An apparatus and method for characterizing a region of interest (ROI) including measuring position and orientation data within the ROI; and generating a geometric data set to include one or more of: length, bifurcation location, angle and curvature characteristics of the ROI. Also, sequentially taking an image of a tool within the ROI; comparing tool dimensions with ROI dimensions; and estimating diameter, length, take-off angle, and/or tortuosity characteristics based on the comparisons.

In an implantable medical device for monitoring glucose concentration in the blood, a blood-glucose concentration analysis is performed using correlations of blood-glucose concentration with measures of metabolic oxygen consumption including oxymetric, and/or temperature. Analysis of electrocardiographic data is used in a parallel method to detect and/or confirm the onset and/or existence and/or extent of hypoglycemia and/or hyperglycemia. Blood-glucose concentration calculation is enhanced by using the combination of the oxygen metabolism analysis and electrocardiographic analysis.

A cardiac analysis system is provided that includes an implantable medical device (IMD), at least one sensor, and an external device. The IMD has electrodes positioned proximate to a heart that sense first cardiac signals of the heart and associated with a clinical ventricular tachycardia (VT) event and second cardiac signals associated with an induced VT event. The sensor measures first and second cardiac parameters of the heart associated with the clinical and induced VT events, respectively. The external device is configured to receive the first and second cardiac signals associated with the clinical and the induced VT events and the first and second cardiac parameters associated with the clinical and the induced VT events. The external device compares the first and second cardiac signals and compares the first and second cardiac parameters to determine if the clinical and induced VT events are a common type of VT event.

Cardiac analysis system for comparing clinical and induced ventricular tachycardia events

Embodiments of the present invention are directed to implantable systems, and methods for use therewith, that monitor and modify a patient's arterial blood pressure without requiring an intravascular pressure transducer. In accordance with an embodiment, for each of a plurality of periods of time, there is a determination one or more metrics indicative of pulse arrival time (PAT), each of which are indicative of how long it takes for the left ventricle to generate a pressure pulsation that travels from the patient's aorta to a location remote from the patient's aorta. Based on the one or more metrics indicative of PAT, the patient's arterial blood pressure is estimated. Changes in the arterial blood pressure are monitored over time. Additionally, the patient's arterial blood pressure can be modified by initiating and/or adjusting pacing and/or other therapy based on the estimates of the patient's arterial blood pressure and/or monitored changes therein.

Implantable systems and methods for use therewith for monitoring and modifying arterial blood pressure without requiring an intravascular pressure transducer

Techniques are provided for controlling ventricular pacing during an episode of atrial fibrillation (AF) for use by a pacemaker, implantable cardioverter-defibrillator (ICD) or other implantable medical device. In one example, upon detection of AF, the underlying intrinsic ventricular rate of the patient is determined prior to delivering any ventricular pacing. Then, a ventricular pacing procedure&#x2014;such as dynamic ventricular overdrive (DVO) pacing&#x2014;is activated to reduce ventricular rate variability to mitigate the adverse effects of AF. The ventricular pacing procedure employed during AF is controlled based on a maximum ventricular rate set relative to the underlying intrinsic ventricular rate so as to keep an overall ventricular rate below the maximum rate.

System and method for controlling ventricular pacing during AF based on underlying ventricular rates using an implantable medical device

Techniques are provided for use in a pacemaker or implantable cardioverter/defibrillator (ICD) for distinguishing cardiac ischemia from other conditions affecting the morphology of electrical cardiac signals sensed within a patient, such as hypoglycemia, hyperglycemia or other systemic conditions. In one example, the device detects changes in morphological features of cardiac signals indicative of possible cardiac ischemia within the patient, such as changes in ST segment elevation within an intracardiac electrogram (IEGM). The device determines whether the changes in the morphological features are the result of spatially localized changes within a portion of the heart and then distinguishes cardiac ischemia from other conditions affecting the morphology of electrical cardiac signals based on that determination. In another example, the device exploits the interval between the peak of a T-wave (Tmax) and the end of the T-wave (Tend). A significant increase in the Tend&#x2212;Tmax interval is indicative of ischemia rather than a systemic condition.

Systems and methods for distinguishing cardiac ischemia from systemic influences on IEGM morphology using an implantable medical device

Techniques are described for detecting ischemia, hypoglycemia or hyperglycemia based on intracardiac electrogram (IEGM) signals. Ischemia is detected based on a shortening of the interval between the QRS complex and the end of a T-wave (QTmax), alone or in combination with a change in ST segment elevation. Alternatively, ischemia is detected based on a change in ST segment elevation combined with minimal change in the interval between the QRS complex and the end of the T-wave (QTend). Hypoglycemia is detected based on a change in ST segment elevation along with a lengthening of either QTmax or QTend. Hyperglycemia is detected based on a change in ST segment elevation along with minimal change in QTmax and in QTend. By exploiting QTmax and QTend in combination with ST segment elevation, changes in ST segment elevation caused by hypo/hyperglycemia can be properly distinguished from changes caused by ischemia.

System and method for distinguishing among cardiac ischemia, hypoglycemia and hyperglycemia using an implantable medical device

An exemplary method includes acquiring patient activity information and/or nerve activity information, detecting one or more episodes of atrial fibrillation, associating the information with atrial fibrillation and, upon occurrence of particular patient activity and/or nerve activity, calling for delivery of an anti-atrial fibrillation therapy. Various other exemplary methods, devices, systems, etc., are also disclosed.

Neural sensing for atrial fibrillation

An implantable cardiac defibrillation device provides pre-shock stimuli to reduce the defibrillation threshold (DFT). The device includes an arrhythmia detector that detects fibrillation of a fibrillating chamber of a heart and a pulse generator that provides a fibrillation therapy output responsive to the arrhythmia detector detecting fibrillation of the fibrillating chamber of the heart. The therapy output includes a defibrillating shock having an output magnitude exceeding a temporary defibrillation threshold of the fibrillating chamber and at least one pre-defibrillating shock output pulse that reduces an initial defibrillation threshold of the fibrillating chamber to the temporary defibrillation threshold. An electrode system having at least two defibrillation electrodes delivers both the at least one pre-defibrillating shock output pulse to the heart and the defibrillating shock to the fibrillating chamber of the heart.

Implantable cardiac stimulation device providing reduced defibrillation thresholds and method

An implantable system applies tiered antitachycardia pacing (ATP) that may be combined with pre-pulsing therapy in order to reduce pain. In one implementation, an exemplary system applies a progression of increasingly potent pacing vectors, progressing in an initial tier from small electrodes inside the heart to later tiers that increasingly use a large electrode surface outside the heart. In the latter tiers, a pre-pulse may be added prior to each ATP pulse to reduce the sensation of pain.
