The invention relates to systems and methods to assist physicians in decision making for stroke patients. In particular the systems and methods can be used to assist physicians to decide on whether a patient with an acute ischemic stroke has a large vessel occlusion (LVO) and should be transferred from a community hospital to a larger hospital to undergo an endovascular thrombectomy procedure.

The embodiments presented herein relate to concepts designed to eliminate the gap between a catheter and guide wire that can otherwise contribute to a catheter getting stuck within the vasculature. In an example embodiment, a microcatheter with an enlarged distal section is described. The enlarged portion of the microcatheter may be located close to the inner diameter of the guide catheter in order to reduce any open space between the microcatheter and the guide catheter, and the guidewire can be placed through the microcatheter and used to guide the system. The microcatheter can include one or more marker bands to aid in aligning the microcatheter correctly relative to the guide catheter. After the guide catheter and microcatheter are tracked to the appropriate treatment site, the microcatheter can then be used to deploy various medical devices to treat a patient.

Intravascular treatment site access

An automated system and method for assisting in decision making for the treatment of stroke patients is provided, and specifically for assisting a physician whether the patient should be administered a drug or transferred to another hospital to undergo an endovascular thrombectomy procedure. A variety of factors are input into the system with limited human intervention and a tool automatically determines the probability of whether the patient will have a better outcome if transferred or not. The factors include clinical factors, imaging factors and time to transfer factors. The tool includes processes for automatically determining several imaging factors, including the determination of clot length, collateral blood flow, the presence of forward blood flow within and around the clot, and the clot permeability. The tool has capability to continuously update the treatment protocol and other output results using current clinical, health system or other relevant information or feedback.

Decision support tool for stroke patients

The invention describes catheter systems and methods for accessing the brain during endovascular/neurointervention procedures in the treatment of ischemic stroke. More specifically, a multi-axial catheter system and kit is described that improves the process of accessing a carotid artery particularly in patients having a tortuous aortic arch and thereafter improves the process of gaining access to the brain to enable the insertion of aspiration and/or the deployment clot retrieval devices. Methods and apparatus for retrieving a blood clot are disclosed. The apparatus comprises an expandable wire frame for deployment using a push wire within a microcatheter. When deployed in the vicinity of a blood clot, the wire frame is configured to expand and trap the blood clot allowing the blood clot to be removed. The expandable wire frame is connected to the push wire at distal and proximal ends. One or more expansion stops define a minimum distance between the distal and proximal ends of the expandable wire frame and a point of maximum extension of the expandable wire frame. This may help the user to control the configuration of the apparatus as it is being used within the vascular system.
