An implantable medical lead includes (i) a proximal end portion including a contact and having a proximal end; and (ii) a distal end portion including an electrode and having a distal end. The electrode is electrically coupled to the contact. The distal end portion is generally flat and sufficiently stiff to be pushed through subcutaneous tissue.

Self-tunneling lead

Leads having distal electrodes may be used in application of test stimulation for purposes of implanting a lead having a fixation element distal to an electrode array. The fixation element is proximal the distal electrode. Accordingly, the distal electrode may be advanced beyond a distal end of an introducer while the fixation element may be retained in a retracted configuration by the introducer. If the test signals applied by the distal electrode indicate that the distal electrode is in the desired location of the patient, a series of markings on the lead may be used to facilitate placement of the electrode array at the location previously occupied by the distal electrode; i.e. the desired location of the patient. The electrodes of the electrode array may then be used to provide therapy to the patient.

Introduction of medical lead into patient

Introducers for implanting a lead having a fixation element distal to an electrode include a window, electrode, or conductive member alignable with the electrode of the lead white maintaining the fixation element in a retracted configuration. The window, electrode or conductive member of the introducer provide a mechanism for applying test stimulation signals to determine whether the lead is properly positioned in a patient without deploying the fixation element.

This disclosure includes an anchor configured to maintain a portion of a therapy delivery element within a desired location of a patient. The anchor comprises a body forming a lumen configured to compressibly engage the outer surface of the therapy delivery element to hold the anchor in place about the therapy delivery element. The anchor may be stretched such that the lumen becomes larger than the cross-section of the therapy delivery element to facilitate positioning the anchor about the therapy delivery element. The body forms two or more channels that facilitate radial stretching of the anchor using a pronged tool. Radially stretching the body via the channels using the pronged tool reduces a holding force of the anchor on the therapy delivery element to facilitate adjusting a position of the anchor relative to the therapy delivery element.

A number of parameters related to the operation of a fluid delivery device are determined based on pressures within the device sensed using multiple pressure sensors. In one example, the volume of therapeutic fluid within a reservoir of a fluid delivery device is estimated based on a measured pressure differential. In another example, the rate at which a therapeutic fluid is added to or removed from the reservoir is estimated based on the measured pressure differential.

A system that includes an anchor assembly, the anchor assembly including: at least one anchoring structure configured to be anchored in a head of a patient; and at least one lead anchoring structure; and b. at least one lead, the at least one lead including a lead body extending from a distal end to a proximal end; at least one electrode located on or in the distal end of the lead body; and at least one lead anchor located on or in the lead body proximal to the electrode, wherein the at least one lead anchor of the lead and the lead anchoring structure are configured to cooperate to secure the at least one lead to the anchor assembly.

Anchor assembly for use in occipital nerve stimulation

An anchor for securing a therapy delivery element in a desired location within a living body is disclosed. The anchor includes an anchor body having a space, which may be a main channel, to receive the therapy delivery element. A fill port is provided in the anchor body to introduce an adhesive into the space when the therapy delivery element is positioned within the space. In one embodiment, the fill port is coupled to a fill port extension that transfers the adhesive from a dispenser located a distance from the fill port into the space. Multiple fill ports, any of which may be coupled to a respective fill port extension, may be provided to introduce the adhesive at various locations of the space. One or more of the fill ports may extend into multiple side channels to introduce the adhesive at various locations of the space.

The locus of electrically excitable tissue where action potentials are induced can be controlled using the physiological principle of electrotonus. In one embodiment, first and second pulses are applied to first and second electrodes, respectively, to generate first and second subthreshold potential areas, respectively, within the tissue. The locus within the tissue where action potentials are induced is determined by a superposition of the first and second subthreshold areas according to the physiological principle of electrotonus. In another embodiment, a two-dimensional array of electrodes are formed. The cathode may be positioned near the center of the two-dimensional array or may be left out. The first and second subthreshold areas may thereby be steered. An array of anodal rings may be used to contain the field of excitation.

Technique for adjusting the locus of excitation of electrically excitable tissue

Techniques capable of selectively affecting and adjusting a volume of neural tissue in the brain, parenchyma of the spinal cord, or a peripheral nerve are disclosed. A lumen having at least one opening at its distal end that is capable of directing a lead outwardly along a predetermined trajectory is preferred. The lumen is capable of accepting a plurality of leads that can project outward in different directions from the distal end of the lumen. The leads have one or more electrodes at its ends and are thereby configured by the lumen in accordance with a predetermined two- or three-dimensional geometry. Anode/cathode relationships may be established between the electrodes by the operator to stimulate the neural tissue surrounding these electrodes. The operator may also adjust the stimulation to selectively stimulate the desired portion of the brain, spinal cord, peripheral nerve. Sensor feedback may be implemented to adjust the treatment therapy.

A medical device known as an implantable neurostimulation system is configured for implanting in humans to deliver a therapeutic electrical stimulation to tissue to treat a variety of medical conditions such as pain, movement disorders, pelvic floor disorders, and many other conditions. The implantable neurostimulation has a housing, a power supply carried in the housing, stimulation electronics coupled to the battery and coupled to a neurostimulator connector block, a stimulation lead, and a lead extension. The lead extension is electrically coupleable between the neurostimulation connector block and the stimulation lead. The extension conductor is composed of an outer surface and an inner core. The outer surface has an outer impedance and the inner core has a core impedance that is substantially lower than the outer impedance. Many embodiments of the low impedance lead extension are possible.
