A system for selection of a medical provider (MP) by a requesting patient based on preferences for the MP includes a first model modeling the preferences of a plurality of patients for select MPs and trained on a first training data set. A second model is provided for modeling the preferences of a plurality of MPs for select patients and trained on a second training data set. A controller controls the first and second models to select at least a portion of available MPs in an available MP data set for input of the associated representative information to the first and second model MP inputs. A patient rank calculator accumulates the output patient determined preferences for MPs associated with the selected at least a portion of the available MPs and generates a MP ranking for each of the selected at least a portion of the available MPs. An MP rank calculator accumulates the output MP determined preferences for the requesting patient for each of the selected at least a portion of the available MPs and generates a patient ranking for each of the selected at least a portion of the available MPs. A plurality of weighting blocks are associated with at least one of the patient rank calculator and the MP rank calculator and with a determined preference of a rank calculator. A control input to each of the plurality of weighting blocks controls a weighting value applied by the plurality of weighting blocks to each of the associated determined preferences. A modifier modifies the patient ranking output of the patient rank calculator with the output of the MP rank calculator to provide as an output in response to the query from the requesting patient a resultant ranking list for each of the selected at least a portion of the available MPs.

The cardiac valve includes a self-expanding frame, vertical posts coupled to the self-expanding frame, and valve leaflets coupled to the self-expanding frame or to the vertical posts. Each of the vertical posts includes a connection to a corresponding control wire.

The MPs in the second training data set have rated at least one of the patients in the second data set by a predetermined set of MP determined preferences. An available MP data set is provided containing information representative of available MPs for selection thereof by the requesting patient. A controller controls the first and second models to select at least a portion of the available MPs in the available MP data set for input of the associated representative information to the first and second model MP inputs. A list generator generates a ranked list of available MPs from the output of the first model for the requesting patient. A combiner modifies the ranked list of available MPs by the output of the second model to output a modified ranked list.

Provided is a process for authentication of a user on a mobile device. The user of the mobile device may authenticate with the mobile device, and credentials may be conveyed to a server via a relying device. The mobile device may directly communicate credentials to the relying device. In some examples, the user of the mobile device may authenticate using the mobile device without inputting credentials on the relying device. Credentials conveyed to the server by the relying device and authenticated by the server may permit user access to the relying device or access to an online resource from the relying device.

An injection device is used for 'tattooing' markers inside the heart of a patient at specific locations. Placement within the heart of markers, such as points marked with a radiopaque dye, may reduce the use of x-ray dye and echocardiography, may allow a quicker cardiac procedure, and thus may improve the procedure outcome for the patient. Alternatively or additionally to cardiac marking with a radiopaque dye, cardiac tissue fibrosis, cardiac tissue contraction, and/or cardiac tissue stiffening (or hardening) can be promoted at specific locations by delivering a suitable substance.

Injection device and method

A microtube guide has a microtube combined with a free-floating and removable core. The microtube is generally hollow with a tube shaft and a distal ring, the tube shaft and the distal ring formed from flexible plastic. The distal ring is conformable to the core and straightenable for insertion into a patient's body, and deploys when the core is withdrawn to form a loop. The core is received by the microtube and is configured to advance into the distal ring to cause a diameter of the distal ring to expand, retract, or straighten. The core comprises a tapered segment that tapers in outer diameter from the diameter of the main core wire to a smaller diameter. A distal end segment of microtubing is frictionally affixed to the tapered segment.

Hybrid microcatheter guidewire

Microtube guide

A microcatheter device for crossing a patient's aortic valve has a guidetube combined with a free-floating and removable guidewire. The guidetube is generally hollow with a main shaft and a distal ring, the main shaft and the distal ring formed from flexible plastic. The distal ring is larger in diameter than the main shaft, and is configured to conform to an internal guidewire for insertion into the aorta, and then to deploy to form a downwardly-extending loop when in place in the left ventricle. A guidewire is received by the guidetube and is configured to advance into the distal ring to cause a diameter of the distal ring to expand, retract, or completely straighten. The distal ring diameter is thus adjustable to fit the size of the patient's left ventricle.

A catheter for positioning a valve during a transcatheter aortic valve replacement is formed from a resilient hollow body conformable to a guide wire when a guide wire is passed in through an upper opening in the hollow body and through the hollow body. When the guide wire is retracted, the catheter deploys to form a substantially straight upper shaft portion that extends downwardly from the upper opening and a distal ring perpendicular to the upper shaft portion. The distal ring approximates the size and shape of the patient's aortic valve annulus. A lower loop connects the upper shaft portion of the catheter to the distal ring. An outer surface of the distal ring is radiopaque, and the distal ring comprises openings for dispersing radio opaque medium used in imaging of the patient's aortic valve annulus. The deployed catheter is advanced until it snugly contacts the aortic valve with the horizontal loop at the level of the aortic valve leaflets and with the lower loop dipping into one the aortic valve cusps. The distal ring will be viewed as a straight line when an x-ray C-arm is properly aligned with the aortic valve.

Above-the-valve TAVR ventricular catheter

A cardiac valve cutter of the present disclosure has a flexible housing with a central lumen for receiving a guide wire. A high-speed cutting blade is disposed within the housing near an edge of the housing. A trough cut into a side of the housing exposes a portion of the cutting blade so that the cutting blade can cut valve tissue. Two pusher wires are disposed on an opposite side of the housing from the cutting blade. Troughs cut into the side of the housing form openings from which the pusher wires can extend. The pusher wires are staggered with respect to one another in the longitudinal direction. When the pusher wires are advanced, they extend outwardly from the troughs and can be used to stabilize the cutter at the cardiac valve and push the cutting blade against the valve on the opposite side.
