip portion can extend to a distal terminus of the delivery device and include a cross-sectional dimension that tapers in a distal direction.

Intravascular delivery systems, devices and methods

A delivery device can provide sequential delivery of a plurality of intraluminal devices held in a compressed state on the delivery device. Delivery platforms on the delivery device can hold an intraluminal device in a compressed position and be positioned between pusher bands that may also be radiopaque markers. A post deployment dilation device can be included. The post deployment dilation device can be a plurality of expansion filaments, a bellows, or a balloon. An intravascular device deployment method can include allowing a self-expanding intravascular device to expand, aligning the post deployment dilation device under the intravascular device, and causing the post deployment dilation device to expand radially to push outward on the intravascular device.

Delivery device and method of delivery

Apparatus for transporting particles through a patient's trachea into the respiratory system include a delivery tube having at least a ventilation lumen and a particle delivery lumen. The delivery tube has a centering device near its distal tip, such as a balloon eccentrically mounted on the tube to position an outlet of the particle delivery lumen near a centerline of the trachea above the carina branching into the right and left bronchus. A proximal end of the delivery tube connects to a source of particles, and a controller may be provided to adjust a rate and/or an amount of the particles delivered to the patient. In a specific example, frozen particles are delivered to control core body temperature. In other instances, the particle may be a medicament or other substance for effecting other therapies or diagnostic procedures.

Apparatus and methods for creating tissue necrosis include an energy delivery apparatus that can be positioned adjacent a target treatment site such as a vessel without direct contact with the treatment site tissue. Collimated energy is then directed to the vessel to create necrotic regions in the tissue. Exemplary use in renal vessels creates necrotic regions in adjacent nerves which can alleviate hypertension in a patient.

The systems and methods employ a use register sized and configured to be carried by the medical instrument, which comprises a memory field for recording a start date and time at an instance of first operation of the medical instrument. The systems and methods also employ a use monitoring controller, which is adapted and configured to be coupled to the use register. The use monitoring controller includes a start validation function that compares the start date and time to a present real date and time and provides a start validation output based upon the comparison. The use monitoring controller also includes an enablement function that enables operation of the medical instrument only if the start validation output meets prescribed criteria.

Ultrasound energy is delivered to an ultrasound transducer. The ultrasound energy is delivered at an output frequency that varies over time within a range of output frequencies, and a tuning function selects from within the range an operating output frequency for the ultrasound transducer based upon preprogrammed selection rules. The ultrasound energy is delivered to the ultrasound transducer at or near the operating output frequency selected, to perform a therapeutic or diagnostic function for a specified treatment time, after which operation of the ultrasound transducer is disabled.

Methods and apparatus are provided for loading therapeutic materials into brachytherapy needles. The apparatus comprises a loading tube with a lumen and two transverse slots, and radioactive seed and spacer cartridges slidably received within the transverse slots. The apparatus may be used in conjunction with a standard brachytherapy plunger and needle coupled to, respectively, the proximal and distal ends of the loading tube. The plunger dislodges seeds and spacers from the cartridge chambers to load the needle with a predetermined packing arrangement.

In a preferred embodiment, the apparatus comprises a material eluting a bioactive substance held in place within the patient's vessel by an anchor. The anchor and eluting material are sized and/or collapsible from a delivery configuration, in which the anchor and material may be delivered into the patient's vasculature within a delivery sheath, to a deployed configuration, wherein the anchor engages an interior wall of the patient's vessel. The eluting material may elute recombinant genes, drugs, or any other bioactive substance.

A steerable percutaneous catheter for guiding at least one functional device to selected surfaces of a body for treatment thereon, particularly adapted for laser-assisted Percutaneous Transmyocardial Revascularization (PTMR). The steerable catheter has a handle portion at its proximal end and a controllably deflectable end portion at its distal end. The distal end of the catheter and the distal end of the functional device maintain alignment automatically during deflection. The invention also includes a surface contact detection system to detect contact with an interior body surface, such as the heart wall.

An elongated steerable catheter for placement within a heart chamber, organ aperture or other body opening and having at least one center tube with hollow passageway for guiding a laser delivery means or other functional device to selected surfaces of a heart chamber, organ aperture or other body cavity for laser or other treatment thereon, particularly adapted for laser-assisted percutaneous transmyocardial revascularization (TMR), is disclosed herein. The steerable catheter has a handle portion at its proximal end and a controllably deflectable end portion at its distal end. The elongated center tube has a distal end, in the region where a curvature is to be formed, and a shim anchor sleeve is slidably disposed over the center tube. The shim anchor sleeve is attached to the inside wall of the outer jacket and coupled to the distal end of the center tube with a bendable shim member which extends between the distal tip of the steerable catheter and the sleeve over the center tube. Opposite the shim is a guide for a pull cable, the pull cable attached to the distal end of the steerable catheter and extending through the guide to the handle. Thus, the shim is maintained radially opposite the pull cable with the center tube in between. An outer jacket has, in a preferred embodiment, distinct sections of different stiffness or durometer. A distal, more flexible portion is coupled to a proximal, stiffer portion. The shim anchor sleeve is positioned at or near the junction of two portions of the outer jacket. Thus, the center tube moves freely through the shim anchor sleeve. Adjacent the handle, the outer jacket terminates at the catheter base. The pull cable extends past the catheter base, through a deflection housing tube, and terminates in a cable stop. Rotation of a deflection knob threadably mounted onto the deflection housing tube will cause the pull cable to be pulled backward, or the outer jacket to be pushed forward, relative to each other, thereby inducing deflection of the distal end of the steerable catheter.
