A device including a clot engaging device having a collapsed delivery configuration and an expanded deployment configuration. The device can include a plurality of struts defining a plurality of cells; a seam extending along at least a portion of a length of a distal end of the clot engaging device, wherein the seam comprises a pattern configured for gripping a clot and extends between cells of the plurality of cells; and a distal scaffolding region being distally tapered, wherein the distal scaffolding region comprises cells smaller than cells of a remainder of the clot engaging device.

The outer elongate body has a plurality of clot receiving openings and a plurality of clot engaging regions. The struts can be radially expandable from a first to a second radially expanded state in a deployed configuration. An actuation cable can be connected to a plurality of connections of the engaging framework of struts to apply an expansion force, by the struts, as the struts are actuated by the actuation cable from the first radially expanded state to the second radially expanded state. In the first radially expanded state, a distal end of the struts is spaced a first axial distance away from the shaft, and wherein in the second radially expanded state, a distal end of the struts is spaced a second axial distance, different than the first axial distance, away from the shaft.

A clot retrieval device for removing an occlusive clot from a blood vessel. The device includes a clot engagement section having a constrained delivery configuration and an expanded deployed configuration, wherein in the expanded deployed configuration. The clot engagement section can include a plurality of peak sections configured to vary contact pressure between the clot and the device along a length of the device; and a plurality of troughs between respective peak sections comprising a clot reception space. Each peak is configured to exert a relatively high compressive force on the clot whereas each trough is configured to exert little or no compressive force on the clot.

An endovascular medical system for use with a clot located in a target vessel. The system including a guidewire formed by a core wire. The guidewire extending in a longitudinal direction from a proximal end to an opposite atraumatic clot-circumventing configured distal end; and a lateral direction of the atraumatic clot-circumventing configured distal end of the guidewire being defined transverse to the longitudinal direction of the guidewire. The atraumatic clot-circumventing configured distal end is designed to prevent injury to an inner wall of the target vessel and prohibit insertion of any portion of the guidewire into a perforator vessel.
